  A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

  Observation. Licensing reviewed 6 children's files and observed the following: - 3 children's DOB not on file - 4 children's MSP # not on file - 3 children's immunization status not on file The manager said she will make sure that the information is obtained and updated in their files.

  Corrective action. Please ensure that all files are up to date and complete.

  Observation. Licensing reviewed 6 children's files and observed the following: - 4 children are missing the name and telephone number of a medical practitioner - 4 children are missing the name and telephone number for an emergency contact The manager said she will make sure that the information is obtained and updated in their files.

  Corrective action. Please ensure that all files are up to date and complete.

  A licensee must keep current records of each of the following: (b) for each employee, the records required under section 19 (1) [character and skill requirements].

  Observation. One casual/floater staff was working today with a regular staff. They are not included in the staff document checklist that is at the facility. That casual staff could not access their documents on the computer either.

  Corrective action. Any casual or floater staff that work at the facility is to be included on the staff document checklist to show that they have current records on file.

  Observation. One staff did not have evidence of training certificates to review.

  Corrective action. Please ensure all staff files are complete.
