    Observation. School age and GCC (30 mos to SA) enter and exit from the same facility door. During pick up and drop off the two groups are mixing for approximately 1 to 2 minutes.

    Corrective action. Ensure groups are divided as per Schedule E.

    Observation. - A child was not signed out from the GCC (30 mos to SA) program when they left the facility with a staff to pickup a school age child. - Sign in/out records are not taken with staff when picking up/dropping off children. Therefore, attendance is not accurately recorded when transfer of care has occured.

    Corrective action. Ensure arrival and departures are accurately recorded.

    Observation. Licensing observed the following: - School age staff only takes a few band aids when providing care off the Community Care Facility Premise. - The first aid kit in the GCC (30 mos to SA) classroom has insufficient supplies.

Licensing observed the following: - 1 bathroom used by the children had no toilet paper (Corrected during inspection) - Dried urine stains on a toilet seat (Corrected during inspection) - Toilet paper rolls placed on the counter or the back of the toilet seat and not placed back onto the dispenser.

    Corrective action. Ensure toilet paper placed on a toilet paper dispenser is available for the children. Ensure the toilet is cleaned and disinfected should bodily fluids be found on the seat.

    Observation. DLSOP- Active Play - The school age program does not ensure a minimum of 60 minutes per day of outdoor active play.

    Corrective action. Ensure all programs meet the active play requirement of 60 minutes.

    A licensee, other than a licensee who provides a care program described as either Preschool (30 Months to School Age) or Group Child Care (School Age), must have in a community care facility a sturdy surface for changing diapers, located outside the food preparation area, next to (a) a covered container for soiled clothing.

    Observation. LO observed and discussed with Manager, re: garbage bins in both washrooms All garbage lids from the washrooms were removed/broken. LO discussed re: sanitizing frequently touched surfaces such as garbage lids daily. There must be a bin with lid to avoid children from accessing/ touching any soiled diapers, used gloves, wipes and used paper towels.

    Corrective action. Ensure that covered container is available within the washroom for soiled diapers/pull ups.

    Observation. Group CC (30 months - SA) - Maximum group size of 25 was exceeded. LO observed and reviewed the facility's attendance record and confirmed that there were 27 children at one given time (on Aug 4). Additionally, 2 different service types (30 months- SA) and (School Age) were combined in one single group.

    Corrective action. Ensure that children are divided into appropriate groups as per Sch. E of CCLR.

    A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

    Observation. Licensing observed the following: - 2, 4ft and 5ft shelves used as cubbies, not secured to the wall. Licensing observed 2 children climbing these shelves to try and grab things. This poses a serious hazard for the children as the shelves could fall on them. - 2 large storage bins, 1 car seat and several other objects stored on top of the unsecured shelves

    Corrective action. Ensure a healthy and safe environment is provided at all times.

    A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

    Observation. Licensing observed the following: - A shelf used for storing toys has a corner piece that is broken off. The wood is now exposed making the surface porrous and unable to clean and sanitize. The same shelf has a large crack on the side. - Several chips on the wall exposing the drywall - Teacher table is broken. Licensing observed a child leaning against the table and the leg of the table was starting to push inwards.

    Corrective action. Ensure the furniture, equipment and fixtures are clean and in good repair.

    Observation. Licensing observed the following: - From 8:30am-9am, 14 children in care with an Early Childhood Educator and a responsible adult. The requirement is to have an Early Childhood Educator and Early Childhood Educator Assistant at minimum for this service type and number of children. - 9 children went for a walk with an Early Childhood Educator and support worker who was not in ratio.

    Corrective action. Ensure the ratio of employees to children attending a facility is no less than that permitted in Schedule E.

Licensing noted the following: - No indication on a few days whether the children were absent or present - Children not signed out of the program - 9 children left the facility with a staff to go for a walk. There was no indication as to which children had left the facility. When Licensing asked the Manager the names of the children that left, the Manager was only able to name 5 of the 9.

    Corrective action. Ensure there is an accurate record showing the daily attendance, indicating for each day whether a child is absent or present and time of arrival and departure.

    A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

    Observation. During the inspection of the physical facility licensing observed that one of the walls inside the facility had chipped paint and dry wall was exposed.

    Corrective action. Please ensure that the facility is in good repair.
