  Observation. School age and GCC (30 mos to SA) enter and exit from the same facility door. During pick up and drop off the two groups are mixing for approximately 1 to 2 minutes.

  Corrective action. Ensure groups are divided as per Schedule E.

  A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

  Observation. - A child was not signed out from the GCC (30 mos to SA) program when they left the facility with a staff to pickup a school age child. - Sign in/out records are not taken with staff when picking up/dropping off children. Therefore, attendance is not accurately recorded when transfer of care has occured.

  Corrective action. Ensure arrival and departures are accurately recorded.

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

  Observation. A random selection of staff files were reviewed. Out of the 5 reviewed, 2 staff do not have site specific criminal record checks.

  Corrective action. Ensure staff files are complete.

  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. DLSOP Safe Play Space - A new climber has been installed. There is insufficient loose fill material for the height of the equipment.

  Corrective action. Provide atleast 6 inches of loose fill material for the newly installed 5 foot climber.

  A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

  Observation. Licensing observed the following: - Emergency cards and attendance records were not taken during a neighbourhood walk The Manager said staff remember which children are in attendance on a daily basis.

  Corrective action. Ensure there is a plan to respond to and recover from an emergency.

• RECORDS AND REPORTING

  A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee.

  Observation. In review of the minor incident log book there were two incidents on Aug 21 and September 16 that a child was involved in or may have been involved in a reportable incident as described in Schedule H as required. This is noted as a repeat contravention.

  Corrective action. Submit a corrective action plan.

  A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

  Observation. A random selection of children's files were reviewed. Out of the 6 reviewed: - 1 file did not contain information for a care card - 2 files did not contain information for a medical practioner. This is noted as a repeat contravention.

  Corrective action. Ensure children's files are complete.

  Observation. Licensing observed the following: - There is no indication whether children are absent or present on a daily basis - Children being moved between the 2 programs are not accurately recorded in each classroom

  Corrective action. Ensure there is an accurate record indicating arrival/departure and absent/present.

  A licensee must have first aid kits that are readily accessible to all employees, including while care is provided off the community care facility premises.

  Observation. Licensing observed the following: - School age staff only takes a few band aids when providing care off the Community Care Facility Premise. - The first aid kit in the GCC (30 mos to SA) classroom has insufficient supplies.

  Corrective action. Ensure there is a system to maintain emergency supplies.

  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. Licensing observed the following: - Parking lot and sidewalk used by parents, children and staff was not shoveled or salted. There was a lot of snow and ice posing as a slipping hazard for all. - Change pad is being stored above a piano outside the bathroom area

  Corrective action. Ensure there is a system to maintain a healthy and safe environment.

  A licensee must immediately notify a parent or emergency contact if, while under the care or supervision of the licensee, the child (b) is involved in, or may have been involved in, a reportable incident described in Schedule H.

  Observation. In review of the Minor Incident Log Book it appears a disease outbreak occured in October 2024. The outbreak was not reported to Licensing.

  Corrective action. Ensure reportable incidents as per Schedule H are reported to Licensing within 24 hours.

  Corrective action. Ensure staff files are complete.

  Observation. Licensing observed the sponge play mats in the 3-5 room were not clean.

  Corrective action. Ensure the sponge mats are cleaned.

• HYGIENE AND COMMUNICABLE DISEASE

  A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.

  Observation. Licensing observed the following: - 1 bathroom used by the children had no toilet paper (Corrected during inspection) - Dried urine stains on a toilet seat (Corrected during inspection) - Toilet paper rolls placed on the counter or the back of the toilet seat and not placed back onto the dispenser.

  Corrective action. Ensure toilet paper placed on a toilet paper dispenser is available for the children. Ensure the toilet is cleaned and disinfected should bodily fluids be found on the seat.

  During an investigation or inspection, a person must not do either of the following: (b) withhold, conceal or destroy any records or information relevant to the investigation or inspection or required by the medical health officer.

  Observation. A staff witheld information during the inspection (i.e. attendance records)

  Corrective action. Ensure Licensing is provided all information during inspections.

  Corrective action. Ensure that children are divided into appropriate groups as per Schedule E of the CCLR.

  Observation. Licensing observed emergency supplies are expired and insufficient to implement the emergency plan.

  Corrective action. Ensure all food and water used to respond to and recover from an emergency is not expired.

  A licensee must provide to children a comprehensive and coordinated program of indoor and outdoor activities that (b) is appropriate for the age and development of children in each group in the community care facility.

  Observation. Licensing observed insufficient programming and equipment available for the group size in care. This is noted as a repeat contravention.

  Corrective action. Ensure the facility provides a program that is appropriate for the age and development of children in care.

  18020 - CCLR s.44(1)(b)
• PROGRAM

  A licensee, other than a licensee providing a care program described as Occasional Child Care, must provide each child with daily outdoor play periods unless weather conditions would make it unreasonable to do so. (4) Despite subsection (1), a licensee who provides a care program described as Occasional Child Care need not provide a program of outdoor activities. (5) Despite subsection (3), that subsection applies to a licensee who provides a care program described as Child-minding only if the program provides care for 3.5 hours or more each day.

  Observation. DLSOP- Active Play - The school age program does not ensure a minimum of 60 minutes per day of outdoor active play.

  Corrective action. Ensure all programs meet the active play requirement of 60 minutes.

  18060 - CCLR s.44(3)(4)(5)

  A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

  Observation. Licensing observed on August 6th children were not signed out on the attendance list.

  Corrective action. Ensure on a daily basis the attendance record indicates the time of arrival and departure.

  Corrective action. Ensure immunization status has been obtained for all staff.

  Observation. A selection of staff records were reviewed. Out of the 7 reviewed: - Character references were not verified for 2 staff.

  Corrective action. Ensure character references have been verified for all staff.

  Observation. A selection of staff records were reviewed. Out of the 7 reviewed: - 1 staff file did not have a criminal record check from the Criminal Record Review program. A police check was available for review.

  Corrective action. Ensure staff have obtained a current and valid criminal record check through the Criminal Record Review program.

  A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. (Educator or assistant certificate validation)

  Observation. A selection of staff records were reviewed. Out of the 7 reviewed: - 1 staff did not have their certificate validated

  Corrective action. Ensure staff who hold an ECE certificate have their certificate validated through the ECE Registry.

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that all indoor and outdoor play materials and equipment accessible to children are (a) suitable for the age and development of the children.

  Observation. LO observed that only one toy (mega blocks) were offered to the whole group of children to play with. LO had a discussion with the Manager, re: Schedule G

  Corrective action. Ensure that all play materials offered to children are age and developmentally appropriate.

• HYGIENE AND COMMUNICABLE DISEASE

  A licensee, other than a licensee who provides a care program described as either Preschool (30 Months to School Age) or Group Child Care (School Age), must have in a community care facility a sturdy surface for changing diapers, located outside the food preparation area, next to (a) a covered container for soiled clothing.

  Observation. LO observed and discussed with Manager, re: garbage bins in both washrooms All garbage lids from the washrooms were removed/broken. LO discussed re: sanitizing frequently touched surfaces such as garbage lids daily. There must be a bin with lid to avoid children from accessing/ touching any soiled diapers, used gloves, wipes and used paper towels.

  Corrective action. Ensure that covered container is available within the washroom for soiled diapers/pull ups.

  15010 - CCLR s.14(4)(a)
• MEDICATION

  If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (b) readily accessible to employees.

  Observation. Medication was not readily accessible to employees.

  Corrective action. Ensure that medication for the child requiring extra support is readily accessible to the staff, but inaccessible to the children.

  Without limiting subsection (1), a licensee must ensure that (a) children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E.

  Observation. Group CC (30 months - SA) - Maximum group size of 25 was exceeded. LO observed and reviewed the facility's attendance record and confirmed that there were 27 children at one given time (on Aug 4). Additionally, 2 different service types (30 months- SA) and (School Age) were combined in one single group.

  Corrective action. Ensure that children are divided into appropriate groups as per Sch. E of CCLR.

  Observation. No fire drill record for July 2023; the Manager stated that it was practiced but not recorded.

  Corrective action. Ensure that monthly fire drills and yearly emergency drill is recorded accordingly.

  Observation. Licensing observed the following: - 2, 4ft and 5ft shelves used as cubbies, not secured to the wall. Licensing observed 2 children climbing these shelves to try and grab things. This poses a serious hazard for the children as the shelves could fall on them. - 2 large storage bins, 1 car seat and several other objects stored on top of the unsecured shelves

  Corrective action. Ensure a healthy and safe environment is provided at all times.

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

  Observation. Licensing observed the following: - A shelf used for storing toys has a corner piece that is broken off. The wood is now exposed making the surface porrous and unable to clean and sanitize. The same shelf has a large crack on the side. - Several chips on the wall exposing the drywall - Teacher table is broken. Licensing observed a child leaning against the table and the leg of the table was starting to push inwards.

  Corrective action. Ensure the furniture, equipment and fixtures are clean and in good repair.

  A licensee must display in a prominent place in the community care facility the certificate, or a copy of the certificate, of any educator or assistant employed to work in the community care facility.

  Observation. Licensing noted 1 staff certificate was not posted.

  Corrective action. Ensure certificates of any educator or assistant is posted in a prominent place in the facility.

  Observation. Licensing observed the following: - From 8:30am-9am, 14 children in care with an Early Childhood Educator and a responsible adult. The requirement is to have an Early Childhood Educator and Early Childhood Educator Assistant at minimum for this service type and number of children. - 9 children went for a walk with an Early Childhood Educator and support worker who was not in ratio.

  Corrective action. Ensure the ratio of employees to children attending a facility is no less than that permitted in Schedule E.

  Observation. Licensing noted no record of an emergency drill being completed within the last year.

  Observation. Licensing noted the following: - No indication on a few days whether the children were absent or present - Children not signed out of the program - 9 children left the facility with a staff to go for a walk. There was no indication as to which children had left the facility. When Licensing asked the Manager the names of the children that left, the Manager was only able to name 5 of the 9.

  Corrective action. Ensure there is an accurate record showing the daily attendance, indicating for each day whether a child is absent or present and time of arrival and departure.

  A licensee must ensure that children have at all times immediate access to an employee who (a) holds a valid first aid and CPR certificate, provided on completion of a course that meets the requirements of Schedule C.

  Observation. Licensing noted the following: - 1 staff and a support worker went for a walk with 9 children. The staff did not take a first aid kit or children's emergency contact cards with them. - A selection of staff files were reviewed, out of the 8 reviewed, 3 staff have first aid certificates that do not meet the requirements of Schedule C

  Corrective action. Ensure children have at all times immediate access to an staff that can administer First Air and CPR.

  Observation. A selection of staff files were reviewed. Out of the 8 reviewed, 5 files did not contain immunization status.

  Corrective action. Ensure there is evidence staff have complied with the Province's immunization and tuberculosis control programs.

  A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

  Observation. During the inspection of the physical facility licensing observed that one of the walls inside the facility had chipped paint and dry wall was exposed.

  Corrective action. Please ensure that the facility is in good repair.
