  Observation. One child's record is missing written consent from a parent to call a medical practitioner or ambulance. In the event of an emergency where a parent or emergency contact cannot be reached, written consent is required for the facility to be able to call a medical practitioner or an ambulance. Submit by November 4, 2025, evidence that written consent has been obtained. One child record is missing a photo or digital image. A photo of a child is required as it can be used to readily identify the child in an emergency. Submit by November 4, 2025, evidence that the child's photo has been taken.
