    Observation. An incident was recorded on file that outlined the small dog at the facility had bitten one child while in care. Information was not presented by the Licensee on how action would be taken to ensure the children's safety after the incident. As a result of the documented incident the dog has demonstrated unpredictability and is a risk to children in care. It is the responsibility of the Licensee to provide a healthy and safe environment at all times. A response is required by April 7, 2026 that outlines how the risk has been mitigated.

  • C1.2 - Does the licensee operate the facility in a manner that will promote the health, safety and dignity of persons in care?Non-critical

    Observation. The facility tested high for Radon gas and mitigation was conducted for the facility. A second test is required to be completed to test the levels of Radon gas in the facility after the mitigation was completed. Exposure to high levels of Radon gas is harmful to human health and is the number one cause of lung cancer in non-smokers. Submit the results of the follow-up Radon test to Licensing by March 1, 2027.

    Observation. The climbing structures in the outdoor play area do not have a six foot clearance around the equipment. A six foot clearance around equipment is required to mitigate the risk of injury in children.
