Observation. Two staff members used inappropriate behavioral guidance with several children under 36 months of age by pulling, pushing and lifting them in an inappropriate manner.
Corrective action. Ensure there is a system to monitor and maintain appropriate behavioural guidance strategies.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following: - Dirty mirrors in the children's bathroom - Walls and high touch areas in all classrooms covered in dirty finger prints - Baby gate securing off the kitchen area is broken
Corrective action. Ensure a clean environment for the children.
11020 - CCLR s.13(2) - HYGIENE AND COMMUNICABLE DISEASE
A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.
Observation. Licensing observed the following: - No toilet paper available in some of the bathrooms - One classroom did not have any papertowel available for children to dry their hands. Another classroom did not have papertowels accessible to the children.
Corrective action. Ensure bathroom supplies are replenished.
15030 - CCLR s.46(1) - OR SUPERVISION
Without limiting subsection (1), a licensee must ensure that (b) the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E.
Observation. Licensing observed the following: - 10 children in the main GCC (30 mos to school age) with 2 ECEA's. The requirement is to have 1 ECE and 1 ECEA - 6 children in the GCC (under 36 mos) with 1 ECE and 1 ECEA. The requirement is to have an I/T Educator and ECE.
Corrective action. Ensure each classroom is staffed as per Schedule E.

Observation. Licensing observed children being moved between classrooms throughout the duration of the inspection. There was no system to indicate how many children were moving and the time when children were moving to another class.
Corrective action. Ensure there is a system that accurately reflects when children are moving to another classroom.

Observation. LO observed ( audit of records) for today. There were total of 9 children grouped in one single group with more than 3 children younger than 36 months old present for the first 30 minutes of the day.
Corrective action. Ensure that appropriate group size is maintained at all times.

Observation. Licensing noted when children move between programs they are not signed out of one program and into another program.
Corrective action. Ensure there is a record of children's arrival and departures within each program.

A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (b) the courses of action recommended by health care professionals to address the needs of the child requiring extra support.
Observation. In review of the children's records, Licensing noted a child requiring extra support. While there is some information in a careplan, it does not outline the courses of action in regards to the child's diagnoses.
Corrective action. Ensure there is a care plan in place that is developed in consultation with the parent that includes diagnoses and actions recommended.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (d) copies of any diplomas, certificates or other evidence of the person's training and skills.
Observation. A random selection of staff files were reviewed. Out of the 4 reviewed, 1 file did not contain copies of any diplomas, certificates or other evidence of the person's training and skills.

Observation. A random selection of staff files were reviewed. Out of the 4 reviewed, an immunization status was not obtained for one employee.

Observation. Licensing observed a support worker who is newly employed. In review of staff records and a conversation with the support staff, they do not have any training relevant to the care that is being provided to the child who requires extra support.
Corrective action. Ensure staff who care for a child with extra support have the training and experience and demonstrates the skills necessary to care for that child.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following: - The fridge in the infant room (up stairs) and the 3-5 room did not have a thermometer recording the temperature in the fridge. - An electric kettle, half full, which was hot to the touch, was located near the edge of the countertop. This is a serious burn hazard.This is an area where children line up to go to the bathroom. A conversation with the manager indicated that the staff are using hot water for drinking. - Children's bedding and blankets were stored in a manner that caused cross contamination. This is a repeat contravention. - The Bi-fold closet doors in the nap room were not secured, posing a pinching hazard. - A large metal box with sharp corners and rusted edges was found in the 3-5 playground. A conversation with the manager indicated that they did not know what the purpose of the box is and that it belongs to the church. - Two garbage containers were located in a corner on the 3-5 playground. A conversation with the manager indicated that the garbage container is used by the church to collect dry leaves and other debris. - Three blue recycling containers were piled in a corner on the 3-5 playground. A conversation with the manager indicated that the recycling bins are used by the church and stored in the corner of the 3-5 playground. - A fan cord was within reach of children in the Infant room posing an electrocution hazard. - A phone charging cord was within reach of children in the 3-5 room. This was corrected during the inspection. - A vent located in the infant room had a gap in it posing an entrapment hazard. This was corrected during the inspection.
Corrective action. Please ensure you are providing a healthy and safe environment at all times.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Licensing observed the following: - A cupboard located in the kitchen are of the infant room was not locked. The cupboard contained a large jug of dish detergent, one bottle of cleaning supply, one bottle of hand sanitizer and some plastic bags. This is a repeat contravention.
Corrective action. Please ensure that children do not have access to any hazardous substances.
11200 - CCLR s.17 - NUTRITION AND FOOD SERVICES
A licensee must ensure that children are not (a) fed by means of a propped bottle.
Observation. At the time of the inspection of the infant room a child was placed in a rocker on their back and was drinking a milk bottle unsupervised by other staff. When asked why the child is drinking the milk in the rocker the manager mentioned that it was requested by the child's parents that the child's neck is not strong enough to till the bottle back and drink it.
Corrective action. Please ensure that any children who drink bottled milk are supervised at all times and supported in feeding time.

Observation. Licensing observed the following in the outdoor play space: - A tricycle that had a lot of rust all over the handle bars and part of the seat Licensing observed the following in indoor play space: - In conversation with the manager, she stated that staff and children are mantaining the same handwashing routines prior to covid. As per the new guidelines for Covid-19 handwashing for staff and children should be increased. - The manager stated the toys are being disinfected every 2-3 days. As per the new guidelines for Covid-19 high touch areas including toys need to be disinfected two times per day.
Corrective action. Please ensure a healthy and safe environment is provided at all times.

Observation (CORRECTED DURING INSPECTION): Licensing observed several cleaning products stored in a children's bathroom that was unsecured. Cleaning products were also stored underneath an unsecured bathroom sink in the infant/toddler room

11200 - CCLR s.17 - EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following inside the facility: - Bleach solution bottles are not labeled. A staff member stated the solution is made every 2-3 days. Bleach solution is no longer as concentrated and strong to disinfect surfaces and needs to be replaced every 24 hours. - Childrens bibs are hung to dry really close to each other and the bibs are touching posing a risk for bacteria and germs to be spread easier. - The fridge temperature was 5 degrees celcius. Licensing observed the following in the outdoor play area: - Little tikes slides and climbers in the outdoor play area have insufficient clearance around the equipment. (CORRECTED DURING INSPECTION) - There is a large ladder stored in the 3-5 outdoor play area posing as a risk to the children - There is a wire running across the outdoor play area and connected along the climber posing as a risk of electrocution for the children. The same wire is dangling off on one end posing as a risk for strangulation. (NEEDS IMMEDIATE ATTENTION) - There are large rubber road ramps stored in the 3-5 outdoor play area - There is a hose stored in the 3-5 outdoor play area posing a tripping hazard - There are large wooden signs stored in the outdoor play area
Corrective action. Please ensure a healthy and safe environment is provided at all times.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed 2 tricycles in the 3-5 outdoor play area. 1 tricycle has several 'chewing' marks probably from an animal all over the tricyle. The other tricycle has rust along the handle bars.
Corrective action. Please ensure furniture, equipment and fixtures are clean and in good repair.

A licensee must supply equipment, furniture and supplies that are (b) located so as not to block or hamper an exit in the case of fire or other emergency. Observation (CORRECTED DURING INSPECTION): Licensing observed children's ride on toys stored in front of an emergency exit.
Corrective action. This is noted to be a repeat contravention.the case of fire or other emergency.

Observation. Licensing observed a facility manager's name posted on the wall. This facility manager is no longer the manager of the facility.
