    Observation. The complaint alleged: Children were being placed in strollers for sleep and rest time. Follow up to this allegation included interviews with staff members, review of facility care policies, and inspection of the sleep space. Two staff members confirmed some children had been assigned a stroller for sleep and rest, rather than a cot or crib. As a result of the follow up investigation, and information provided by persons interviewed; it has been determined that the allegation is substantiated. Equipment used for sleeping must be appropriate for the size, age and development of the child intended to use it. Submit by May 06, 2026, written confirmation that children will have a cot or crib for sleeping and resting.

Ensure that all employee records contain verification of completion of a criminal record check through the Criminal Records Review Program. Provide a response to Licensing with evidence of compliance by November 10, 2025.

    Observation. One child record did not contain contact information for a medical practitioner. A name and telephone number of a medical practitioner is required by legislation. Ensure child records include identifying personal information outlined in Section 57 of the Child Care Licensing Regulation. Provide a response with evidence of compliance by November 10, 2025. There is no current record of play space inspections. Regular play space inspections are required as part of maintaining a healthy and safe environment, and to reduce the risk of injury to children. Ensure there is a record of play space inspections maintained at the facility. Provide a response to Licensing with evidence of compliance by November 10, 2025.

    Observation. The complaint alleges that the facility has not conducted a fire drill for approximately 4 months, and that facility records falsely indicate that monthly fire drills have occurred. The Licensing Officer conducted a site visit. The record posted at the facility indicates that the last fire drill was conducted in October of 2024. During the investigation, the Licensing Officer was informed that the last month a fire drill had actually occurred was June 2024. - Substantiated The Licensing Officer was informed that the facility is in the process of reviewing their system to ensure that routine monthly fire drills are occurring, and that facility records are maintained factually in accordance with the requirements set out in Legislation. Implementing the fire drill system monthly is required to ensure that staff and children are prepared to respond in the case of an emergency. Failure to conduct monthly fire drill practice puts children at risk. Ensure that records are current and reflect accurate practices of the facility.

    Observation. The complaint alleges that a child was left alone and unsupervised on the school bus. The Licensee confirmed that this incident did occur in September 2023. - Substantiated

During the investigation, Licensing was informed that a child was involved in an incident in September 2023 which is reportable as per Schedule H of the Child Care Licensing Regulation. This incident was not reported to Licensing. Through a separate investigation, Licensing identified a second reportable incident which was not reported to Licensing in December 2023.

    Observation. Licensing was not informed that the Group Child Care - School Age program has not been operating for the past 2 years.

    Observation. One staff file was missing reference checks.

  • C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical

    Observation. - A heavy box was sitting on top of the slanted roof of a storage container in the outdoor program space for the preschool program. Licensing noted this as having potential risk for injury to children if the box slid off the roof. (*This was corrected during the inspection.) - A spring-loaded bar mousetrap was set in an unlocked cupboard, and was accessible to children. - A low cabinet in the Infant Toddler space is broken and has been wrapped in packing tape to secure the doors. The cabinet is located in the main play area of the program, and is accessible to children. Debris is collecting on the edges of the tape and tape is peeling. This has potential to pose a choking risk to children if they were to pick /peel pieces of the tape. Additionally, the cabinet cannot be properly cleaned. - A large plastic treehouse play structure in the Infant Toddler room has a small sharp point of metal exposed due to a missing piece from the equipment. Clear packing tape has been used to cover the sharp point. The sharp metal has poked through the tape and is fully exposed. The sharp metal poses a potential risk for injury to children. Additionally, the exposed packing tape poses a potential choking risk to young children who may pick at or peel. - Outdoor sandbox is uncovered. This poses a potential risk to health due to potential contact with animal excrement.

    Observation. - Some child files were missing information including: Immunization status, sex, allergy and medical concerns. - Some child files were missing care plans for children requiring extra support. - Minor incident logs in the preschool room were only available from January 1, 2022. Staff indicated that records prior to that date had been shredded.
