• C1.1 - Does the licensee continually inform licensing of any significant changes to the structure or operation of the facility?Critical

  Observation. The Licensee did not inform Licensing that the large outdoor play space has not been usable since November 2024. The Licensee must inform Licensing of any changes to Schedule B. Corrected during the inspection.

• C4.1 - Are facility records current and complete?Non-critical

  Observation. Staff did not record the amount of medication given. The Licensee must record the amount of medication administered. Submit by May 14, 2025, confirmation and evidence that when medication is given to a child, staff record the amount administered. The digital image was outdated for several children and no longer bared a good likeness of the child. The Licensee is required to keep a current digital image of each child that can be used to identify the child in an emergency. Submit by May 14, 2025, written confirmation that digital images have been reviewed and updated. A child with a life-threatening allergy had an expired epi-pen. Expired medication may pose a risk to the child. Submit by May 14, 2025, written confirmation and evidence that the care plan has been reviewed and the child has access to their required medication.

  Observation. A random selection of child registration records were reviewed, one child was missing their immunization status. Attendance records were reviewed, some were found to have not recorded whether a child attended or was absent, and several children were missing their departure times.

• C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

  Observation. A random selection of employee records were reviewed, some were found to be missing required information such as, immunization status and character reference checks.

• C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical

  Observation. Children had access to items that were noted as "keep out of reach of children".

• C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

  Observation. Employee records were reviewed, some required information was missing. There was no record that all employees have practiced the fire drill system each month.
