Observation. - The facility minor incident log lacks essential details, including the time of incidents and immediate notification to parents. Detailed documentation and timely parental notification supports appropriate follow-up, monitoring, parental involvement in care decisions and up to date records. Submit a response with evidence of compliance by March 19, 2026. - Further review of minor incidents log identified that a child was involved in a reportable incident in October 2025. This incident was not reported to Licensing as required. It is important that all reportable incidents are submitted within 24 hours to allow Licensing to assess risk and provide guidance as necessary. Submit a response with evidence of compliance by March 19, 2026.

Observation. There was no covered container observed, next to diaper change area and wash basin, for soiled items. The Licensee indicated that the soiled diapers are discarded in a covered garbage bin under the kitchen sink. Ensure soiled diapers are discarded in a covered container that is located next to the diaper change area and a washbasin, outside the food preparation area.

Observation. The Licensing Officer observed a prescribed epi-pen in a child's backpack that was accessible to all children in care. Ensure children do not have access to any object or substance that may be hazardous their health and safety.

Observation. C4.1J - Reg.57 - Upon reviewing children's records, it was observed that there were no current photos of children that can be used to readily identify the child in an emergency. Ensure children's records are complete by the required follow-up date. C4.1T - SOP - At the time of inspection, there was no evidence of October's play space inspections log being completed. September's play inspections log was also observed to be partially completed. Ensure a system is developed and provided to licensing how play space inspections will be completed, logged, and retained.
