Observation. A Service Delivery Problem that occurred in May 2026 was not reported to Licensing . All reportable incidents, as identified in Schedule H, must be reported within 24 hours. The incident must be submitted immediately through the online reporting portal. In addition, provide written confirmation by June 11, 2026, that all future reportable incidents will be reported in accordance with regulatory requirements. The name and contact details of an emergency contact is missing for one child. Ensure that all required emergency contact information is obtained and documented in the child’s records. Submit written confirmation by June 11, 2026, that the missing information has been collected and added to the child’s file. Permission to contact a medical practitioner or ambulance in an emergency is missing for one child. Written consent must be provided for employees to call a medical practitioner or ambulance in the case of an accident or illness if the parent cannot be immediately reached. Observation. The complaint alleged: The Licensee had not informed Licensing that they were operating outside of the approved hours of 7.00am-5.00pm. Follow up to this allegation included surveillance, interviews with staff members, parents and the Licensee. The Licensee and a staff member confirmed care was being offered to an enrolled child prior to the operating hours of 7.00am. As a result of the follow up investigation, and information provided by persons interviewed; it has been determined that the allegation is substantiated. The Licensee must notify Licensing of any changes to the information provided in the application process. Submit by February 05, 2026, written confirmation of any changes made to the hours of operation.

Observation. The complaint alleged: • There was not a full time Infant/Toddler certified educator working during the months of October-December 2025. • An Early Childhood Educator Assistant was being left on their own with children. Follow up to this allegation included reviewing staff attendance records, pay records, staff schedules provided by the Licensee and the complainant, interviews with staff members, parents, and the Licensee. Staff schedules and sign in and out records were inconsistent; what was presented differed from other information provided and gathered during the investigation. Staff and parent interviews confirmed the lack of qualified employees. As a result of the follow up investigation, and information gathered; it has been determined that the allegation is substantiated. The Licensee must ensure children are adequately supervised by qualified employees. Submit by February 02, 2026, written confirmation of how employee to child ratios are being met. The complaint alleged: Children were left unsupervised in the nap room for extended periods of time. Follow up to this allegation included interviews with staff members, and the Licensee. As a result of the follow up investigation, and information gathered; it has been determined that the allegation is unable to be substantiated.

Observation. The complaint alleged: A person over the age of 12 years who is regularly at the premise during the hours the daycare was operating, does not hold a criminal record check issued through the criminal record review program. Follow up to this allegation included surveillance, interviews with staff members, parents and the Licensee. The person was observed at the premise while care was being provided, and staff and parents confirmed the presence of the person at the premise during operating hours. As a result of the follow up investigation, and information provided by persons interviewed; it has been determined that the allegation is substantiated. The Licensee must not permit a person aged 12 years or older to be present on the premise of a community care facility while children are being provided with care, unless they hold a valid criminal record check. Submit by February 03, 2026, written confirmation that any person who is regularly present at the facility holds a valid criminal record check.

Observation. Unrelated to the complaint. Contravention identified during the investigation: Child attendance records do not record the accurate arrival and departure time for each child. Accurate attendance records are required for each child. Submit by February 05, 2026, written confirmation that child attendance records are accurate.

Observation. The complaint alleged: There is dog feces in the child care outdoor play spaces. Follow up to this allegation included inspecting the outdoor spaces, interviews with staff members, and the Licensee. The outdoor spaces were inspected and noted dog feces on the deck. As a result of the follow up investigation, and information gathered; it has been determined that the allegation is substantiated. Children must not have access to substances that pose a risk to their health and wellbeing. Submit by February 04, 2026, written confirmation the outdoor play space is free of dog feces at all times. The complaint alleged: There are large sharp rocks in the outdoor sandbox. Follow up to this allegation included inspecting the sandbox, interviews with staff members, and the Licensee. The sandbox was inspected, and it was determined there were no large sharp rocks. As a result of the follow up investigation, and information gathered; it has been determined that the allegation is unable to be substantiated. The complaint alleged: The Licensee and other persons smoke and vape on the premise during the daycare's operating hours. Follow up to this allegation included surveillance, interviews with staff members, parents and the Licensee. It was confirmed by staff and a parent that a person had been observed smoking on the premise while care was being provided. As a result of the follow up investigation, and information provided by persons interviewed; it has been determined that the allegation is substantiated. The Licensee must ensure no person smoke or vape on the premise while child care is being provided. Submit by February 04, 2026, written confirmation that smoking and vaping is not occurring while children are in care. The complaint alleged: The diaper change mat is not being adequately cleaned between use. Follow up to this allegation included review of procedures, inspection of the diaper mat and diaper change area, interviews with staff members, and the Licensee. As a result of the follow up investigation, and information provided by persons interviewed; it has been determined that the allegation is unable to be substantiated. Unrelated to the complaint. Contravention identified during the investigation: Children had access via an unlocked door and unlatch gate to the bar area. As a result of this, a lighter, a cigarette butt and cannabis products were accessible to children. To ensure the health, safety and wellbeing of children, hazardous objects and substances must be inaccessible at all times. Corrected during the inspection.
