  Observation. Two play pen used for sleeping had mattresses that did not fit.

  Corrective action. The manager will ensure that the play pens are used as stated in manufacturers instructions. In the mean time, manager stated that the child will sleep only on the mattress not in the play pen.

  Observation. 2 staff files were reviewed. One staff was missing evidence of character references.

  Corrective action. The manager will obtain character references

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.

  Observation. Immunization status was not obtained for one employee

  Corrective action. The manager will obtain immunization status for the employee.

• HYGIENE AND COMMUNICABLE DISEASE

  A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.

  Observation. The children's bedding were stored together. The Licensee advised that the typical practice is to store the bedding in the storage compartments.

  Corrective action. The Licensee said she will store children's bedding separately.

  Observation. Upon Licensing's arrival, there were 8 children in care. One child was being picked up. For a family child care service type, the maximum number of children in care cannot exceed 7.

  Corrective action. The Licensee is to submit a plan as to how this will be addressed.

  A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.

  Observation. The attendance record did not accurately reflect the numbers of children in care as only 4 children were signed in. This is considered a repeat contravention as this contravention was identified during the last three routine inspections.

  Corrective action. All children in care must be signed in upon arrival.

  Observation. Upon inspection of the outdoor play area, Licensing observed a rock in a child's mouth posing a chocking hazard.

  Corrective action. Please ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. Upon inspection, Licensing observed a play structure with a slide located on concrete with no adequate ground surfacing.

  Corrective action. Please ensure that adequate ground surfacing is available.

  A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): Upon inspection Licensing observed knives accessible to children in the outdoor play area. The Licensee explained that she is always present with children. These were moved during the inspection and stored in a secured shed with a lock.

  Corrective action. Please ensure that children do not have access to any objects or substance that may be hazardous to the health or safety of a child.

  A licensee must ensure that children are supervised at all times by a person who is an educator, an assistant or a responsible adult.

  Observation. Upon arrival to the outdoor area, Licensing observed the Licensee talking on the phone with her back turned to the children. The Licensee was unaware that Licensing was on site.

  Corrective action. Please ensure that children are supervised at all times.

  A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

  Observation. Upon inspection, it was found that the fire extinguisher has not been serviced annually. The Licensee explained that she is aware of this.

  Corrective action. Please ensure that the fire extinguisher is serviced annually.

  Observation. Upon review of child records, there is no care plan available for a child with an allergy. The Licensee explained that she will follow up with the parents.

  The licensee must not permit a person over the age of 12 to be ordinarily present on the premises of a community care facility while children are present, unless the person is of good character and the licensee has obtained a criminal record check for that person.

  Observation. Upon inspection, Licensing observed a person at the facility over the age 12 who was ordinarily present without obtaining a criminal record check. The Licensee explained that a criminal record application has been submitted.

  Corrective action. Please ensure that criminal record check is obtained.

Upon inspection, LO observed the following -Gate latch is broken on the gate closest to the entrance of the facility -Medal chain links accessible to children in the outdoor play area -Panel on the side of the facility near the entrance is coming off and has visible screws, which are accessible to children

  A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

  Observation. During inspection of the facility, the following was found: 1. The washroom sink had brown stains behind the faucet and around the sink. The toilet has a brown ring in it and appears to require cleaning. 2. The fridge was inspected and has brown stains and residue under the vegetable and fruit drawers. 3. A wall in the main play area has chipped paint and exposed drywall.

  Corrective action. Ensure the facility is clean and in good repair

Observation (CORRECTED DURING INSPECTION): During inspection of the washroom it was found that toilet cleaning liquid was stored above the toilet on a shelf and is in reach of some of the children.

  Corrective action. Ensure the children do not have access to any substances that may be hazardous.

  Observation. During review of children's files it was found that only 1/2 of the children's files contained a written statement for persons who prepay part of the cost of services

  Corrective action. Ensure a copy of the written statement regarding prepayment of the cost of services is on file at the daycare for review.
