    Observation. Peeling paint on the cubby wall.

    Corrective action. The licensee will submit a plan outlining how this will be permanently addressed.

    A licensee must supply equipment, furniture and supplies that are (b) located so as not to block or hamper an exit in the case of fire or other emergency. Observation (CORRECTED DURING INSPECTION): The second emergency exit door only opened halfway due to a mesh material at the bottom of the door hindering full access. During the inspection, the licensee removed the mesh material. The emergency exit door now fully opens.

    Corrective action. Please ensure the emergency exit is not blocked or hindered in any way.

    The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent. Observation (CORRECTED DURING INSPECTION): A care plan was not annually reviewed. During drop-off time, the care plan was reviewed with the parent and a date of review and signature was obtained.

    Corrective action. Please ensure care plans are annually reviewed.

    Observation. Multiple areas of chipping paint observed on the mustard, blue, and red wall. As per the Licensee, she has a plan to address this.

    Corrective action. Please submit a plan to Licensing regarding how areas of chipped paint will be addressed.

    A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance. Observation (CORRECTED DURING INSPECTION): -Fabric on all 4 black child-sized chairs is heavily ripped and fraying. As per the Licensee contact, they will be discarding all 4 chairs. -Small brown table with metal frame that is displaying rust. By the 2nd day of the inspection, all 4 chairs and the small brown table was removed from the program.

    Corrective action. Please ensure the facility's equipment and furniture are clean and in good repair.

    A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure. Observation (CORRECTED DURING INSPECTION): The attendance sheet for the School Age program does not indicate a sign-out time for when the children leave the facility to go to school in the morning. There is also no sign-in time that indicates when children return to the facility after school. Licensing and the Manager discussed adding 2 new columns to the attendance sheet that will indicate these times. By the 2nd day of the inspection, the attendance sheets have been updated to indicate the information above.

    Corrective action. Please ensure the attendance sheet is revised to indicate the information above.

    Observation. Licensing observed the following: -there is a hole in the mesh of one of the windows that exceeds 3.5 inches which poses as an entrapment hazard -a couple linoleum tiles are chipping -dent on corners of baseboard

Observation (CORRECTED DURING INSPECTION): Upon reviewing children's files, siblings (2) were sharing one registration file.

    Corrective action. Please ensure that each child has his/her own individual registration file. During the inspection, the Licensee created a file and photocopied the appropriate documents to be put in the other sibling's file. Each sibling now has their seperate registration file.

    A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

    Observation. During the inspection it was noted that 3 linoleum tiles were chipped causing a potential tripping hazard. The staff stated that the tiles have been replaced two years ago and reinforcing material was added to the tiles at that time. Staff stated she will look into replacing the tiles.

    Corrective action. Please ensure the facility is in good repair.

    A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

    Observation. Licensing observed the following: - There are a few floor tiles that are broken and the material is become porous, therefore the surface cannot be cleaned and disinfected appropriately. The manager stated there is product that will be purchased to help resurface these tiles. - There are several areas on the wall that have chipped pain. The manager stated a painter has been hired to complete the painting of the facility, this will be completed over a long weekend when children are not at the facility.

    Corrective action. Please ensure the furniture, equipment and fixtures are clean and in good repair.

There were other large wooden games stored near the emergency exit which reduced the overall space by the egress door.

    Observation. Licensing observed the following: - The garbage cans in the bathroom have several rusted areas on the lid. The garbage cans also appear to be dirty on the outside. - Some areas of the bathroom floor were dirty and beginning to stain the flooring - The facility has a few floor tiles that are broken and are no longer cleanable - There are several areas on the wall that have chipped paint

    Corrective action. Please ensure the furniture, equipment and fixtures are clean and in good repair.

    Without limiting subsection (1), a licensee must ensure that (a) children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E. Observation (CORRECTED DURING INSPECTION): In conversation with the Manager the day before the inspection, Licensing was informed 3 children who will be going to Kindergarten in September were enrolled in the school age program. The Manager has applied for a temporary placement for the 3 children.

    Corrective action. Please ensure children attending a community care facility are divided into groups, with each group having no more than that permitted in Schedule E.

    Observation. Licensing noted the following: - The fire drill record was not available for review from Dec 2019- February 2020. The Manager and staff stated the fire drills were completed for those months. - The emergency evacuation record was not available for review for 2019.

    Corrective action. Please ensure a record respecting compliance with section 22 (2) (b) and (c) is on file.

    Observation. Upon review of the minor incident log book, Licensing noted a reportable incident from March 17, 2020 that was not reported to Licensing.

    Corrective action. Please ensure the Licensing officer is notified within 24 hours after a child is involved in, or may have been involved in, a reportable incident described in Schedule H.

Out of the 3 reviewed, 1 contained character references which appeared to not have been checked by the Manager. The Manager stated she called the references but did not write it down.
