- RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. Upon reviewing 4 children's files, Licensing observed the following: -3 children's emergency contacts are the parents of the child -1 child's file listed emergency contact names, but phone numbers were not available Licensing and the Licensee discussed emergency contacts to be individuals that can reach the parents when the Licensee cannot. As per the Licensee, she has already connected with the parents to obtain emergency contacts that are not the parents.
Corrective action. Please ensure children's files are complete with required information.

A licensee must ensure that each employee (a) is trained in the implementation of the fire drill system and emergency plan described in subsection (1), including the use of any equipment noted in the fire drill system and emergency plan.
Observation. The fire extinguisher displays a next service due date of: November 5, 2022 and therefore, servicing is overdue.
Corrective action. Please ensure the fire extinguisher is servicing is up to date.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.
Observation. On review of 5 childrens files, licensing observed 1 that was missing immunization status which is important information in case of a disease outbreak. Licensing discussed the incorrect information that was included in the file and to ensure files are audited regularly to be current.
Corrective action. Please ensure all children's files are current and contain the required information.
