Observation (CORRECTED DURING INSPECTION): A fire drill was not recorded for the months of September and October. As per the manager, the fire drill was practiced for these months. During the inspection, this information was recorded.

  Corrective action. Please ensure a fire drill is practiced and recorded monthly.

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.

  Observation. Expired criminal record check for one staff. As per the manager, this staff is currently away from the facility.

  Corrective action. Please ensure a current valid criminal record check is obtained for this staff.

  A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry.   Observation. Validation from ECE Registry is missing for one staff.

  Corrective action. The manager will obtain the validation for this staff to be placed in the staff's file.

  A licensee must have first aid kits that are readily accessible to all employees, including while care is provided off the community care facility premises.

  Observation. No instant ice pack in the first aid kit. As per the manager, there is not instant ice pack because it was already used. This is a repeat contravention from inspection #SMOY-BZ7UUL.

  Corrective action. Please ensure the first aid kit is complete with an instant ice pack.

  Observation. 6 wooden shelves on wheels that are unsecured and present a tipping hazard that could potentially fall onto children.

  Observation. Majority of food supply is expired in the emergency kit. This is a repeat contravention from inspection #NSAO-CPYQG7.

  Corrective action. Please ensure there is a sufficient amount of food available to sustain all staff and children for wat least 72 hours.

  Corrective action. Please ensure validations match the staff's current certification.

  Observation. 2/3 staff without documentation of verified references. References should also be child-care related. Licensing and the manager discussed documenting when the reference was contacted and what key points were shared. Alternatively, the manager can also ask for reference letters.

  Corrective action. Please ensure references are verified and checks are documented.

  A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): Licensing observed a loose screw on the floor of the children's washroom which poses as a choking hazard.

  Corrective action. Please ensure that children do not have access to any object or substance that may be a hazard.

  A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

  Observation. Upon reviewing the emergency kits, Licensing noted four emergency kits for children, three packagegs of rice crackers, and some water bottles. The current amount is insufficient to sustain the needs of all children and staff of the program for at least 72 hours. This is a repeat contravention.

  Corrective action. Please ensure there is a sufficient amount of emergency food supply and water to provide for all the children and staff of the program.

  A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.

  Observation. Upon reviewing 6 children's files, Licensing observed 1 child's file missing a doctor name and telephone number. As per the Manager, the child is new to the country. Licensing advised to put a note identifying this information on the child's file instead of leaving it blank. Families can also put in a walk-in clinic information to fulfill this information. This is a repeat contravention.

  Corrective action. Please ensure children's files are complete with all required information.

  A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure. Observation (CORRECTED DURING INSPECTION): Licensing observed that for two days in March, the attendance was not recorded for any of the children.

  Corrective action. Please ensure that children's daily attendance is recorded for each day that the facility is operating.

  A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

  Observation. Upon reviewing 6 children's files, Licensing observed the following: -1 child missing MSP # and immunization status -another child missing MSP # The Manager stated that these 2 children are new to the country and do not yet have an MSP #. Licensing advised the Manager to put a note of this on the forms instead of leaving them blank. This will also act as a reminder for the facility to follow-up with the families regarding the missing MSP #s. This is a repeat contravention.

  Corrective action. Please ensure children's files are complete with all required information.

  A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

  Observation. During the inspection of the physical facility it was noted that under the kitchen sink there were a multitude of plastic bags. Children could access the plastic bags with ease.

  Corrective action. Please ensure all plastic bags are stored in a manner that are inaccessible to children. Please ensure children do not have access to any hazardous items.

  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. During the inspection of the outdoor area of the facility it was noted that the rubber tiles located within the outdoor play area are separating and shifting. The tiles are posing a tripping hazard. The rubber surface is compromised and is no longer a continuous surface.

  Corrective action. Please ensure that the rubber tiles are installed/repaired in a manner that do not pose a tripping hazard.

  Observation. During the inspection of the facility it was noted that an Active Play and Screen Time policy was not available.

  Observation. During the inspection it was noted that some of the emergency supplies had expired food. It was also noted that there was not enough supplies to prepare, mitigate, respond, and recover from any emergency. This is a repeat contravention from 2021.

  Corrective action. Please ensure that you have enough supplies to prepare, mitigate, respond, and recover from any emergency.

  Observation. During the inspection 9 children's files were reviewed. It was noted that one child was missing a doctor's name, doctor's phone number, and emergency contact. Manager stated that the family is new to Canada and do not have a family doctor yet.

  Corrective action. Please ensure all children's files are completed.

  Observation. During the inspection it was noted that the staff have not implemented a fire drill since August 2021. Manager confirmed that fire drills have not be conducted since last year. This is a repeat contravention from 2019.

  Corrective action. Please ensure that fire drills are implemented and practiced at least once a month.

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.

  Observation. During the inspection 3 staff files were reviewed. One staff did not have any references on file. It was noted that the manager had two references on file. Licensing officer discussed with the manager that good practice is to have 3 character references for the manager. This is a repeat contravention from 2019.

  Corrective action. Please ensure all staff files are completed.

  A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.

  Observation. During the review of the staff files it was noted that two out of three staff did not have a record of immunization. This is a repeat contraventions from 2019.

  Corrective action. Please ensure all staff files are completed.

  Observation. During the inspection 3 staff files were reviewed. It was noted that validation of the ECE certificate for all staff was missing. This is a repeat contravention from 2019 and 2021.

  Corrective action. Please ensure all staff ECE certificates are validated through the system maintained by the early childhood registry.

  A licensee must ensure that each employee (c) practices implementing the emergency plan at least once each year.

  Observation. During the inspection it was noted that the emergency plan was not implemented.

  Corrective action. Please ensure that the emergency plan is implemented and practiced at least one a year.

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

  Observation. Licensing observed the following: 1) baseboard molding in the hallway missing leaving gaps between the edge of the flooring and the base of the walls 2) baseboard molding in the children's washroom appears to have fallen off the wall and is laying on the ground adjacent to the edge of the flooring and the base of the walls 3) cupboards in the kitchen are chipped along the edges and 4) in the main activity room there are posts and these posts have exposed gyproc due to damage.

  Corrective action. While licensing acknowledges facilities will have wear and tear the Licensee must address the issues as they arise. The above noted items are in need of repair or replacement to ensure the facility is in good repair and are non-porous to allow the surfaces to be cleaned and sanitized.

  A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.

  Observation. Licensing observed 3 containers of vitamins located on the lower shelf of the kitchen upper cabinets. Licensing also observed 2 containers of dish soap located under the kitchen sink. Both areas were unsecured. Staff personal belongings were accessible to children inside the doorway to the office/storage area. During the inspection Licensing observed several children enter this area unsupervised.

  Corrective action. The Licensee must secure these items in a manner that children do not have access to them.

• EQUIPMENT AND FURNISHINGS

  A licensee must ensure that all indoor and outdoor play materials and equipment accessible to children are (b) safely constructed, free from hazards and in good repair.

  Observation. Licensing noted the following: 1) the southwest corner of the wooden fence has portions that are rotting 2) the treads on the red slide are cracked 3) a green light pole is rusting 4) a few 'eye screws' are rusted on the south wooden wall of the play area near the steps 5) the upper gate is unable to close.

  Corrective action. The Licensee will need to submit a written plan as to how and when the above noted items will be addressed.

• EQUIPMENT AND FURNISHINGS

  A licensee must supply equipment, furniture and supplies that are (a) of sturdy and safe construction, easy to clean and free from hazards.

  Observation. In the main activity area licensing noted several tables being used for the children for various activities including eating lunch. The surface of the tables appear to be made of a porous material and are not able to be washed and sanitized.

  Corrective action. Licensee will need to replace or repair these items in a manner that are easy to clean and sanitize.

  A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

  Observation. Licensing observed a number of wooden shelving units within the program. These units are over 4' high and there the potential for them to tip over resulting in an injury to an individual.

  Corrective action. Secure any free standing units over 4' high to prevent it from inadvertently falling over.

  A licensee must have first aid kits that are readily accessible to all employees, including while care is provided off the community care facility premises.

  Observation. The staff of the program said there are times when children are taken off site for neighbourhood walks, parks etc. Upon review of the first aid kits there is nothing in it to address swelling such as an instant ice pack.

  Corrective action. Ensure the first aid kit includes items for use both on and off site.

  Observation. Licensing reviewed the facility's emergency supplies. In a conversation with the Manager licensing was informed that some of the food had expired and the facility is in the process of replacing it.

  Corrective action. The Licensee must have sufficient emergency supplies to meet their emergency supplies which applies to both children and staff.

  A licensee must keep current records of each of the following: (b) for each employee, the records required under section 19 (1) [character and skill requirements].

  Observation. Upon review of the staff files there was one file where the individual's criminal record clearance letter expired in May 2020.

  Corrective action. The Licensee must ensure all criminal record checks are current.

  A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the licensee using a system established and maintained by the director of the early childhood educator registry,

  Observation. Licensing reviewed all staff files and was unable to locate any evidence that staff early childhood educator certificates have been verified.

  Corrective action. The Licensee can verify all staff's certificates by going on line to the ECE Registry website and completing the request and making a copy of the results for the staff's file.

  A licensee must not employ a person in a community care facility unless the licensee is satisfied, based on the information available to the licensee under subsection (1) and the licensee's or, in the case of an employee who is not the manager, the manager's own observations on meeting the person, that the person (a) is of good character.

  Observation. Upon review of the staff files licensing was unable to locate any system or documentation that the Licensee has verified the staff's references.

  Corrective action. The Licensee will need to demonstrate that they have verified the 3 references for all staff.

  Observation. Licensing observed on several days children were not signed in/out or indicated if they were absent. A discussion was had with the licensee about different ways to ensure the sign in/out is completed.

  Corrective action. Please ensure a record indicating whether a child is absent, present and the time of arrival and departure is on file.
