A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. The Licensee is listed as the emergency contact for one child. Licensing explained that an emergency contact must be an individual (other than the parents) that the Licensee can contact in an emergency. The Licensee explained that she has followed up multiple times with the parents trying to obtain an appropriate emergency contact.
Corrective action. Please ensure an emergency contact that is neither the parents or the Licensee is listed on the child's registration file.
19100 - CCLR s.57(2)(d) - RECORDS AND REPORTING
A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (b) the courses of action recommended by health care professionals to address the needs of the child requiring extra support.
Observation. The care plan on file for a child with extra support needs has a documented expiry date of June 2023. As per the Licenseee, she has attempted multiple times to obtain a current care plan for the child.
Corrective action. Please ensure a current care plan is obtained for the child.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status. Observation (CORRECTED DURING INSPECTION): 1/6 children missing MSP#. This information was obtained and recorded by the Licenee during the inspection.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that, on the premise of the community care facility or in any vehicle used by employees to transport children, (c) no one grows or stores cannabis, except medical cannabis.
Observation. Licensing observed multiple packages with cannabis labelling and paraphernalia in one of the children's outdoor toy houses. As per the Licensee, she was unaware that this was here and did not know who it belonged too; the child care facility has not been using this backyard and have been going to the park as clean-up and maintenance is needed for the outdoor area. The Licensee immediately removed the items from the toy house and relocated the items to the other side of the perimeter fence off of her property.
Corrective action. Please ensure cannibis is neither stored or grown on the premises of the community care facility.
11032 - CCLR s.13(3)(c) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following in the backyard area: -2 big piles of dead leaves -multiple pieces of loose clothing and tarps scattered around -multiple pieces of garbage and broken items scattered around The Licensee is aware that the outdoor area is in need of a clean-up and has scheduled maintenance workers for the weekend to address this. The child care facility has been using the nearby park for outdoor time until the backyard is fully addressed.
Corrective action. Please ensure all areas of the facility are in clean and good repair while children are in attendance.
11020 - CCLR s.13(2) - POLICIES AND PROCEDURES
If a person prepays part of the cost of services provided by a class of community care facilities designated by the Lieutenant Governor in Council, the licensee must, at the time of the prepayment, deliver to the person a written statement setting out the terms and conditions on which a refund of all or any of the prepayment will be made.
Observation. Upon reviewing policies and procedures, the facility is missing a Repayment Policy. This is a repeat contravention from inspection #ONIR-CDGT6F.
Corrective action. Please ensure that a Repayment Policy is created and made available to parents.

A licensee must ensure that each employee (a) is trained in the implementation of the fire drill system and emergency plan described in subsection (1), including the use of any equipment noted in the fire drill system and emergency plan.
Observation. The label on the fire extinguisher displays a service due date of February 2023 making it overdue for servicing.
Corrective action. Please ensure the fire extinguisher is serviced and that a service label is attached that clearly indicates when the next service is due.

Observation. During the inspection of the physical facility the following was observed: - the fall zone for two (2) Little Tikes slides was non-existant. This was identified as a contravention in the inspection dated April 14, 2022 Insp #ONIR-CDGT6F. This was not addressed by the date mentioned in the inspection (April 28, 2022). This is a repeat contravention.

Observation. During the inspection of the emergency supplies it was noted the presence of expired food in the emergency kit. A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. During the inspection of the physical facility the following were observed: - several fallen branches were tucked away by the fence, however the branches were reaching in the play yard causing a hazard to young children. - two planks, approximately 1 foot long with rusted nails sticking out were scattered around the yard. - the fall zone for two Little Tikes climbers was non existent. - one box containing chalk and stencils was covered with green residue and the top of the box is beginning to peal. - one long plank was in the way of children reaching play equipment. - the majority of the play structures were covered in green residue and were in need of cleaning and maintaining. - a mat was scattered on the stairs to the facility, causing a tripping hazard. The licensee stated that she will use the weekend to clean up the back yard.
Corrective action. Please ensure a healthy and safe environment is provided at all times.
11010 - CCLR s.13(1) - POLICIES AND PROCEDURES
A licensee must ensure that the policies and procedures referred to in subsection (1) are implemented by employees.
Observation. During the inspection of the physical facility the following was noted: - the TV was on playing a children's movie. - three school age children were on various electronic devices (I-pads and laptop) playing various games and watching videos. - two younger children (over 2 years of age) were walking to the living room and glanced at the movie and then walked back to the kitchen. The children repeated this action several times. - one child (under 2 years of age) glanced at the TV several times. After approximately 40 minutes, the staff turned off the TV playing the movie and asked the school age children to turn off their devices. Licensee stated that this is not common practice and that today was a special circumstance because the school age children were on a pro-d day and they were allowed to watch TV and use their I-pads.
Corrective action. Please ensure that all policies and procedures are implemented by all employees.

In respect of a record referred to in this Division, a licensee must (c) produce records, on demand, to the medical health officer.
Observation. During the routine inspection the staff person was unable to produce her own file. Both staff person and licensee stated that a file is somewhere at the facility but were unable to produce it on demand.
Corrective action. Please ensure all files are available to be produced on demand to the medical health officer.
19340 - CCLR s.59(c) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.
Observation. During the inspection 8 random children's files were reviewed and one child's file was missing the gender of the child and one child's file was missing the Medical Service Plan number.
Corrective action. Please ensure all children's files are complete.
19070 - CCLR s.57(2)(a) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. During the file review of the facility and conversation with the licensee it was noted that a CRC for the licensee and another person living it the house were not obtained. License stated that they will apply for the CRC tomorrow.
Corrective action. Please ensure all persons ordinarily present have a CRC available.
12040 - CCLR s.19(1)(a) - STAFFING
A licensee must ensure that each employee (a) is trained in the implementation of the fire drill system and emergency plan described in subsection (1), including the use of any equipment noted in the fire drill system and emergency plan.
Observation. During the inspection of the physical facility it was observed that the fire extinguisher was present but it has not been serviced. Staff on site stated that the fire extinguisher was serviced sometimes in January but the tag was not available to identify the date the service took place.
Corrective action. Please ensure the fire extinguisher is serviced annually.
12180 - CCLR s.22(2)(a) - STAFFING
A licensee must ensure that each employee (b) practices implementing the fire drill system at least once each month.
Observation. During the inspection it was observed that fire drills for the year 2020 were present. Fire drill for January, 2022 was present, however fire drills for February and March were not noted down. Licensee stated that she has conducted the fire drill but she did not mark it down.
Corrective action. Please ensure fire drills are practiced and noted monthly.
