A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. -A staff's bag was stored in a cupboard that is accessible to children. Upon notifying the staff, the bag was stored elsewhere. Corrected during inspection. -4 bags of ice melt are stored in a washroom accessible to children.
Corrective action. The facility will store the bags of ice melt in an area that is inaccessible to children.
11200 - CCLR s.17 - HYGIENE AND COMMUNICABLE DISEASE
A licensee, other than a licensee who provides a care program described as either Preschool (30 Months to School Age) or Group Child Care (School Age), must have in a community care facility a sturdy surface for changing diapers, located outside the food preparation area, next to (a) a covered container for soiled clothing.
Observation. In both classrooms on the main floor, there is no covered containers available for soiled clothing or diapers. As per the mananager, both classrooms previously had covered containers, but the whereabouts of these containers are unknown.
Corrective action. The manager will have a covered container available in both washrooms.
15010 - CCLR s.14(4)(a) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. Two staff on-site without criminal record checks. As per a program staff, the facility has applied for the criminal record checks for the individuals and awaits the results. .
Corrective action. While waiting for the criminal record checks to be obtained, these two individuals will not be left alone with children. The facility will provide a copy of the criminal record checks to Licensing once obtained. Moving forward, all staff must have required documentation obtained before caring for children.

A licensee must supply equipment, furniture and supplies that are (b) located so as not to block or hamper an exit in the case of fire or other emergency.
Observation. Toy structure on wheels blocking one of the emergency exits. Licensing and staff discussed storing this structure elsewhere in order to ensure a clear pathway to the exit.
Corrective action. Please ensure emergency exits are not blocked.

A licensee must keep current records of each of the following: (d) written policies and procedures that are intended to guide employees in the care and supervision of children.
Observation. Reportable Incident policy currently named "Accidents" policy needs more detail and clarification. Licensing emailed the Reportable Incident bulletin to the Manager as a reference regarding what should be included in the policy.
Corrective action. Please rename this policy to relfect "Reportable Incident". Please add in more detail as outlined in the RI bulletin.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. 2/7 children with parents as Emergency Contacts. 1 of the children's last day is tomorrow (Dec. 8, 2023). Licensing explained to the staff and Manager that the Emergency Contact must be an individual that is not the parent.
Corrective action. Please obtain an Emergency Contact that is not the parent for the 1 child.

A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following: -in Class B, a shelving unit by the nap cots is no longer secured to the wall which has a potential to be a falling hazard. -in Class C, there are 4 unstable shelving units that are used to store toys and equipment. These shelves are wobbling and may collapse. -Outdoors: one of the slide structures if broken. As per the Manager, she has already contacted her maintenance worker to address this. -Outdoors: multiple pieces of garbage around the outdoor area. As per the program staff, the garbage items may have flown out of the garbage can that does not have a lid.
Corrective action. Please ensure the facility and its equipment are in clean and good repair while children are in attendance.

Observation. During the routine inspection 5 random children's files were reviewed and 2 children's files were missing proof of immunization. In the last inspection the manager said that she is moving towards a hybrid format (paper and on-line) for the children and staff files.manager said that the It person who is incharge of the digital portal might not have addded the Immunization hystory tab as per her request. At today's follow-up inspection the manager texted the IT person and the IT person provided a screen shot of the "immunization history" tab via text on the manager's phone. IT person said that the tab is now available to the application form.
Corrective action. Please ensure all children's files are complete.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. During the inspection of the physical facility the following was observed: - 6 cement pillars supporting the balcony of the facility had sharp and rough corners/edges presenting a hazard to young children. This item has been identified in the inspection dated February 24, 2022, Inspection #ONIR-CBXW3K. Manager stated that she is looking into addressing the issue. This is a repeat contravention. - a 4x4 plank that is separating the sand play space from the cement area was moving presenting a hazard to young children. During the inspection children were observed trying to balance on the plank and the plank was observed moving causing a fall hazard to young children. - around the perimeter of the facility (at the back of the facility, on Stewardson Street), on what appears to be the city lot, there is a field covered in blackberry bushes and morning glory. The plants often invade the chainlink fence of the facility causing a hazard to young children and staff. Manager stated that staff often trim the blackberry bushes and the morning glory. The blackberry bushes and morning glory must be maintained regularly to prevent injury to young children and staff.
Corrective action. Please ensure a healthy and safe environment is provided at all times.
11010 - CCLR s.13(1) - POLICIES AND PROCEDURES
A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. During the inspection licensing observed that some food in the emergency supplies bin was expired.
Corrective action. Please ensure you have enough supplies to prepare, mitigate, respond and recover from any emergency.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. During the inspection a random selection of children's files were reviewed for compliance and 2 children's files were missing a doctor's name and phone number. This is a repeat contravention from November 12, 2019, inspection #MPEE-BHVUWU and October 20, 2020, inspection #MPEE-BULQX3.
Corrective action. Please ensure all children's files are completed.

Observation. During the inspection a random selection of children files were reviewed and 15 children's files were missing proof of immunization record. The manager stated that they are presently moving towards a hybrid format (paper and on-line) for children and staff files and that some information might not be available on paper forms. This is a repeat contravention from November 12, 2019, Inspection #MPEE-BHUWU.
Corrective action. Please ensure all children's files are complete.

Observation. During the routine inspection the manager could not produce evidence of emergency plan being conducted annually. This item has been identified in the inspection dated November 12, 2019, inspection #MPEE-BHVUWU. This is a repeat contravention.

Observation. During the routine inspection,the manager was unable to provide evidence of the fire drill. This item was identified November 12, 2019, inspection #MPEE-BHVUWU. This is a repeat contravention.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. During the inspection of the physical facility the following were observed: - few floor boards in the preschool room have been removed to check for water damage and have not been replaced - the hardwood floor in the day-care classroom is separating in several places. Manager stated that she is waiting to receive quotes from contractors to replace the flooring. - the resilient surface (wood chips) in the playground is not sufficient to prevent a child from becoming injured
Corrective action. Please ensure that the facility is in clean and good repair.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): During the inspection of the physical facility it was observed that children had access to a variety of plastic bags stored in a cupboard under the sink in the classroom. The manager removed the plastic bags during the inspection.
Corrective action. Please ensure children do not have access to any hazardous objects or substances.

Manager stated that she has brought the child in and forgot to inform the staff.

A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.
Observation. During the inspection it was noted that the log book in the preschool room was not available. Staff stated that she has taken the log book home to record an incident and forgot to bring it back to the facility.
Corrective action. Please ensure a log book of minor incidents is readily available.

Observation. During the review of the staff files it was observed that one staff did not have a resume. Manager stated that she has moved from paper files to a digital format and that the portal sometimes is acting up, therefore some files are harder to access.
Corrective action. Please ensure all staff files are completed.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child. Observation (CORRECTED DURING INSPECTION): Licensing observed hand sanitizer located in the main entryway both indoors and outdoors. These items are accessible to children and the manufacturer's recommendation is to keep out of children's reach.
Corrective action. The Manager indicated that the hand sanitizer is for families and not children. Ensure all hand sanitizers are out of reach of children.
11200 - CCLR s.17 - EQUIPMENT AND FURNISHINGS
A licensee must supply equipment, furniture and supplies that are (b) located so as not to block or hamper an exit in the case of fire or other emergency. Observation (CORRECTED DURING INSPECTION): In the group child care (school age) program licensing noted equipment, on wheels, was placed in front of an exit door.
Corrective action. The Manager relocated this item during the course of the inspection.
11090 - CCLR s.15(1)(b) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. The following items need to be addressed: 1) the Manager said she has been actively looking to hire a company to come in and paint the walls however she has been unable to secure a company at this time. The walls and window frames have peeling paint and/or drywall mud (where the Manager has started to address the issue). 2) in the group child care (30 months to school age) program there are 3 adult sized, black chairs. Two of the chairs are in need of repair (ripped and exposed foam) and one is beginning to show signs of wear.
Corrective action. The Manager will continue to try and secure the services of a company for painting and if unsuccessful she will need to ensure the peeling paint and areas of mud are repaired and sealed in order for proper cleaning and sanitizing. The Manager said she would remove the chairs and have them repaired.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Due to the extreme heat this summer the Manager has installed a portable air conditioner which sits on a tabletop. The air conditioner is located in the group child care (30 months to school age) program. The unit is unstable and there is the potential for it to fall onto an individual.
Corrective action. The Manager said she would remove the air conditioner as it is no longer needed.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the entire outdoor play area is (a) enclosed in a manner that is suitable for the age and development of children, and will ensure that children are free of harm.
Observation. In the outdoor area at the south end of the playground the ground has eroded over time and there is now a gap of approximately 3" at the base of the fence in which a child could get stuck.
Corrective action. Ensure ongoing maintenance occurs and any gaps are enclosed to prevent entrapment.
11160 - CCLR s.16(3)(a) - OR SUPERVISION
Without limiting subsection (1), a licensee must ensure that (a) children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E.
Observation. There were children attending the group child care (school age) program who have not started school. The Manager said she was of the understanding that the children were enrolled to start school in September and therefore thought they could attend the program. Discussion occurred regarding this requirement and the Manager will apply for temporary placements for these children.
Corrective action. The Manager is aware of the legislative requirements and will apply for a temporary placement for these children.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed staff immediately store children's mats away after nap time. As per the CDC guidelines for Covid-19 children's mats need to be disinfected daily. The manager stated mats are always sprayed down after nap however the staff must have forgotten.
Corrective action. Please ensure a healthy and safe environment is provided at all times.

A licensee must ensure that children are adequately supervised by qualified employees in sufficient numbers to meet the needs of each child.
Observation. Upon arrival at the facility there were 16 children signed in with an ECE I/T and Responsible Adult. A review of the staff schedule showed an ECE I/T and 2 Responsible Adults in the 3-5 room with 21 children. The licensee contact (ECE) stated she was also present in the morning and was on her break at the time Licensing arrived.
Corrective action. This is noted as a repeated contravention. Please ensure children are adequately supervised by qualified employees in sufficient numbers to meet the needs of each child.
