Observation. A change in facility manager has occurred. Licensing must be notified of any change to the operation of the facility. Name and contact information for the Manager must be provided to Licensing.

Observation. Licensing Officer observed that one employee's immunization information was not in their employee file. This was corrected during the inspection.

Observation. Licensing Officer observed that the name and phone number of a physician was missing from one child's registration information. This is required information. Registration forms have been updated to include the information, however a method of ensuring that all families are using the correct form is required. An inspection of the attendance records shows that a record of arrival and departure times exists, however when children are absent, the space remains blank, which may cause confusion in an emergency when accounting for all children is necessary. A method of indicating that a child is absent is required. Inspections of the outdoor play space are being conducted, but have not been recorded this month. A system for ensuring that play space inspections are recorded is required.
