    Observation. An inspection of children's files revealed one child's record missing a name and phone number of a physician. This information is required under Section 57 of the Child Care Licensing Regulation. The information must be obtained if applicable, and if it is not applicable, an explanation must be indicated in the child's record. The facility currently does not have a log of daily, monthly and yearly play space inspections. A method for recording these inspections is required. This was corrected during the inspection.

    Observation. One employee file missing a required Criminal Record Check and Immunization information.

    Observation. The pump for the only toilet in the facility appears to be malfunctioning, tripping the breaker and creating a loud noise. It was observed that the toilet doesn't completely flush after each use, making difficult to sanitize and leaving a sewage odour.

    Observation. All required information appeared to be on site for each child, however it was not kept in a single location.

    Observation. One fence board is loose and crooked, creating a large gap at the corner of the yard. Fence board must be affixed and all gaps be less than 3.5".

    Observation. Practice of the emergency plan hadn't been recorded in the past year, however the plan was being practiced as the inspection ended.

Please ensure all reportable incidents are reported to licensing within 24 hours. Several attendance records were incomplete. Please ensure all attendance records are complete with both arrival and departure times.

    Observation. The supervision policy needs to include guidance for staff regarding what would occur in the event a child would stand on the bench and attempt to climb out the window. Also if a child was to stand on the bench to climb on to the sink area.

    Observation. There are gaps that exceeds 9 cm (3.5") at the base of the playhouse next to the fence, on both sides, that could pose an entrapment risk. Large storage bins, with a hinged plywood lids, have been installed along the fence, for the garbage bins, close to to the left side of the playhouse. A child could lift the lid and it could close on their fingers causing an injury. A method of preventing the children from opening these bins is required.
