  • C1.2 - Does the licensee operate the facility in a manner that will promote the health, safety and dignity of persons in care?Non-critical

    Observation. It was alleged that a staff member was video recording a child without parental consents. The video recording was reviewed and confirmed a child was being video recorded. The licensee confirmed there is no policy in place to allow for parental consents for photographing or video recording of children. The staff said they video recorded the child to obtain evidence of the language the child was using towards the staff. The health, safety and dignity of the child in care was not considered as the staff disclosed the intent of the recording was "I felt I was not being taken seriously by the parent or my supervisor" rather than assisting the child to regulate. This allegation is substantiated. A response is required by September 4, 2025, that outlines how the licensee will operate the facility in a manner that will promote the health, safety and dignity of children in care.

  • C10.1 - Are children provided with a healthy and safe environment, and protected from harmful actions?Non-critical

    Observation. It was alleged that a staff member did not use appropriate judgement when the staff video recorded a child. The staff video recorded a child that was acting out and using inappropriate language towards the staff rather than assisting the child to regulate. Interviews with staff reveals the staff will use a loud voice with this child which is listed as a trigger in the child's care plan. It is the responsibility of the staff to assist a child to regulate and to support interactions with other children and staff. This allegation is substantiated. A response is required by September 4, 2025, that outlines how the licensee will ensure that behavioural guidance is appropriate.

  • C8.2 - Is each child provided a comprehensive and coordinated program of developmentally appropriate activities that encourages physical, intellectual, language, emotional, and social development?Non-critical

    Observation. Unrelated to the complaint allegation the licensee disclosed that children have exceeded 3 or 4 times this summer the allowable 30 minutes or less of screen time per day. Watching a 2-hour movie is not considered providing a child with a developmentally appropriate activity to aid in development. A response is required by September 4, 2025, that outlines how the licensee will ensure that screen time is limited to 30 minutes or less per day.

  • C4.1 - Are facility records current and complete?Non-critical

    Observation. Unrelated to the complaint allegations, a care plan for a child does not have evidence that the plan was made in consultation with the parents. Consultation with the parents ensures the plan is current including all areas where a child may need extra support. This is a reoccurring contravention originally found out of compliance on May 27, 2025. Submit by September 4, 2025, evidence that the care plan has been made in consultation with the parents. Unrelated to the complaint allegations, a care plan for a child was not implemented as developed. A review of the care plan revealed it did not contain a record of compliance with the care plan requirements. A licensee must record compliance with the care plan and any behavioural guidance provided to the child as well as it's effect. A response is required by September 4, 2025, that outlines how documenting compliance with care plan requirements will be managed.

    Observation. Thorough daily, monthly and annual play space inspections are not occurring. To ensure a safe environment for children, thorough inspections of the play space are required so hazards can be identified and removed. This is a reoccurring contravention, originally found out of compliance on June 20, 2023. Submit by June 6, 2025, a thorough daily, monthly and annual play space inspection checklist and the plan to achieve and maintain compliance with completing the required inspections.

  • C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical

    Observation. Several employee records were missing work history and references. Prior to an employee working a manager is required to have complete records to ensure that the employee possess adequate training and experience to meet the care, supervision, and needs of the children. Submit by June 6, 2025, evidence that the staff records are complete with the missing information added and a written plan on how compliance will be maintained. Several employee records were missing evidence of compliance with the Province's immunization and tuberculosis program. In the event of disease outbreak, a licensee is required to have evidence of employee's complying with the Province's immunization and tuberculosis program. Submit by June 6, 2025, evidence that employee records are complete and a written plan on how compliance will be maintained. A review of the fire drill log noted that fire drills were not conducted for the months of November 2024, January, and February 2025. Implementing the fire drill system each month will ensure employees are prepared in the event of a fire emergency. Submit by June 6, 2025, a written plan to address how all employees will conduct the required monthly fire drills. The manager disclosed that children in care have not always had immediate access to an employee who holds a valid first aid and CPR certificate. Valid first aid and CPR ensure employees have the skills required in the event of an emergency. Submit by June 6, 2025, a written plan to address how children will, at all times, have immediate access to an employee who holds a valid first aid and CPR certificate.

    Observation. Several children's files were missing the name and telephone number of a medical practitioner, the medical insurance plan number, and the date on which a child stops attending the facility. In the event of an emergency this information can be used to identify a child. This is a reoccurring contravention, originally found out of compliance on June 21, 2022, and June 20, 2023. Submit by June 6, 2025, evidence that these files are complete with the missing information added and a written plan on how compliance will be maintained. Several children records were missing written consent to call a medical practitioner or ambulance. In the event of an emergency, where a child's parents and emergency contacts cannot be reached, the licensee is required to have written permission to call a medical practitioner or ambulance from the parent. This is a reoccurring contravention, originally found out of compliance on June 20, 2023, and May 22, 2024. Submit by June 6, 2025, evidence of written consent by the parents and a written plan on how compliance will be maintained. The daily attendance was reviewed and it was noted that several departure times were missing. Accurate daily attendance ensures that employees are aware of the number of children in care. This is a reoccurring contravention, originally found out of compliance on June 20, 2023. Submit by June 6, 2025, a written plan detailing how accurate attendance records will be maintained. A child's care plan did not contain evidence that the plan had been made in consultation with the parent. Consultation with the parents ensures the plan is current and a child will receive safe and supportive care. Submit by June 6, 2025, evidence that the care plan has been made in consultation with the parents and a written plan on how compliance with creating care plans will be maintained. Three children's files were observed to be missing a photo or digital image. Photos are required to be used to readily identify a child in an emergency. Submit by June 6, 2025, evidence that photos have been added to these records and a written plan on how compliance will be maintained. The facility does not have a log for all play space inspections, maintenance and repairs. Play space inspections ensure a safe environment free from hazards for children to play in.     Observation. The Licensee has not ensured that policies and procedures to guide employees in all matters regarding the supervision of children have been implemented by the staff. A review of the care and supervision policy, indicates that the following parts of the policy were not being followed: - Always be able to see and or hear the children. During outdoor and gymnasium play staff will remain with the children at all times. The entire group of children were outside. The Licensing Officer observed a child had come inside alone and was unsupervised in the music room. - Monitor children in the bathroom while respecting their need for privacy. It was disclosed by the Licensee/Manager that children are not being supervised when they leave the care program to go to the washroom.

    Observation. A review of the system in place to ensure children's records are complete indicated the following items missing; - physician name and phone number - date of enrolment - date of withdrawal Children's records are missing written consent to call a medical practitioner or ambulance. Facility is licensed for two groups of School Age Care. The children begin the program outside all together as one group. At this time there is no evidence on the daily attendance which shows when a child moves from outside to one of the three licenced rooms in the school.

    Observation. It is a requirement that ECE and Assistant certificates be validated. The FLO guided the Manager to the ECE registry website and provided education on how to verify the certificate. The Assistant's certificate was verified at the time of the inspection. This item is considered as "CDI" - corrected during inspection.

    Observation. Observations: Licence, and Manager name posted, no changes to the structure or operation of the facility. Contravention It is a requirement that any Educator or Assistant Certificate be displayed in a prominent place. Manager posted Assistant certificate immediately. This item was corrected during the inspection.

    Observation. Observations: Log of minor injuries/incidents unexpected events recorded on attendance sheet, , children's arrival and departure times consistently recorded, reportable incidents reported and mitigated appropriately. Contraventions: A review of the ystem in place to ensure children's records are complete indicated that some children are missing Dr's Names and phone numbers. Play space inspections are recorded on attendance sheets - occasionally the evidence that the inspection has been conducted is missing.

    Observation. Manager self identified that when foods are served to children, it is not recorded anywhere.
