Observation. Linoleum in one of the bathrooms beside a toilet is missing leaving the wood under the linoleum exposed. With the flooring in poor repair and wood exposed proper cleaning and sanitizing of the floor cannot occur. A response is required by June 17, 2026, on how this risk will be mitigated.

Observation. Hand towels for children were observed to be hanging on hooks touching each other. Single use hand towels are required to prevent the spread of germs. This is a reoccurring contravention originally found out of compliance on October 30, 2023 and June 6, 2025. A response is required by June 17, 2026, on how this risk has been mitigated.

Observation. A review of the current written polices and procedures that are intended to guide employees in the care and supervision of children was conducted. It was noted and confirmed by the manager that there is no care and supervision policy. The licensee must develop and provide staff with all the current facility polices and procedures to ensure staff are following best practices in the care and supervision of children. Submit a copy of the developed care and supervision policy with evidence that staff have been provided a copy by August 6, 2025.
- C10.1 - Are children provided with a healthy and safe environment, and protected from harmful actions?Non-critical
Observation. It was alleged that an educator grabbed a child by the arm and threw the child to the ground. During staff interviews an educator stated the child was running out onto the street, and therefore, they held the child's hand to walk the child back to the group. The child said "you are holding too tight" so the educator released the child's hand not realizing the child was pulling back. When the hand was released, the child fell backwards to the ground. Based on the information received and having no witnesses to the incident, this allegation is unable to substantiate. It was alleged that an educator used scissors to cut a child's soiled underwear off of their body while the child was standing. During staff interviews, an educator confirmed that the underwear was cut off the child and that the child was upset by this action. A child must not be subjected to harsh, belittling or degrading treatment by an employee. Based on the educator's confirmation, this allegation is substantiated. Submit a plan detailing how you have addressed the above harmful action by August 4, 2025.

Observation. Secondary to the complaint received, it was confirmed that an emergency restraint was used on a child and the parents were not notified. An incident report for the use of an emergency restraint was submitted to licensing, however, it is the responsibility of the licensee to ensure parents are notified immediately if their child has been involved in a reportable incident. Submit details on how parents will be notified as required by August 6, 2025. Secondary to the complaint received, it was reviewed and confirmed that a child was involved in an incident that should have been recorded in the minor accidents, illnesses and unexpected events log. It is a requirement that the licensee maintain current records for incidents involving children. Submit details on how incidents involving children will be recorded as required by August 6, 2025. Secondary to the complaint, the most current care plan reviewed was noted to have been developed on April 4, 2024. It is the responsibility of the licensee to ensure care plans are reviewed at least once each year and are maintained and revised as necessary in order to support the child. Submit the updated care plan for review by August 6, 2025. The same care plan was found to have no evidence of being developed in consultation with the parent. The parent confirmed they were not consulted in the development of the plan. Parents input is vital to the development of a plan which will support their child and the staff working with that child. Submit evidence that the parent has been included as part of the development of the care plan by August 6, 2025.

Observation. Changes to the outdoor play space were made without notification and approval by licensing. Notification of changes with approvals are required prior to making the changes to ensure that the health and safety of the children has been taken into consideration. Submit by June 6, 2025, the changes which occurred, when the work was completed, and the health and safety plan for the children during the work.

Observation. A sleep mat was observed to be in poor repair with rips in the vinyl allowing the foam to be exposed. When furniture and equipment are in poor repair proper cleaning and sanitizing cannot occur. Submit by June 6, 2025, how you have addressed the requirement for furniture and equipment in the facility to be in good repair.

Observation. Two children files are missing the name and telephone number of a medical practitioner and the date on which a child stops attending the facility. In the event of an emergency current records for each child identifying personal information is required. Submit by June 6, 2025, evidence that these files are complete with the missing information added. Several children records are missing written consent to call a medical practitioner or ambulance. In the event of an emergency, where a child's parents and emergency contacts cannot be reached, the licensee is required to have written permission to call a medical practitioner or ambulance from the parent. Submit by June 6, 2025, evidence of written consent by the parents. The daily attendance is not reflecting when a child moves from one program to another program in the first half an hour of the day. Accurate attendance ensures that employees are aware of the number of children in care. Submit by June 6, 2025, evidence that the daily attendance is accurately reflecting when a child moves from one program to another. Several children records are missing other information, such as physical characteristics, which can be used to readily identify the child in an emergency. Submit by June 6, 2025, evidence that the information has been added to the records.

Observation. An epi-pen, Benadryl, and Tylenol were stored in a child's backpack accessible to children. The medication was removed and put out of reach of other children during the inspection. The licensee did not have a completed medication record by the parent indicating permission, dosage or when to administer medication. The epi-pen did not have a prescription label allowing for confirmation of who the prescription was for, and if the prescription was current or expired. Submit by June 6, 2025, evidence that the medication form is complete, and the epi-pen has a prescription label.

Observation. A review of staffing files, and a discussion with the manager, indicated all files are missing evidence that certificates have been verified using a system established and maintained by the director of the early childhood educator registry.

Observation. A review of the staff records indicates one employee is currently working without a criminal record check on file.

Observation. A play pen used for napping was observed in the washroom for the Group Child Care (under 36 months) program. As this washroom is used by the program it would not meet the definition of an area located away from activity.

Observation. The facility registration forms are on a data base. A review of children's records, with information printed from the data base, indicates the following missing items: - immunization status, medical insurance plan number on emergency cards - a record of any person not permitted access to a child - the date on which a child stops attending the community care facility The children records do not indicate written consent to call an ambulance or medical practitioner in the event the parents cannot be contacted. A review of the daily attendance indicated several missing departure times. All children records are missing other information that can be used to readily identify the child in an emergency.

There were items labelled "Keep out of reach of children" in the cupboard under the sink in the School Age care program. - this was resolved at time of inspection There is one broken vertical plank in the back right hand corner of the play house in the Group Child Care (30 months to School Age) program. The gate in the School Age program cannot be closed There is a section of fence within the play yard that is leaning over and missing two vertical planks

Observation. A medication administration authorization and safety plan for administering the medication was missing for one child, however information has been shared with program staff.

The information printed out from the data base is missing a Dr's Name and phone number or an indication that there is no doctor and consent to call an ambulance or medical practitioner in the event the parents cannot be contacted.

Observation. Observations: Licence and ECE, IT and Assistant Certificates posted, no changes to the structure or operation of the facility. Contravention It is a requirement that the Manager's name be posted in a prominent place

Observation. Staff ECE, IT and Assistant certificates are posted, however there needs to be a method of recording the certificate validations

Observation. Observations Facility uses an electronic data base for record keeping. All rooms have tablets that are continually being updated throughout the day. Staff and families sign in via a QR code on their smart phones. There are some families without smart phones, these families are signed in by staff. The FLO reviewed the information in the data base with the Manager and found child files to be complete. Logs of minor injuries/incidents unexpected events updated regularly in all rooms, children's arrival and departure times consistently recorded, reportable incidents reported and mitigated appropriately. Contravention Daily play space inspections recorded appropriately. Monthly and yearly play space inspection need to be recorded

Observation. It is recognized that a verbal exchange with the previous manager had occurred prior to starting work on the yard. Changes were discussed, however a written plan for the installation of permanent features was not received or assessed by licensing prior to installation.
- C2.1 - Are written policies and procedures in place to guide employees in all matters regarding the care and supervision of children?Non-critical
Observation. The Director of Licensing's Standards of Practice - Safe Play space states "Use zones are surfaces under and around equipment, generally extending 6 feet in all directions (swings should extend a minimum of 6 feet from the outer edge of the support structure on each side and the front and back of the swing should extend out a minimum distance of twice the height of the swing as measured from the ground to the swing hangers on support structure). Children should not be allowed in the use zone to hide, relax, play, etc. The tire swing installation has a use zone on the side that is closest to the chain link fence that is less that twice swing hanger height. Due to the tire swings being installed in a manner that does not meet the use zone requirements of the Director of Licensing's Standards of Practice - Safe Play space a detailed plan for the use of and the supervision of the tire swings are required. There are a variety of features in the play area that require constant active supervision by staff. Provide written policies and procedures for staff to ensure that the play areas are adequately supervised. Ensure that the following are included in your procedures : - Activation/deactivation and maintenance of the stream Supervision for the following areas : - Stream - Climbing stumps next to the stairs - Climbing Ramp - Small bridge that crosses the stream - Rope that hangs off the tire swing support - Climbing handles that are attached to the tire swing support.

Observation. The tire swing horizontal support has additional eyes installed either side of the eye that supports the tire swing chain. Senior staff in attendance today confirmed that these are for the installation of swings with horizontal seat and two chains for the seat to attach to. Due to the lack of safe use zone, the installation and use of this type of horizontal swing seat is not approved. There are several climbing handles that have been installed on the tire swing support that exceed 5' There is a stump scrambling slope installed next to a set of wooden stairs. The stairs have no railing. A child could fall off the stairs on to the stump area. The upper horizontal bar that the chain link fence attaches to is missing, making the fence unstable. There are several short sharp sections of wire that attached the fence to the upper bar sticking out.

Observation. Facility staff are appropriately trained and experienced. System in place to review completeness of staffing files, first aid, ECE and Assistant certificates validated. One staff member's file has a CRC from the RCMP in Ontario. CRC from the Ministry of Justice was submitted early in January 2020. Manger stated that until the Ministry of Justice CRC is received this staff will not work alone with children.

Observation. Observations : Written policies and procedures in place and available for staff to review. Fire Drills practiced and recorded appropriately. Contravention : FLO reviewed records which indicate that the Emergency Response plan has been reviewed by all staff. Manager indicated that the Emergency Response plan has not been implemented in the past 12 months.

Contravention: In January the facility was unable to provide care due to a "Snow Day". A service Delivery Incident Report needs to be submitted.
