    Observation. A review of facility records show that the criminal record checks have expired for both staff. New acceptable results are required immediately and a system for ensuring that new results are obtained prior to expiration is required.

    Observation. A review of the log of minor incidents showed that a past incident that should have been reported to licensing was not. The past incident must be reported, and a method created for ensuring that future incidents are reported in a timely manner. Some children's records were missing required information. Files must be updated, and a system to ensure all information required under Section 57 is obtained before a child attends the facility is required.

    Observation. This facility has not been tested for radon gas. This is a repeat infraction. Radon testing is in progress.

    Observation. No record exists of emergency plan practice within the past 21 months. This is a repeat infraction. A supervision plan is required for the rope in the outdoor play space. Emergency kit contained inadequate resources to enable staff to respond in an emergency situation.

    Observation. Several children's files were missing required information, including personal health numbers, physician name and phone number, immunization status, emergency contact person, and persons authorized to pick up the child. Arrival and departure times were indicated on the attendance record, however some attendance records were missing an indication that a child was absent.

    Observation. Several gaps along the back fence are between 3.5" and 9".

    Observation. Evidence of employees practicing an emergency plan each year is required.

    Observation. Some children's registration forms were found to be missing their Personal Health Numbers.

    Observation. Parts of the bathroom and kitchen seemed dirtier than daily use might cause. A system for daily cleaning/disinfecting of high contact surfaces like toilets, door knobs and door frames is required. Diaper change mat is torn, making it hard to disinfect, and must be covered or replaced. First aid kits require re-stocking of supplies. A safety and supervision plan is required for the new trampoline, teeter totter and slide. Daily outdoor play space inspections were recorded, but some items were missed. There is no record of monthly or yearly inspections.

At this facility the child has a care plan. Ensure that a care plan is on site for this child.
