Observation. While on site, it was noted that the plastic on the changing mat had started to crack in some areas. Not allowing the changing mat to be adequately cleaned and sanitized.
Corrective action. Manager stated that they would replace the change mat.

Observation. LO observed and discussed the following with the Manager, re: outdoor play space - No sufficient protective/resilient surfacing to reduce the risk of injury from fall - Overgrown grassy weeds and uneven muddy surface pose tripping and slipping hazards
Corrective action. Ensure that a healthy and safe environment is provided at all times while children are in care.
