Observation. - A ceiling tile is missing from the infant room bathroom - Paint is chipping in the shared toddler/3-5 bathroom - DLSOP Safe Play Space - Standing water was observed in both playgrounds
Corrective action. Submit a corrective action plan.

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation. A child was not signed out when they left the facility in the middle of the day.
Corrective action. Ensure accurate attendance records are kept.

Observation. A staff lifted a child (under 36 months of age) by their upper arm off the climber, and held the child by his upper arm and carried him with his feet off the ground.

Observation. Licensing observed the following: - Ants in the nap room accessible to the children - Cubbies not secured to the wall posing a hazard to the children if it falls over - Garbage bins with no lids, children have access to dirty tissues, gloves, etc. - Staff lunches and children's water bottles stored near the bathroom and diaper change area
Corrective action. Ensure a healthy and safe environment.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following: - The white fence separating playgrounds is showing signs of wear - The landscape fabric in the toddler playground is exposed, posing a tripping hazard - High touch areas in the childrens bathroom (i.e. door, door handles and casing around door) are covered in dirty fingerprints.
Corrective action. Ensure the facility is clean and in good repair.
11020 - CCLR s.13(2) - HYGIENE AND COMMUNICABLE DISEASE
A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.
Observation. Licensing observed the following: - Staff are not sanitizing the change mat after each diaper change. A baby wipe is used to wipe the surface. - Bleach solution is being shared between the main classrooms and the bathroom/diaper change area. - Staff lunches and children's water bottles are stored near the bathroom and diaper change area.
Corrective action. Ensure there is a system to practice health and hygiene.
15030 - CCLR s.46(1) - LICENSING
A licensee must display in a prominent place in the community care facility the certificate, or a copy of the certificate, of any educator or assistant employed to work in the community care facility.
Observation. Licensing observed a staff certificate not posted.
Corrective action. Ensure all certificates are posted.
10070 - CCLR s.11(4) - POLICIES AND PROCEDURES
A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Licensing observed the following: - All First Aid kits are low on supplies - Emergency supplies are insufficient to implement the emergency plan
Corrective action. Ensure there are sufficient supplies in the first aid kits and emergency kits to implement the emergency plan.

A licensee must keep, for each child, a record showing the following information: (b) date of enrolment in the community care facility.
Observation. A random selection of children's files were reviewed. Out of the 9 reviewed, 1 file was missing an enrollment date.
Corrective action. Ensure an enrollment date is obtained.

The licensee must (a) develop the care plan in consultation with a parent of the child requiring extra support and any person requested by the parent.
Observation. A random selection of children's files were reviewed. Out of the 9 reviewed, 4 children's files did not contain care plans that were developed in consultation with the parent or any person requested by the parent.
Corrective action. Ensure a careplan is created for all children requiring extra support.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. GCC 302- Fridge temperature not maintained between 0 and 4 degrees Celsius. GCC 301- Broken/ripped foam pieces, loose adhesive tapes and pulled/snagged carpet around the play furniture.
Corrective action. Ensure that a health and safe environment is provided at all times while children are in care.
11010 - CCLR s.13(1) - MEDICATION
If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (b) readily accessible to employees.
Observation. Medication was not readily accessible to employees for a child requiring one. Child's file indicated that facility has permission from the parent to administer medication in response to child's medical needs.
Corrective action. Ensure that medication is available onsite.
16020 - CCLR s.53(1)(b) - POLICIES AND PROCEDURES
A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].
Observation. No records indicating that fire drill in GCC-302 (for May 2023) and emergency drill in GCC- 301 were practiced with the children and staff.
Corrective action. Ensure that yearly emergency drill and monthly fire drills are practiced and accurately recorded.

Observation. The fridge temperature was not maintained between 0 and 4 degrees Celsius (corrected during inspection) - The gate leading out of the playground can be easily pulled and opened by anyone. There is no lock or latch to prevent the children from leaving the back yard area. - 2 of the classrooms inspected had areas were the drywall was exposed
Corrective action. Ensure the facility provides a safe environment at all times while children are in attendance.

A licensee must keep, for each child requiring extra support, a current care plan showing the following information: (b) the courses of action recommended by health care professionals to address the needs of the child requiring extra support.
Observation. A review of children's files found that one file did not contain a care plan with instructions for a child who has a severe allergy. The facility had medication box with directions on how to administer medication but did not contain a plan about when to administer and also what to do after medication had been administered.
Corrective action. Ensure children who require a care plan have one developed prior to attending the facility

Observation. A review of 6 files found the following: - 3 files were missing the name and telephone number of a medical practitioner (or clinic) - 2 files were missing the name of an emergency contact
