    A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.

    Observation. One child missing MSP#. As per the Licensee, she has connected with the family to obtain this information. The family is new to the country and awaits the MSP#.

    Corrective action. The Licensee will have the parents fill out this missing information once obtained.

    Observation. One child missing doctor name and number. As per the Licensee, she has connected with the family to obtain this information. Licensing and the Licensee discussed walk-in clinic information as being sufficient if families do not have a family practitioner.

    Corrective action. Please ensure all required information for children is obtained before starting at the facility.

    Observation. One staff without a validation from the ECE registry and validation on file for the Licensee is for the previous certification.

    Corrective action. The Licensee will validate the staff's certification and place the validation in the staff's file. The Licensee will validate her current certificate and place the validation in her file.
