A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following: -The diaper mat was ripped. This is a repeat contravention from inspection #CLAN-D5JQ66. Please replace by August 25, 205
Corrective action. In discussion the Manager stated she corrected this item at the previous inspection.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. -The landscape fabric in the outdoor play area was exposed. This is a repeat contravention from inspection # CLAN-D5JQ66 Please replace by September 21, 2025 -A CPR mask was missing from the first aid kit -In reviewing registration forms it was observed that a child requires emergency medication. An allergy list or care plan was not posted and the medication was not on site. -Access to staff bags was observed behind a cabinet. This must be secured.
Corrective action. In discussion the Manager stated that they replaced the woodchips over the landscape fabric last year but this has reoccurred. In discussion the Manager stated she will talk to the family of the child who has an allergy that requires emergency medication and she will post the allergy list in a place where everyone can see. In discussion the Manager said she will put the staff bags higher away from the children.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. During the inspection the following was observed: -Three windows on the second floor were open but were not locked. The windows were easy to open. This is a potential hazard. This was corrected during the inspection. -A playpen had wood on the bottom and tears in the fabric. This could be a serious hazard as the playpen was not being used according to the manufactures guidelines -Blackberry thorn bushes were observed hanging over the fence in several areas. This is a repeat contravention from inspection #CLAN-D5JQ66
Corrective action. In discussion the Manager said she has reminded the staff to lock the windows and the playpen will be replaced. The Manager discussed that the blackberry thorn bushes were cut three weeks ago. Licensing reminded the Manager of regular maintenance in the outdoor play area.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following: -A bottle of bleach and water for cleaning diaper mats was not observed in the washroom. One bottle of bleach and water is required for the kitchen and one is required for the washroom to avoid cross contamination. In discussion the Manager stated this was the practice at the facility yet this was not observed during the inspection. -Landscape fabric in the outdoor area was exposed in several places. This could present a tripping hazard. Please correct by June 28, 2024

Observation. During the inspection Licensing observed several tall shelves over 6 feet tall in the hallway were not secured to the wall and were tippable. This is a repeat contravention from inspection #KCHG-CT6P9X
Corrective action. In discussion the Manager stated that the shelves were secured after the previous routine inspection. Please ensure active supervision is provided in this area. Please ensure all shelves are secured.

Observation. The following was observed in the facility: -A diaper mat had a ripped edge and the foam was exposed. -A door was missing a door knob
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Licensing observed the following: -A plastic bag was sticking out of the cupboard in the washroom. This was accessible to children -A bottle of air freshener was on the edge of the counter beside the sink. This was accessible to children -A bottle of liquid soap was observed under the sink in the washroom. This is a repeat contravention from inspection #MZIN-C5ET6Y -Two windows were open 9.5 inches and 12 inches wide. The facility is located on the second floor of the home.This could present a falling hazard. Window limiters were recommended. This was corrected during the inspection. -Blackberry bushes, with thorns, were located at the children's level in the outdoor play area.

If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (a) administered to the child in the amount and at the times specified by the child's parent or in the child's record or care plan.
Observation. Licensing observed Child A fall in the playground. First aid was given to the child. The Child's wound was cleaned with a product with a Drug Identification Number (DIN)
Corrective action. In discussion a signed permission form was not completed by the parent prior to the application of this medication. The Manager removed the items from the first aid kit.
16010 - CCLR s.53(1)(a) - POLICIES AND PROCEDURES
A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. The emergency supplies were observed. Supplies were located in a plastic bag without a handle and a backpack. The food was expired.
Corrective action. Licensing and the Manager discussed options for transporting emergency supplies and the Manager disclosed that a wagon/suitcase used for the supplies was broken and had not been replaced. The Manager will replace these items and find a new solution for transporting the emergency supplies.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed a few rotting wood boards in the outdoor play area. These boards which appeared to have mold, were attached beside the railings of the stairs, and other ones were laying on the ground beside the outer wall of the house. The manager stated that the rotten wood board attached to the railings of the stairs will be replaced. This item is a repeat contravention, and was identified and contravened on July 28, 2021 on routine inspection report #MZIN-C5ET6Y. In addition, Licensing also observed in the outdoor play area that a wood slat of the fence was detached leaving a gap on the fence. The wood slat was then placed vertically leaning up against the fence, which could potentially fall on children while playing. The manager stated during the inspection that the fence wood slat has been detached from the fence due to high wind.
Corrective action. Ensure that the rotting wood boards are all removed from the outdoor play area, as it poses a health hazard to children while playing in the area. Furthermore, ensure that the wood slat is not leaning up against the fence and that the wood slat is to be placed back to make sure there are no gaps on the fence.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Licensing observed that the garden hose outside the play area was unravelled which could pose as a tripping and strangulation hazard while children are in the outdoor play area. In addition, Licensing observed an exposed nail beside the railings of the stairs, which is within reach of children as they come down the stairs.
Corrective action. Ensure that the garden hose is rolled up and placed out of reach from children. Ensure there are no exposed nails within reach of children outdoors.
11200 - CCLR s.17 - HYGIENE AND COMMUNICABLE DISEASE
A licensee, other than a licensee who provides a care program described as either Preschool (30 Months to School Age) or Group Child Care (School Age), must have in a community care facility a sturdy surface for changing diapers, located outside the food preparation area, next to (a) a covered container for soiled clothing.
Observation. Licensing observed that in the bathroom, the lid of the garbage bin for soiled diapers has been removed. The lid was placed just beside the garbage bin. This item was identified and contravened on Apr. 21, 2020 on routine inspection report #JKIM-BNVSFK.
Corrective action. Ensure that the lid of the garbage bin for soiled diapers is placed back.

Corrective action. Ensure that the name of the manager is posted in a prominent place inside the facility.

Observation. Licensing observed that one child did not have the doctor's phone number listed on file. This item was previously contravened and identified on Apr. 21, 2020 on routine inspection report #JKIM-BNVSFK.
Corrective action. Ensure that the doctor's phone number is also included on file. The manager stated during the inspection that this information will be retrieved from the parents.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. The following were observed and discussed with the manager: - Garbage bin with soiled diapers and paper towels was not covered. This is not considered a sanitary practice. - Children's sleeping cots with fitted sheets/blankets are touching each other. Some strategies to store children's blanket differently was discussed to avoid any germ spreading between the cots.
Corrective action. Please ensure that a healthy and safe environment is provided at all times while children are in care.

Observation. LO observed and discussed the following with the manager: Napping arrangement: currently there are 4 large playpens and 4 sleeping mats available within the naproom. LO noticed that all 4 playpens have been modified, i.e. plywood were placed on the bottom of the playpens. Also the mattresses used on these 4 playpens are very thin. LO discussed different options to provide safe sleep environment while maximizing the current nap room space.
Corrective action. Please ensure that children are provided with equipment, furniture and supplies that are sturdy, safe construction, easy to clean and free from hazard.

A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.
Observation. Handwashing for children: LO noticed children did not dry their hands using available paper towel after washing their hands. LO discussed about not sharing a hand towel among the children for this purpose, i.e. drying hands.
Corrective action. Please ensure to establish a program to instruct children in, and to practice the rules of, health and hygiene.
