- EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. The following was observed during the inspection: -Two slides in the outdoor area are cracked and must be replaced. This is a repeat contravention from inspection CLAN-D8KUWA. Please correct by November 17, 2025 -Foam mats are soiled and have several cracks. Licensing strongly recommends replacement of these tiles This is a repeat contravention from inspection CLAN-D8KUWA Please correct by November 17, 2025 -The mesh on a playpen has holes. This must be removed. -The couch has tears and must be repaired.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following during the inspection: -A bottle of hairspray was observed in the washroom. This is a repeat contravention from inspection # CLAN-D8KUWA. This was removed during the inspection. -A cord in the naproom was close to a bed and must be secured. 11010 - CCLR s.13(1) - HYGIENE AND COMMUNICABLE DISEASE
A licensee, other than a licensee who provides a care program described as either Preschool (30 Months to School Age) or Group Child Care (School Age), must have in a community care facility a sturdy surface for changing diapers, located outside the food preparation area, next to (a) a covered container for soiled clothing.
Observation. A garbage can for paper towels and soiled diapers was not observed in the washroom.
15010 - CCLR s.14(4)(a) - POLICIES AND PROCEDURES
A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Licensing observed the emergency supplies. Cans of food were expired, a can opener was not present and bowls and utensils were not available for use.
Corrective action. Please replace food prior to the expiry date and ensure you have supplies available
13020 - CCLR s.22(1)(b) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure. Observation (CORRECTED DURING INSPECTION): Licensing observed that the daily attendance record was not completed for two days. This was corrected during the inspection. This is a repeat contravention from inspection # MZIN-CFURX2
19090 - CCLR s.57(2)(c) - STAFFING
The licensee must not permit a person over the age of 12 to be ordinarily present on the premises of a community care facility while children are present, unless the person is of good character and the licensee has obtained a criminal record check for that person.
Observation. A criminal record check for a person over the age of 12 who is ordinarily present is not available. The Manager's CRC has not been renewed and is expired
Corrective action. Please ensure to apply for Criminal record checks prior to the expiry date. Please contact Licensing when this is completed.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. During the inspection Licensing observed the following: -Two slides in the outdoor play area are cracked . One is repaired with duct tape -The foam mats in the indoor play area appear soiled and had visible debris
Corrective action. In discussion the staff stated she swept yesterday and that she cleaned the tiles with bleach. The staff determined it was time to replace the tiles.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following: -A bottle of bleach and water was not available for use as the bottle was broken. Another empty bottle was provided during the inspection. -Hairspray was observed on the counter (this was removed during the inspection) -A cord in the naproom was observed close to a bed. This could cause a potential hazard. -Landscape fabric was observed in the outdoor play area. More woodchips or other material is required to provide a resilient surface. Please correct by September 28, 2024
Corrective action. In discussion the staff stated that she was using windex to clean surfaces. Licensing is unsure if Windex will disinfect surfaces appropriately. The staff will prepare two bottles of bleach and water, one for washroom and one for other cleaning purposes. In discussion the Licensee stated he has ordered more woodchips for the outdoor play area.
11010 - CCLR s.13(1) - POLICIES AND PROCEDURES
A licensee must keep current records of each of the following: (d) written policies and procedures that are intended to guide employees in the care and supervision of children.
Observation. The policies and procedures were reviewed. The following policies were not present: -Reportable Incident policy -Active play policy -Screen use policy -Emergency evacuation policy
Corrective action. Please ensure policies are onsite for review.

A licensee must keep, for each child, a record showing the following information: (b) date of enrolment in the community care facility.
Observation. Licensing observed four children's records. Three children's records were missing a date of enrollment. This is a repeat contravention from inspection #MZIN-C56T38

Observation. Licensing observed four children's records. Two children's records were missing an emergency contact other than the parents.

A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency. Observation (CORRECTED DURING INSPECTION): Licensing observed that the fire extinguisher was due for service. It was noted that it was still fully charged.
Corrective action. This was corrected during the inspection with a new service tag.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.
Observation. Licensing observed that one child did not have medical insurance plan number on-site.
Corrective action. Ensure the child's file is updated. During the inspection, the staff reached out to the parent of the child to ask for this information.

Observation. Licensing observed that the CRC documentation for the staff and individuals living in the home were not on-site
Corrective action. Ensure CRC documents are available on-site.

A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. Licensing observed the outdoor play area, and noted that the pointed end of some nails were protruding out from the wooden fence. The pointed ends of these nails were located just above the ground and easily accessible to children.
Corrective action. Ensure children do not have access to objects that may be hazardous to their safety.

A licensee must display in a prominent place in the community care facility both the licence, including any terms or conditions of the licence, and the name of the manager. Observation (CORRECTED DURING INSPECTION): Licensing observed that the name of the manager was not displayed in a prominent place in the community care facility.
Corrective action. Ensure that the name of the manager is displayed in a prominent place in the community care facility.

A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure. Observation (CORRECTED DURING INSPECTION): During inspection, Licensing noted that the sign in/out sheet has not been used for the past week. This item was identified and contravened on June 04, 2019 on routine inspection report #KSEY-BCTNKC.
Corrective action. Ensure that the sign in/out sheet is completed throughout the day on a daily basis. This information is taken with staff during an emergency, and it is used to determine the children who are present and in care.

Corrective action. Ensure that substances that may be hazardous to the health and safety of a child is not accessible.

Observation. The following items were observed and discussed today: - Outdoor fence: Rusty nails sticking out - Washroom: Wooden cabinet door is broken These items were identified and contravened on May 12, 2020 on routine inspection report #JKIM-BPJSUZ.
Corrective action. Please ensure items stated above are in good repair.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. The following items were observed and discussed today: -Outdoor fence: wood rot, paint peeling, missing screws, rusty nails sticking out, missing planks. -Broken Slide: bottom of the slide is broken. -Washroom: wooden cabinet door is broken. -Nap room: corner of electrical outlet wall plate is broken. LO discussed about developing a system to regularly monitor outdoor play space and log all inspections, maintenance and repair as per DOLSOP Safe Play Space
Corrective action. Please ensure that facility's furniture, fixtures and equipment are clean and in good repair.
