Observation. Required information is missing from several children’s records, including start date, gender, immunization status, illnesses and allergies, emergency contact details, and the name and contact information of a medical practitioner. All required information must be obtained and documented in each child’s record. Submit written confirmation by July 6, 2026, that the missing information has been added to the children’s records. Written consent to contact a medical practitioner or ambulance in an emergency is missing for one child. Consent must be in place to allow staff to seek medical assistance when a parent cannot be immediately reached. Submit written confirmation by July 6, 2026, that the missing consent has been added to the child’s record.

Observation. An outdoor plastic ride-on car is cracked and not maintained in good repair. Equipment must be maintained to prevent injury. The item was removed from the play space during the inspection.

Observation. Radon results are required for all licensed facilities. Results have not been provided for this facility.

Observation. The record of the emergency plan practiced in the last twelve months is not completed. A record must be maintained indicating that the emergency plan has practiced at least once annually. Submit written confirmation and supporting evidence by July 06, 2026, that the record has been completed.
