Observation. Allegation: A school age child was left unattended on the school bus after arriving back at the facility following school pick up on January 22, 2026. The bus was observed parked in its spot in the parking lot and was not running. All the staff and the other children were inside the facility. - The arrival of the bus and the unloading of the children into the facility was observed. The bus driver then drove the bus to the far end of the lot and parked in its usual spot. The manager was observed standing inside the entryway door until the children put away their belongings into their cubbies. The children and the manager then went upstairs. Upon entering the facility and proceeding upstairs, some of the children were sitting at tables and some of the children were playing in the main play room. There were 15 children present with 3 staff, including the manager. The manager was informed of the complaint concern. Interviews were conducted with staff who were working in the facility at the time of the alleged incident on January 22, 2026. One staff person, who was working in the facility on the day of the alleged incident, declined to be interviewed. During the interviews, it was confirmed that a child had fallen asleep on the bus while being transported back to the facility after being picked up from school on January 22, 2026. It was also confirmed during the interviews that the child who was sleeping was left on the bus while the staff (including the bus driver) took the other children inside the facility. Based on the information gathered (including interviews with staff and the complainant), and the balance of probabilities, it is determined that continuous supervision of a child in care was not maintained on January 22, 2026. Therefore, the allegation that the licensee contravened Section 39(1) of the CCLR - Continuous supervision required - is substantiated. *A compliance plan detailing how immediate compliance will be achieved and how the licensee will prevent the contravention from re-occurring must be submitted to Licensing by: February 4, 2026.*

Observation. - Based on the information gathered, including staff interviews, it is determined that Licensing was not notified of a reportable incident involving a child in care that occurred on January 22, 2026. Specifically, an incident of a child who was left unattended on the school bus while the child was under the care and supervision of the licensee. This incident meets the definition of a "missing or wandering" child as described in Schedule H of the Child Care Licensing Regulation (CCLR). This contravention is not listed as an allegation in the complaint received. A licensee is required to notify Licensing within 24 hours after a child is involved in, or may have been involved in, a reportable incident described in Schedule H of the CCLR while under the care and supervision of the licensee. Reporting serious incidents to Licensing triggers a review to ensure appropriate, immediate actions were taken and that necessary preventative measures are implemented to prevent re-occurrence. Reporting helps track, analyze, and identify patterns in incidents, fostering a culture of safety and improving quality care standards. *An incident report and written proof that the parents of the child have been informed of the details of the incident must be submitted to Licensing by: February 4, 2026.*

Observation. Allegation #1: The required staff to child ratio for the Group Child Care (School Age) program is not maintained during school pick up. - The manager confirmed verbally to Licensing that, on multiple occasions, they were left alone at one school to pick up anywhere from 11-15 children (depending on the day) while the other staff person drove the bus to another school to pick up another 5 children. After picking up those children, the other staff person drove back to the first school to pick up the manager and the 11-15 children in care. The majority of the children being picked up from school are in kindergarten. As per Schedule E of the CCLR, the minimum required staff to child ratio for a Group Child Care (School Age) program in which any child present is in kindergarten or grade 1, is 1 responsible adult to 12 children (1:12). The staff to child ratio for the program is required to be maintained at all times when the children are under the care and supervision of the licensee. Based on the information gathered, it is determined that the following allegation is substantiated: CCLR, Section 34(2)(b) - Group sizes and employee to children ratios - Substantiated. *Confirmation of this contravention being resolved and how the Licensee will prevent re-occurrence must be submitted to Licensing by: January 23, 2026.* Allegation #2: Continuous supervision is not maintained while the children are engaged in outside play at the facility, while a child is eating food in the entrance way to the facility, and while children are being picked up from school. - The procedure for unloading children from the bus upon arrival at the facility after school pick up was observed. The manager exited the bus first followed by 15 children. The second staff person/bus driver remained on the bus and then parked the bus once everyone was inside the facility. Once parked, the second staff person/bus driver went inside the facility. - The manager confirmed verbally that a few children at a time were recently permitted to line up and take turns riding on a scooter on the sidewalk area near the entry door to the facility. The children are not permitted to go outside the sidewalk area and into the parking lot at any time. At least one staff person is supervising this activity at all times. Another staff person supervises children who are playing in the fenced outside play yard. The main level entryway is where the children's belongings are stored. When the children are upstairs, they must ask a staff person first if they need to go downstairs to the storage cubbies for any reason. The staff person stands at the top of the stairs to supervise. There is also a retractable gate available at the top of the stairs. The children eat food from their lunches at the tables located upstairs and which are supervised by the staff. The manager confirmed that on a few occasions, a child in care did not want to leave their classroom during school pick up time. The manager had to leave the other children who were waiting to depart the school at the end of the hallway while they were responding to the one child. The manager stated that they could still see the other children who were at the end of the hall and that they were never out of the manager's sight. Based on the information gathered during this inspection, a determination has not yet been made regarding the allegation of non-compliance to Section 39(1) - Continuous supervision required. Licensing will continue to follow-up on this allegation and make a determination once the investigation is complete. There are 15 children attending the Group Child Care (School Age) program with 4 responsible adult staff, the manager, and a floater staff/bus driver present.

Observation. - Based on the information gathered, it is determined that some of the licensee's current policies and procedures specific to the care and supervision of children were not implemented by the employees. Specifically, the licensee's Active play/Screen time policy and the Off site/Transportation safety plan. This contravention is not listed as one of the allegations in the original complaint. Ensuring that employees implement facility policies and procedures promotes consistent practices, confirms that the staff know their roles and responsibilities, minimizes risk to the children in care, builds trust with parents, and demonstrates adherence to legislative requirements. *Correct by: January 23, 2026.*

Unless weather conditions make it unreasonable to be outside and/or special instructions regarding a child playing outdoors has been identified in a care plan, all of the children in care need to participate in active play outside. Active play outdoors in the fresh air provides a vital break for children and recharges them for learning, fosters a connection with nature, and improves overall health and mental well-being. *Correct by: January 23, 2026.* - Based on the information gathered, it is determined that the children are offered "movie day" every Friday. The movie offered to the children varies in length from 1 to 1.5 hrs. The Group Child Care (School Age) program operates from 2:45pm to 5:30pm. As per Standard #5 of the "Director of Licensing Standards of Practice - Active Play", programs where children are in attendance for 3 hours or less should not include screen time activities in the daily routine. The use of screens may be permitted, however, for special occasions or for educational purposes, provided that any screen time is supervised by the staff and the time spent on screens is limited as per the Active Play Standards. The licensee's Screen Time policy must outline when and why screen time will be offered and how it will be monitored. This contravention is not listed as one of the allegations in the original complaint. *Correct by: January 23, 2026.*

The hot water temperature at one water cooler measured at 90°C and the hot water temperature at the second water cooler measured at 85°C. Based on the information gathered, it was determined that the following allegation is substantiated: CCLR, Section 47 - Heated water - Substantiated. *Confirmation of this contravention being resolved and how the Licensee will prevent re-occurrence must be submitted to Licensing by: November 6, 2025.*

In order to ensure that fire drills are practiced and recorded monthly as required, the record must be displayed in a prominent place in the facility. *Correct by: October 17, 2025.* The Manager confirmed that the required policies and procedures for the facility were still in place and that they had not been revised or updated since the facility became licensed.

Some information in the children's emergency records, such as care card numbers, doctor/clinic contact information, and an emergency contact other than the child's parents was missing. To ensure that the staff have immediate access to the children's personal information while taking the children away from the facility and/or in the event of an emergency evacuation, this information must be complete and up to date. *Correct by: October 23, 2025.* - A child's record was missing written permission from the child's parent to call an ambulance. Written consent from the child's parent must be obtained in order for the Licensee to call for an ambulance in the event of an emergency. *Correct by: October October 23, 2025.* Other records such as the daily attendance, minor incident logs, and the play space inspection log were reviewed and they appeared satisfactory.
