Observation. Upon reviewing employee records, there was no evidence observed reflecting that educator's and assistant's certificates are verified with the Early Childhood Educator Registry. Ensure a person must not be employed as an educator or assistant unless they hold a certificate which must be verified by the Licensee using a system established and maintained by the director of the Early Childhood Educator Registry.

Observation. The enclosed outdoor play space has a grey rock climbing play structure which requires protective surfacing to reduce the risk of injury from falls. The Licensing Officer notes outdoor climbing play structures and resilient surfacing have been discussed in the past.

Observation. A storage shed in the outdoor play space has several holes in the siding. The holes have sharp, jagged edges which pose a risk of injury to children.

Observation. C1.1F - Regulation 11(3) - The child registration records were observed today with an option to sign up for full day preschool which is not an applicable service to offer. Each child may only attend a preschool program for a maximum of 4 hours per day.
- C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical
Observation. C7.1AH - The enclosed outdoor play space has a yellow slide that requires resilient surfacing to reduce the risk of injury. Licensing notes this piece of equipment has been discussed in the past. The Director of Licensing Standards of Practice - Safe Play Spaces document has been discussed and provided with this report. C7.1AN - The staff advised that weekly playground checks are being conducted. It is required to conduct daily inspection and maintain a log.
- C3.1 - Does the facility have sufficient employees who possess adequate training and experience to meet the care, supervision, and needs of the children?Non-critical
Observation. C3.1L - Regulation 22 - The annual evacuation drill has not been conducted for the Preschool or Group Child Care (30 Months to School Age) program staff. These two programs operate from September to June. Licensing notes this contravention may not be resolved until the programs resume in September 2022.

Observation. C4.1E - Regulation 57 - There are children enrolled that have special instruction (care plan) in writing from the parent regarding a potential medical situation. This instruction should be thorough so any staff is able to identify, assess, and manage an event. A couple records were observed with unclear instruction or discrepancies between medication to administer and what is available on site. C4.1J - Regulation 57(2) - The system this facility has implemented is to have child emergency cards with attached photos. A couple child records were missing emergency cards and/or photos. The program staff has digital images however they do not have a means of identifying the child's full name.
- C9.1 - Are there plans and actions in place to ensure medications are stored and administered properly?Non-critical
Observation. C9.1C - Regulation 53 - Upon entering the facility the program backpack was present on a low hook with the front pouch open with a prescribed inhaler and an un-prescribed epi-pen accessible. Licensing notes this backpack was moved to a high ledge during the inspection.

Observation. C2.1B - Regulation 22 - The emergency plan identifies food supplies which were observed today beyond the expiry date. C2.1G - Regulation 56(1)(d) - Licensing observed an active play policy specifically for the Preschool program however there was not one available for the Group Child Care (30 Months to School Age) program. A resource was provided during the inspection. C2.1I - Regulation 56.1(2)(a) - Child registration forms identify that parents pre-pay for services on the 1st of the month. There is no policy specific to withdrawal or termination and the process for refunds/repayments.

- C1.1 - Does the licensee continually inform licensing of any significant changes to the structure or operation of the facility?Critical
Observation. C1.1D - Licensing observed the correct manager name posted. C1.1E - The upper level program room continued to have the Community Care Licence and manager name posted in a non-prominent place - Licensing notes this was resolved during the inspection with the documents removed from the non-prominent location and they will be relocated to the program entrance/cubby area. C1.1F - The facility website has been updated to reflect preschool only being offered in the morning from 8:30 - 11:30. C1.1G - The upper level program did not have the ECE licence to practice certificate posted; the document that is posted is an ECE course completion certificate and it was removed during the inspection. The staff ECE licence to practice was observed in the staff file and expired in 2019. The staff will be required to provide a valid ECE licence to practice certificate and it will be verified by management and posted in a prominent place. An alternate ECE staff is available if necessary.

Observation. C4.1D - The facility has a separate book to log minor accidents, illnesses, and unexpected events. C4.1E - Applicable child records are now complete with special instruction in writing from the parent and correct enrolment dates. C4.1J - This contravention was observed on July 28, 2021 and it was noted that there were no child photos available. Licensing would like to acknowledge that child digital images were available however there was no means to identify the child on the digital images to cross reference with their emergency information. The associated risk assessment has been revised. This contravention has been resolved and currently there are child emergency cards with an attached photo for all children in care.

Observation. C7.1L - During this inspection a bottle of cleaning product was available in the upper level program storage room that was not inaccessible to the children. This item was made inaccessible during the inspection and education was provided regarding an "off limits" area and "inaccessible". A storage closet with hazardous items was accessible during the previous inspection and it has since been locked and a sign posted to keep locked at all times.

Observation. C1.1D - Regulation 11(1) - A manager name is posted however it is incorrect as to who the current manager is. C1.1E - Regulation 11(1) - The preschool and group child care (30 months to school age) program stores the community care licence in a mail slot and it is not posted prominently. C1.1F - Regulation 11(3) - The facility is advertising and offering full day options for preschool on their website. The preschool program only operates from 8:30 - 11:30. C1.1G - Regulation 11(4) - The preschool room does not have the ECE educator certificate posted. Note: There has been staff/organizational changes recently that have not pro-actively been reported to licensing.

Observation. C2.1B - Regulation 22 - There is an emergency evacuation plan in place however one of the relocation sites is periodically closed throughout the year. The emergency evacuation bag in school age program contains expired snacks and the emergency evacuation bag in the preschool is lacking water.

Observation. C3.1C - Regulation 19 - A staff file was missing a current criminal record check. Another staff file did not contain a certificate/evidence of the persons qualification/skills.

Observation. C4.1D - Regulation 56(f) - There was a general communication book present at the facility that does not meet the intent of a minor incident log book. Please ensure there is a specific book to note minor incidents, illnesses, or unexpected events. C4.1E - Regulation 57(b)(h)(k) - A few child records were missing special instruction in writing from the parent regarding a potential medical event. Also multiple child records did not contain a date in which the child/ren were enrolled. C4.1J - Regulation 57(2)(i) - All of the children attending the summer school age program did not have photos to readily identify them in the event of an emergency.

Observation. C7.1L - Regulation 17 - The lower level school age program had an accessible storage room that contained paint thinner and paint. Licensing notes this room was immediately locked during the inspection. The upper level summer school age room had cleaning products accessible to the children.

Observation. Fire extinguisher's annual service was due June 2019. -Fire drill system posted in prominent location. -Fire drill and annual emergency evacuations plan appears to be practiced. -Licensee contact recently updated the repayment agreement based on inspections at other Appletree facilities. Please ensure an updated version is available at this site as well.
- C7.1 - Is the facility safe, sanitary, free from hazards, in good repair, and appropriate for the age and development of children?Non-critical
Observation. -Hot water temperature in bathrooms measured at 54 degrees Celsius exceeding the maximum of 49 degrees. -Lysol wipes were accessible to children located in an unlocked under sink cupboard in the upstairs bathroom. Facility, equipment and toys inspected appear to be age/ developmentally appropriate, safe, clean and in good repair. The outside play space was not inspected due to snow level.

Observation. Digital images of children were not accessible to Licensing as they are kept on a cell phone which was not charged. Please ensure there is a system in place in which the cell phone used to store these images is charged during all operational hours. -Child records reviewed appeared complete. -Minor incident log book appears to be complete and current. -Play space inspection logs appears to be complete and current. -A minimum of an annual review of care plans was discussed with manager. The "care plans" in place at this facility are more accurately described as special instructions as the children do not require extra support but have dietary and medical conditions.
