Observation. - There is a large wired dog kennel accessible to children in indoor play space that contains torn blankets exposing various small pieces of stuffing. This poses a potential choking hazard to children and can contribute to poor sanitation. All areas accessible to children must be maintained in a clean and hazard free condition. Submit a response with evidence of compliance by April 29, 2026. - It is identified that two children share a mattress to sleep/rest on. It is important to ensure that each piece of furniture or equipment used for sleeping is designated for only one child at a time in order to reduce the risk of cross contamination and the spread of communicable diseases. Observation. - A few child records are missing the date on which children stopped attending the facility. Recording children's end date in the program ensures accurate enrollment records and proper record retention. Maintaining up to date emergency contact information for each child ensures that appropriate individuals can be promptly notified in the event of an illness, injury, emergency or unforeseen circumstance. Observation. The Licensing Officer observed that the manager's name was not displayed in facility at the time of inspection. Ensure the name of the manager is displayed in a prominent place at the facility.

Observation. The Licensing Officer reviewed child records and observed missing information including immunization status, name and telephone number of medical practitioner, and emergency contact information. Ensure children's records are kept complete.

Observation. • Upon reviewing facility records, Criminal Record Check clearance letter for the Licensee was found expired. This must be addressed immediately as valid criminal record checks must be available at all times. • A substitute employee's file was reviewed and was observed to be missing references and immunization status. Ensure all required documentation is received prior to this employee working with children again.

Observation. The stool used for handwashing in the bathroom has a smaller stool stacked on top. The stools are not sturdy. This poses a fall risk for children as the standing surface is not secure. A new playground structure has been added to the outdoor play space. The structure is taller than 18 inches. There is no resilient/protective surfacing under and around the play structure. Rain barrels are stacked on top of wooden pallets in the outdoor play area. The rain barrels are hanging over the edge of the pallets and pose a risk to children if they were to fall.

Observation. C3.2A - Reg. 19 - Item remains outstanding - During the routine inspection on October 26, 2022 the Licensee was informed by Licensing that their Early Childhood Educator certification was to expire in February 2023. On the March 7, 2023 inspection the Early Childhood Educator certification is expired therefore the Licensee does not meet the minimum staff qualifications for Multi-Age Child Care. The Licensee was required to submit a compliance plan by March 13, 2023 by 8:00 am. A plan was received on March 13, 2023 at 5:15 pm advising only a maximum of 7 children would be in attendance as the Licensee is qualified as a responsible adult. On the March 14, 2023 inspection Licensing advised that the Community Care Licence is for Multi-Age Child Care and the staff requirement remains to be a qualified ECE. On the March 29, 2023 inspection the current compliance plan is that the facility is closed until March 31, 2023. The Licensee has submitted their Early Childhood Educator certification renewal to the ECE registry for processing as well as submitted a staff exemption request to Licensing which is currently in process.

Observation. C10.2C - Reg. 34 - Item remains outstanding - During the March 7, 2023 inspection there were 9 children present when the facility is licensed for a maximum of 8 children at any one time. Of these 9 children there was more than 5 children under the age of 36 months. On the March 13, 2023 Licensing received a schedule of attendance for operational days Tuesday - Friday. During the March 14, 2023 inspection the Licensee was present with 7 children as identified on the schedule. On the March 29, 2023 inspection Licensing observed a schedule of child attendance with a system to identify children that are under 36 months of age. There were no children in care today and prior to this contravention being resolved Licensing will observe with children in care.

Observation. C3.1H - Reg. 20 - Item remains outstanding - During the inspection on March 7, 2023 a person over the age of 12 was present and had been regularly present who does not have a valid criminal record check. On March 14, 2023 the person over 12 was not present in the area where care was being provided however the licensee was in process of submitting the required criminal record check and awaiting verification. On March 29, 2023 the Licensee advised the additional criminal record check has not been submitted due to difficulties with the online submission process. Licensing made arrangements with the Licensee to have this check submitted through a manual process with the support of Licensing Direct. Licensing Officer T. Ruhr received confirmation after the inspection confirming this had occurred and the result is pending. C3.1M - Reg. 23 - Item remains outstanding - During the routine inspection on October 26, 2022 the Licensee was informed by Licensing that their First Aid certification expires in February 2023. On the March 7, 2023 inspection the Licensee's First Aid certification was observed expired. On March 7, 2023 the Licensee submitted confirmation of enrollment in a First Aid course on April 3, 2023. A plan was submitted to Licensing on how children will have access to someone with valid first aid certification until the course is completed. On March 29, 2023 this item remains outstanding as the course date has not occurred. The facility is currently closed until April 4, 2023. Please provide evidence of the course completion when achieved.

The licence is attached to the physical address and any care provided is deemed to occur in the Community Care Facility. On March 13, 2023 Licensing received a schedule of attendance for operational days Tuesday - Friday. During the March 14, 2023 inspection the Licensee was present with 7 children as identified on the schedule. Licensing notes that the schedule for other days identifies non compliance with Section 34 and Schedule E of the Child Care Licensing Regulation. Please submit a revised schedule of attendance.

The Licensee advised that a parent has not returned completed documentation and Licensing advised that records are to be on site prior to the child receiving care. Ensure all children enrolled and in attendance have complete records on site as identified in section 57 and 59 of the Child Care Licensing Regulation. Licensing notes this has been a noted contravention during previous inspections.

Of these 9 children there was more than 5 children under the age of 36 months. Licensing notes this has been a noted contravention during previous inspections and the child schedule of attendance submitted to address the contravention from the October 26, 2022 inspection is not reflective of what is occurring. The Licensee advised that the other person present over the age of 12 was "babysitting" two of the children. Licensing advised that the licence to operate is for the "premise" and it is a maximum of 8 children at any one time. The licence is attached to the physical address and any care provided is deemed to occur in the Community Care Facility. Resources have been provided with this report.

Observation. C10.2C - Regulation 34(2)(b) - There was a total of three children in attendance today and all were under 36 months. Upon review of children enrolled and attendance records it was observed that there are occurrences where more than three children under 36 months are in attendance. This is non-compliance with Schedule E; please identify how you will ensure ratios are maintained at all times. Licensing is additionally requesting a schedule of attendance for October 28, 2022 - November 4, 2022.
- C2.1 - Are written policies and procedures in place to guide employees in all matters regarding the care and supervision of children?Non-critical
Observation. C2.1A - Regulation 22(1)(a) - Upon inspection today the fire extinguisher is overdue for servicing. Fire extinguishers must be professionally inspected and tagged once a year. C2.1B - Regulation 22(1)(b) - There is an emergency evacuation plan that includes emergency supplies. Upon review of the emergency supplies, expired food was present. Please advise what system has been implemented to ensure supplies are adequate and within the expiry dates. Licensing notes the annual evacuation drill has not been implemented and logged for 2022.

A couple records were missing child photos, one was missing a medical number, and one child was missing special instruction in writing from the parent regarding a potential medical event. Observation. C7.1 - Reg. 17 - The nap room had a corded baby monitor that was not secure and accessible beside a playpen. Please secure or relocate. An electrical outlet in the play room was accessible to the children without plug safety covers. Only the lower level of the home is used for multi-age child care. Children have assigned playpens/cribs for napping and bedding is laundered at the facility weekly. There are three dogs on the premise that integrate with the children.

C4.1 - Reg. 57(1)(2)(i) - A couple child emergency cards did not contain child identifying information such as hair color, eye color, visible birth marks, etc. The Licensee is conducting outdoor inspections.
