    Observation. On October 25, 2024 licensing observed the following: Lower kitchen cabinet fronts and doors had chipped and broken laminate and are no longer in good repair.

    Observation. 1) Lower kitchen cabinet fronts and doors had chipped and broken laminate and are no longer in good repair. 2) The activity room had a large hole in the drywall. When the door is opened the handle hits the wall.
