Observation. The following items remain outstanding from the July 14, 2025 routine inspection: 1) Walls had chipped drywall in some areas of the facility. The largest worn areas were in the kitchen area by the build in bench and in the nap area. Chipped areas are no longer in good repair and cannot be properly cleaned. A discussion was held with the Licensee to start addressing these areas. 2) One baseboard coving strip beside the kitchen door was missing. 3) Dirty heavily stained cushions on the entry bench.

A discussion was held with the Licensee on July 14 that this is not a secure permanent solution and to either remove the electrical cords or put a child proof gate.

This poses a risk of a child leaving the yard unnoticed as there is no line of supervision along the side of the house. 2) Some of the wood fence slats are loose and rotten at the bottom with protruding screws. 3) The gate latch on the north side of the house cannot be properly closed as the latch does not properly secure over the bolt. On August 13, 2025 licensing observed the following: 4) The gate to the back alley was unsecured, posing a risk of a child exiting the outdoor play space. The gate was secured at the time of inspection.

Observation. Licensing observed an unsecured bi-fold closet door leading to the hot water tank. Licensing was informed that the lock is broken. This poses as a hazard. Licensing observed large items on top of one another including puzzles, plastic toys, and other various materials stored in the window sills in the second large play room. This poses as a hazard. Licensing observed the main entry gate to the outdoor play area open on arrival. Licensing observed the gate to be open again when leaving the facility. This poses as a hazard as children may be able to leave the outdoor play space unnoticed. Licensing observed a large and heavy folding table leaning up against a wall in the outdoor play space. The table was unsecured and poses a risk of injury.
