    Observation. On July 28, 2025 licensing observed cracked and missing tiles on the backyard deck posing a tripping hazard. Some loose broken tile pieces with sharp edges were scattered on the deck. They were removed during the inspection. On August 25, 2025 licensing observed cracked and missing tiles on the backyard deck posing a tripping hazard. Some loose broken tile pieces with sharp edges were scattered on the deck. They were removed during the inspection. On October 28, 2025 licensing observed that this item has not yet been completed. The manager informed licensing that they were unable to find tiles that fit. They unsuccessfully tried to even out the surface with concrete patch. They are in the process of trying a different compound to fill in the gaps.

    Observation. Licensing observed very limited outdoor materials and activities in the outdoor environment. There was a sandbox with no sand and no toys, a plastic slide, a plastic rocking horse, a ground level playhouse and a water table with no water and no toys. Currently Schedule G is not met with regards to the outdoor activities.

    Observation. Licensing observed a blind cord in a gross motor room that was accessible. This poses as an entanglement hazard. Licensing observed a mattress and a disassembled bed frame on the third floor of the house. Licensing inquired if there was anyone living or residing on the premise. Licensing was informed that that no one is residing on the premise at any time.
