Observation. Licensing observed the two drain grates secured with duct tape to address the previous infraction. During today's visit, the duct tape was peeling and had several slits. When standing on the taped drain grates, they appears to sink and were not securely taped. This observation was shared with the program manager - she explained that the duck tape were partially removed to prevent water pooling during heavy rainfall. When the duct tape was fully lifted, the underlying metal drainage grates appeared damaged and had a hole. The metal grates are no longer in good repair and free from hazards - this could impact its intended functionality and pose as a safety hazard.

Observation. On June 16 2025, Licensing observed the following: *2 drain covers taped with duct tape. The duct tape appeared to be worn and no longer easy to clean. *A kitchen set with a broken microwave. This item is no longer in good repair. - this item was addressed. *A damaged coat rack/ hooks with shelf. The wooden shelf appeared to be broken and damaged. During today's visit, the program manager informed that the maintenance request has been submitted to VSB to address the remaining outstanding items.

Observation. During the afternoon playground safety check, staff noted the south east entry to the playground next to the climber (on 12th) was damaged. Principal and VSB facilities was informed. Lead staff was informed that the repair would be forthcoming. Currently the gate is held together with a chain and padlock.

A licensee must not employ a person in a community care facility as a responsible adult unless the person holds the qualifications required under Division 2.
Observation. In reviewing the Child Care Staff List noted that one of the staff does not have their Responsible Adult qualifications. Lead staff informed me this person is in the process of obtaining their training online. CCFL has discussed this issue with MPNH in the past (see inspection reports for March 2016 and March 2017). Staffing exemptions must be applied and approved prior to unqualified staff working with the children. A staffing exemption can allow recently hired staff without full RA qualifications to work in place of RA while they are taking their training. However the staffing exemption must first be applied for and approved by CCFL.

The entire outdoor play area is enclosed and constructed in a manner that is suitable for the age and development of children, and ensures that children are free of harm. [CCLR Sec. 16(3)(a)(b)]
Observation. During my inspection of January 22, 2021, it was noted that the resilient ground cover in the southeast corner of the play yard (under the play structure) had shifted. The fibar had drifted away from the high impact areas where it was needed the most. At that time, it was noted that while it was not an immediate health and safety risk, the fibar should be redistributed to the high impact areas and if lacking then replenished. During my inspection today (September 21, 2021), noted the condition of the fibar has worsened to the point where the landscape membrane cloth is exposed and there is a potential health and safety risk to children using the equipment. The lead staff reported that the maintenance request of January 2021 was never addressed by the VSB, i.e. no one came to attend to the redistribution or top up of the fibar. I expressed surprise as the VSB facilities folk typically engage in such maintenance over the summer.
