    A healthy and safe environment is provided at all times [CCLR Sec. 13(1)].

    Observation. Two step method of cleaning and sanitizing was used in the Big Y room, before snack and in between groups having snack. Noted the spray bottles are stored in the kitchen out of reach of the children. Appliances are reportedly in satisfactory working order. Staff informed me that about two weeks ago, during outdoor playground inspections, they noted a hole in the fence. Manager said they reported this to the principal at least 2 times. The hole in the chain link fence is still there - on the east side of the dirt playing field, just north of the two gates along St Georges Street. The hole is large enough for a child to slip out unnoticed. CCFL concurs with YMCA staff that this is a safety & supervision hazard.

    The facility, the furniture, equipment, fixtures and supplies are of sturdy and safe construction.  They are easy to clean, and are clean and in good repair and free from hazards. [CCLR Sec. 13(2) and Sec. 15(1)(a)]

    Observation. Overall the furnishings and equipment for the YMCA program spaces are clean and in good repair. One exception is the elementary school's Family Washroom that the mini Y children are to use. Observed children accessing the washroom as per protocols and child came out saying the room "stunk." Indeed the room smelled of soiled diapers. There appeared little to no capacity for ventilation and the garbage had not been removed from the day. The washroom is equipped to facilitate school children requiring toileting assistance and diaper changing. There is a long vinyl covered table along the wall next to the toilet and steps up to the sink with handrails. While the school is older and one cannot expect to turn back the hands of time, the condition of this space must be improved. The space requires a deep clean and repair of peeling paint along the walls. Greater attention must be paid to the overall cleanliness and regular removal of soiled items to reflect compassion and care for the health, hygiene and dignity of users.

    No child is subjected to emotional abuse, physical abuse, sexual abuse or neglect as defined in Schedule H. [CCLR Sec. 52(2)]

    Observation. On September 12, 2025, staff team and Manager failed to provide due care and supervision to a child enrolled in their care. A staff marked a child absent who was in fact with their group. The Manager left phone tracking late and did not do a physical head count/ face count to confirm accuracy of sign in sheet, even after father reported child was meant to be with program. The child was at that point "missing" but staff team and Manager did not follow missing child protocols. It was only when grandparent arrived that staff team and Manager realized child was in fact present. There was a lapse of 25 minutes from time Manager received sign in sheets, began calls & the grandparent arrived. During this time, the father experienced undue stress and hardship as he was left thinking his child may be unaccounted for. CCFL concurs with the Licensee that a lack of staff communication and Manager oversight were contributing factors. CCFL finds staff and Manager's actions or lack of action meet the definition of neglect, i.e. failure of meet the needs of the child, including care or supervision.

    The policies and procedures referred to in subsection 1 are implemented by the employees. [CCLR Sec. 56(2)]

    Observation. The staff team and Manager failed to follow written procedures as provided by the Licensee (YMCA) including sign in and tracking procedures, care & supervision, incident reporting and missing child procedures. A staff marked a child absent who was in fact with their group.     Licensing is notified within 24 hours after a child is involved in, or may have been involved in, a reportable incident described in Schedule H or has a reportable communicable disease. [CCLR Sec. 55(2)(a)(b)]

    Observation. On July 17, 2025, CCFL met with the Manager to review reportable incidents and reporting requirements. Hand outs were provided. Scenarios were worked through, including those involving a missing child. The meeting was in relation to prior incident where the Manager and staff team failed to notify CCFL of a reportable incident. Manager assured CCFL they understood what a reportable incident was and reporting requirements under the CCLR. The Manager further assured CCFL that they would apprise staff of this understanding. Despite this additional support and the YMCA's internal policies and procedures, the Manager and staff team failed to report the incident of September 12 ,2025. This is contravention of Section 55 (2) as noted above.

    Observation. LO noted a child listed with medical condition did not had a care plan. Staff working directly with the child was not aware of the medical condition. LO will provide a care plan form.

    Maintenance of Equipment: A log is kept of all inspections, maintenance and repairs. [Director of Licensing Standard of Practice - Safe Play Space]

    Observation. The outdoor play space was not checked on the day of the inspection. When LO asked staff who inspected the outdoor play space and where is the record,staff stated that the outdoor play space was not inspected. Staff appeared not to be aware of such requirements;one staff stated that the team lead did not provided training. The team lead -manager was not present at the inspection. During the manager's absence no inspections and records were maintained. The log for the outdoor play space inspections was not available for licensing review and staff were not aware of it.

    Staff are trained in the implementation of the fire drill system and the emergency plan, including the use of any equipment noted in the fire drill system and emergency plan. [CCLR Sec. 22(2)(a)]

    Observation. No written fire drills and emergency drill records were available for review. One staff stated that some drills were practiced last year,however fire drills were not practiced monthly. One staff stated that the team lead provided very little training.

Staff informed Licensing that the fire drill has not been done in September, and were unsure when the last emergency drill will practiced.

    Observation. Today inspection revealed several children for whom the registration documentation indicated that they need emergency medication for a life threatening allergy, asthma, and other allergies, however care plans were not kept on file.
