  Observation. The Licensee’s criminal record check expired earlier this year. The other ordinary present person holds a valid check expiring on November 26, 2029. Licensing to provide information for submitting a new criminal record check.

  Observation. A child who was present today had a valid medication; however, a care plan has not been developed or obtain from the parent.

  There is current log of minor incidents, illnesses and unexpected events, that did not require medical attention and were not reportable incidents described in Schedule H. [CCLR Sec. 56(1)(f)]

  Observation. The log of minor incidents has not been use regularly. A conversation occurred regarding information to be considered when recording minor incidents in the log of minor incidents: - Date and time, child's name, number of children in care, type of injury, location, equipment involved, if any, first aid applied, who was informed and at what time, witness, etc. - Challenging behaviors - Unexpected events

  A record for each child containing a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency is not kept. [CCLR Sec. 57(2)(i)]; A record for each child containing the name, sex, date of birth, medical insurance plan number and immunization status is not kept. [CCLR Sec. 57(2)(a)]

  Observation. Licensing observed that records was not kept for the licensee's own child who attend the facility.

  The licensee has written parental consent to call a medical practitioner or ambulance in case of accident or illness, in cases where the parent cannot immediately be reached. [CCLR Sec. 57(3)(a)]

  Observation. Licensing observed emergency contacts were not on file for two children.
