Observation. Discussion with the facility manager and review of employee files, indicates that criminal record check (CRC) results have not been received for a substitute teacher who has occasionally been present at the facility. All required records must be obtained prior to employment. Notify Licensing when the CRC results have been received.

Observation. A review of a random sampling of children's files found one child's file to be missing the name and phone number of an emergency contact person. This is required information. Notify Licensing when an emergency contact has been added to the child's record.

Observation. Evidence of training relevant to child development is required for one responsible adult on the casual list.

Observation. Fire drill practice was not recorded for January. A system for ensuring fire drills are practiced each month is required.

Observation. Gaps between both gates and the fence in the outdoor play space are required to be 3.5" or less. Toy bin in the outdoor play space requires venting in case of accidental entrapment.

Observation. There is no record of employees having practiced the emergency plan annually. A system for practicing and recording the emergency plan is required.

Observation. Log of minor accidents, illnesses and unexpected events is incomplete and inconsistent. Sometimes notebook is used, sometimes day-timer is used. A consistent system for noting the date, time and details of minor incidents is required. A child who is entering kindergarten in September requires a care plan, so that staff have direction to meet the child's unique needs.

Observation. Manager explained that bleach sanitizing solution for tabletops and general disinfecting is changed weekly. Bleach solution must be created fresh daily to maintain its sanitizing properties.
