Observation. Children in the Multi-Age Child Care program are using a portable toilet located in their outdoor play space. Management advised that if children wash their hands they use an outdoor sink which is located on the other side of the Group Child Care (30 Months to School Age) and Preschool programs outdoor play space. Combining children from different licensed care programs is not permitted under Schedule E of the Child Care Licensing Regulation and may pose a risk to children as child to staff ratios would not be maintained. Submit by June 23, 2026, a written response outlining how ratios will be maintained.

Observation. A review of daily attendance records and discussions with management indicate that children are being transferred to another facility when staffing levels at this location are insufficient to maintain the required child-to-staff ratios. This practice represents a change to the employee plan that was submitted with the application to amend the licence. At that time, Licensing was provided with information indicating that employee levels were appropriate and could be maintained for the care programs being offered. Submit, by June 26, 2026, notification of a Schedule B change reflecting changes to the staffing plan.

Observation. Management reported that children using the outdoor portable toilet may not consistently wash their hands afterward, as no handwashing sink is located adjacent to the unit. Handwashing after toileting is essential to reduce the spread of germs and support proper hygiene practices. Submit by June 26, 2026, a written response outlining how staff will ensure that children are given opportunity to wash their hands after using the outdoor portable toilet.

Observation. A review of several care plans identified outdated and inaccurate information, including incorrect birth dates and program room assignments. In addition, several care plans do not identify individualized goals for the child or describe the strategies educators would use to support achievement of those goals. Care plans must contain current and accurate information and clearly document individualized goals and the supports implemented to assist each child. Submit, by June 26, 2026, evidence that all care plans have been reviewed and updated to include complete, current, and individualized information. The care plans reviewed do not contain documentation demonstrating ongoing compliance with care plan requirements. Maintaining records of compliance supports consistent implementation, allows educators to evaluate the effectiveness of strategies, and promotes informed decision-making to support positive outcomes for children. Submit, by June 26, 2026, a written response outlining how compliance with care plans will be documented and monitored. Although educators conduct daily outdoor play space inspections, a thorough inspection checklist has not been developed. A thorough inspection checklist is necessary to ensure all staff consistently assess all required elements and to support timely maintenance and repair of identified hazards. Submit by June 26, 2026, evidence that thorough daily, monthly, and annual outdoor play space checklists have been developed.

Observation. A review of daily attendance records and discussions with management indicate that children are being signed out of this facility and signed into another facility to participate in enrichment programming and to provide a change of environment. Documentation reviewed identified that one two-year-old child was transitioned between two different facilities on four occasions within a single day. Frequent transitions for children of this age may not support their health, safety, and dignity. Repeated changes in caregivers, peer groups, routines, and environments may contribute to stress, anxiety, feelings of uncertainty, difficulties with emotional regulation, and behavioral changes. Submit, by June 26, 2026, a written plan outlining how risks associated with frequent transitions will be mitigated.

Observation. A review of fire drill records indicates that monthly fire drills were not conducted for a period of three months. Conducting fire drills at least once each month is required to ensure that staff understand and can effectively implement emergency procedures in the event of a fire. Submit, by June 26, 2026, a written response outlining how monthly fire drills will be consistently conducted and documented.

Observation. The Multi-Age Child Care program's outdoor play space contains heaved asphalt at the base of the stairs used to access the play space. This condition may create a tripping hazard for children using the stairs. Submit by June 26, 2026, a written plan outlining how this risk will be mitigated. Clean blankets used during nap time, children's drinking water bottles, and a jug used for play are stored adjacent to a toilet. The proximity of these items to the toilet may present a health risk, as airborne aerosol particles generated during flushing can travel beyond the immediate toilet area and settle on nearby surfaces. Submit by June 23, 2026, a written response outlining how this risk will be mitigated. A portable toilet located in the outdoor play space is not being adequately maintained. It appears to be unsanitary, contains human waste and toilet paper up to just below the toilet seat, and emits strong odours. The manager was unable to identify when the portable toilet was last emptied or who is in charge of ensuring it is emptied, cleaned and sanitized on a consistent basis. This condition presents a significant health and safety risk by exposing children to unsanitary conditions and potentially harmful air quality. Submit by June 23, 2026, a written response outlining how this risk will be mitigated.

Observation. Several classrooms contain toilets within the activity areas, with no partitions or barriers to provide privacy for children using the facilities. The absence of privacy does not support or promote the dignity of children in care. Submit, by March 19, 2026, evidence on how privacy will be provided for children using the toilets in order to maintain their dignity while using the washroom.

Observation. Several classrooms have toilets located within programming areas with no separation from children’s activity spaces. Direct access to toilets within activity areas may pose potential risks to children’s health and hygiene. Submit, by March 19, 2026, evidence demonstrating how the risk associated with children’s direct access to toilets within programming areas has been mitigated.

Observation. Rooms providing the care program type Group Child Care (Under 36 Months) do not have the required separate sleeping area located away from activity spaces. Sleep is critical for early brain development, growth, and emotional regulation. Inconsistent or interrupted sleep environments can negatively affect mood, development, and overall well-being in infants and toddlers. Submit, by March 19, 2026, evidence demonstrating how a separate sleep area has been provided for children in the Group Child Care (Under 36 Months) program.

Observation. Thermometers in the refrigerators of 7 of the 14 classrooms indicated temperatures higher than the safe food storage temperature of 4 degrees Celsius. A system for ensuring safe storage of children's food is required. Inform Licensing of the method of ensuring safe food storage that will be used.

Observation. During the inspection children in one of the fourteen classrooms were observed to be eating a snack without having washed their hands. Handwashing is known to be the most effective way to reduce the spread of infection. Inform Licensing of your system for ensuring all programs follow the rules of health and hygiene.

Observation. One cabinet that has access to items that may be hazardous to children, was left unlocked after recently being painted. This cabinet was made inaccessible during the inspection. This contravention has been corrected.

Observation. During a review of a random sampling of staff files, it was noticed that one staff member has a criminal record check that is not from the Criminal Record Review Program.

Observation. LO observed a bottle of bleach and 2 bottles of dish soap in an unlocked cabinet under the sink in Room 4. Licensee removed the bleach immediately. A system for ensuring that hazardous items are not accessible to children is required. LO observed standing water (without plugs in) in both sinks of an upstairs classroom. The sinks appear to drain into each other. This contravention was noted on the previous inspection, and was reportedly corrected. A more permanent solution to the problem is required. The bark mulch (protective surfacing) directly under the outside climbing dome has dispersed and must be replenished.

Observation. A new sink has been installed in classroom 8. When the tap on the sink on the right is turned on the sink on the left fills with water from the drain. There are additional sinks in the building available for use There are gaps between the building and the railing/fencing on the fire escape - children would only have access to the fire escape during emergency evacuation of the building. The FLO verified the floor plans and determined that there is sufficient space to add a service type of Group Child Care (30 months to School Age). When verifying the plans , the measurements did not reflect the physical attributes of the space. The FLO measured the space and provided the measurements to LC. LC will update and submit revised floor plans for the amendment file. There is sufficient outdoor play space for the maximum capacity of the facility. At the time of the inspection there was no physical barrier between the areas assigned to each service type. Ensure that when no physical barrier separates the play spaces, only same service types are outside in the outdoor play area at the same time.
