Observation. The following was noted: -Small fleece blanket placed on top of diaper change mat. Blanket is not non porous or easy to clean in between uses. Diapering mat underneath the blanket has some rips which also does not allow for it to be easy to clean. -High shelving within both programs is open and do not have a lip to keep items on the shelf from falling. Items on shelf can potentially act as a topple hazard.
Corrective action. Please provide a written response to indicate how you will address the contravention noted above on or before September 18, 2025

Observation. There are cleaning supplies stored in an unlocked closet right beside the bathroom. Manager stated that there is a stop sign posted on the door so children know not to open the door. Discussion had with Manager that this is not enough as some children in care cannot read or understand the sign. (repeat)

11200 - CCLR s.17 - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. At the time of inspection the following was noted: -Various sections within the outdoor play space where the fence is rotting. Where the rotten wood has worn away has now exposed rusted nails that may eventually fall on the group or a child may come in contact with. This was noted in the last routine inspection (repeat). -Beside the gate within the outdoor play space, there are areas where the paint has started to chip. -The green paint within the Preschool room has started to peel, leaving pieces of paint on the ground. -Some areas of the baseboard within the Multi-Age room has some peeling paint.
Corrective action. Please provide a written response to indicate how you will address the contraventions noted above on or before September 13, 2024

Observation. Licensing observed the gate for the outdoor play area rotting and difficult to open. Additionally, there are many areas of the wooden fence enclosing the outdoor play area that is broken and/or rotting.
Corrective action. Ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair.
11020 - CCLR s.13(2) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following; - The fridge temperature was measured at 7 degrees Celsius. Licensing explained that the fridge temperature needs to be between 0-4 degrees Celsius. Repeat. - A tall mirror on a wooden stand on top of the shelf where children obtain toys from. Potential to fall on a child. The Manager placed it down on the ground, however, a child bumped into it causing it to fall on another child and hurt them. The Manager removed it and placed it the office.
Corrective action. Ensure that a healthy and safe environment is provided at all times while children are in care.

Observation (CORRECTED DURING INSPECTION): Licensing observed that the Manager's name was not posted anywhere within the facility.

- STAFFING
Despite this section, during the half hour before a care program begins and the half hour after a care program ends, a licensee may permit children who would otherwise be grouped in different care programs, and who are arriving at or leaving the community care facility, to be grouped together and supervised other than as required under Schedule E if (a) the care program is described as Multi-Age Child Care, Group Child Care (Under 36 Months), Group Child Care (30 Months to School Age) or Group Child Care (School Age).
Observation. Upon arrival at the facility Licensing observed that the Manager had placed an attendance sheet in a drawer. Licensing reviewed the attendance record, that was on the counter, for the Multi-age program. Upon review of the attendance sheet that was place in the drawer, Licensing observed that it was for the Preschool program and two children had been signed in on it. Upon discussion with the Manager, Licensing observed that there were three children from the Preschool program that were being mixed with five children in the Multi-Age program. The Manager informed Licensing that the Preschool staff was not at the facility today. Licensing provided education on when care programs are permitted to be grouped together. This is a repeat contravention and was observed at the last follow-up inspection.
Corrective action. Ensure that different care programs are grouped together only half hour before a care program begins and half hour after a care program ends.

- LICENSING
A licensee must display in a prominent place in the community care facility the certificate, or a copy of the certificate, of any educator or assistant employed to work in the community care facility.
Observation. On November 2, 2022, Licensing observed a Early Child hood Educator Assistant (ECEA) staff working with the children. The staff is a regular substitute however, their ECEA certificate was not posted anywhere within the community care facility.
Corrective action. Ensure that all staff's certificate or a copy of their certificate is displayed in a prominent place within the community care facility.
10070 - CCLR s.11(4) - OR SUPERVISION
Without limiting subsection (1), a licensee must ensure that (a) children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E.
Observation. On November 2, 2022, Licensing asked the staff how many children they had in care and was informed that there were 7 children in care. However, Licensing observed 9 children in the multi-age program. The maximum capacity is eight for this program.
Corrective action. Ensure that the maximum capacity is maintained at all times.
14020 - CCLR s.34(2)(a) - STAFFING

Licensing provided education to the Manager via telephone call that different care programs can only be grouped together for the first half hour before a care program begins and last half hour after the care program ends.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following; - There is no lock or latch on the inside of the playground gate. Potential for a child to get out of the playground. The Manager has bought a new lock but has not installed it yet. - A piece of wooden panel resting against the inside of the front wooden fence of the playground. - There isn't sufficient resilient surfacing under the plastic slide. Repeat. - The black mat at the front entrance is curled up and Licensing almost tripped when entering the facility. Potential tripping hazard. - The fridge temperature was measured at 11.7 degrees Celsius. Children's milk is stored in the fridge. Licensing provided education that the fridge temperature should be between 0-4 degrees Celsius. - There is a camera sitting on a high windowsill that has it's wire hanging down and accessible to children. Potential strangulation hazard and hazard of the camera falling on a child and injuring them. - A dirty plunger beside the toilet in one of the children's bathroom. - Bleach and water solution bottle not labelled. Repeat.
Corrective action. Ensure that a healthy and safe environment is provided at all times while children are in care.

10050 - CCLR s.11(1) - POLICIES AND PROCEDURES
A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].
Observation. Upon a review of the fire drills from Nov. 2021 to October 2022, Licensing observed that there was no record available of the Group Child Care (SA) practicing a fire drill each month. The Manager informed Licensing that a fire drill is practiced each month but hasn't been recorded. Licensing provided education on ensuring that all fire and emergency drills for each program are recorded or if practiced together then need to provide that information on the sheet.
Corrective action. Ensure that current records are kept for fire and emergency drills.
13050 - CCLR s.56(1)(c) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. Upon a random review of five children's records, Licensing observed the following; - One child's record missing the telephone number of the medical practitioner. - One child's record missing the name and telephone number of a medical practitioner. Licensing provided education on what wold meet the intent of this regulation.
Corrective action. Ensure that children's records are complete prior to the child attending the community care facility.
19100 - CCLR s.57(2)(d) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (b) date of enrolment in the community care facility.
Observation. Upon a random review of five children's records, Licensing observed one child's record missing the date of enrolment.
Corrective action. Ensure that children's records are complete prior to the child attending the community care facility.
19080 - CCLR s.57(2)(b) - RECORDS AND REPORTING
A licensee must have in writing from a parent, and maintain at the community care facility, consent (a) to call a medical practitioner or ambulance in case of accident or illness if the parent cannot immediately be reached.
Observation. Upon a random review of five children's records, Licensing observed that two children's records were missing written consent, from a parent, to call a medical practitioner or ambulance in case of an accident or illness if he parent cannot immediately be reached.
Corrective action. Ensure that children's records are complete prior to the child attending the community care facility.
19190 - CCLR s.57(3)(a) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.
Observation. Upon a random review of five children's records, Licensing observed three children's record missing a photograph. This is a continuous repeat contravention.
Corrective action. Ensure that children's records are complete prior to the child attending the community care facility.
19160 - CCLR s.57(2)(i) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation. Upon a review of the attendance record for the month of October, Licensing observed the following in the Group Child Care (SA); - four days that children were not identified as absent in the AM. - one day children not signed-in in the PM. - one day that children not signed-out in the AM. Licensing provided education on how to maintain accurate attendance records. This is a continuous repeat contravention.
Corrective action. Ensure that attendance records are accurate on a daily basis.
19090 - CCLR s.57(2)(c) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. Upon a review of three staff records, Licensing observed that one staff's criminal record check (CRC) has not been obtained. Licensing only observed a police check available.
Corrective action. Ensure that staff records are complete prior to the staff working at the community care facility.
12040 - CCLR s.19(1)(a) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.
Observation. Upon a review of three staff records, Licensing observed there is no evidence that one staff's references have been checked. Licensing provided education on what would meet the intent of this regulation.
Corrective action. Ensure that staff records are complete prior to the staff working at the community care facility.

A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. Observation. Upon a review of three staff records, Licensing observed that two staff's certificates have not been verified by the early childhood educator (ECE) registry. Licensing provided education that all new certificates need to be verified.

Observation. One playpen in the Multi Age room has a ripped pad. -One shelf in the Preschool room is showing signs of wear and tear as paint is chipping along most of the edges.
Corrective action. Please ensure that all equipment is in good repair.

A licensee must ensure that all indoor and outdoor play materials and equipment accessible to children are (b) safely constructed, free from hazards and in good repair.
Observation. The outdoor play area was recently vandalized and cannot be used by the children.
Corrective action. Please ensure that the outdoor play area is safe, free from hazards and in good repair.
11190 - CCLR s.16(4)(b) - EQUIPMENT AND FURNISHINGS
A licensee must supply equipment, furniture and supplies that are (a) of sturdy and safe construction, easy to clean and free from hazards.
Observation. The tall red shelf used for diaper changing supplies is not secured to the wall. The cloth that is on top of the diaper change pad is not secure so could make it difficult to sanitize fully as it moves around
Corrective action. Please ensure all tall shelving is secured to the wall to prevent tipping and causing injury. Please ensure diaper change table pad is easily wipable to be sanitized.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.
Observation. 4 staff files were reviewed and one is missing immunization status.
Corrective action. Please ensure all staff have immunization status on file.

A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (d) copies of any diplomas, certificates or other evidence of the person's training and skills.
Observation. Two staff files had verification of an ECE certificate but no certificate is on file. One staff member was able to show a digital copy on their personal cell phone.
Corrective action. Please ensure that any staff's ECE Assistant, ECE and IT certificates are on file at the facility.

The Manager informed Licensing that they will have this addressed during the winter break. - A lounge chair with cloth cover that has many brown and black stains on it. Licensing received a written response from the Manager stating that this had been addressed. This is a repeat contravention.
Corrective action. Please ensure that the furniture, equipment and fixtures within the facility are clean and in good repair while children are in care.
11020 - CCLR s.13(2) - OR SUPERVISION
A licensee must ensure that children are supervised at all times by a person who is an educator, an assistant or a responsible adult.
Observation. Upon arriving at the facility Licensing observed 2 children walking around in the hallway. The gate to the program was left open. Licensing observed 3 other children in the program and the staff in the office. The 3 children in the program were not being supervised and the staff did not know that 2 of the children were in the hallway. Licensing provided education on the important of ensuring children are supervised at all times while in care by staff.
Corrective action. Please ensure that children are supervised at all times by a person who is an educator, an assistant or a responsible adult.
14060 - CCLR s.39(1) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation. Upon a review of 10 days of the attendance record for the months of October and November Licensing observed the following; - 3 days that children's time of departure was not indicated. - 1 day that a child's time of arrival was not indicated. - 6 days that children were not identified as absent or present. Education provided to the Manager about ensuring that attendance records are accurate on a daily basis.
Corrective action. Please ensure that attendance records are accurate on a daily basis.
19090 - CCLR s.57(2)(c) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.
Observation. Licensing could not verify if a child's photograph has been added to the child's record as the staff was unable to locate the child's record. The staff attempted to locate the child's record for 10 minutes however was unsuccessful. Licensing provided education to the Manager to ensure that children's records are easily accessible and all staff are aware where records are placed. Licensing received a written response from the Manager stating this had been addressed.
Corrective action. Please ensure that children's records are complete prior to the child attending the facility.

Observation. Licensing observed that 1 staff's certificate still has not been validated through the Early Childhood Education Registry. The Manager informed Licensing that they had placed the validation in the staff's file. However, upon a review of the file that was provided to Licensing, Licensing did not see the validation. Licensing received a written response from the Manager stating this had been addressed. This is a repeat contravention.

Observation. Licensing observed a small plastic slide and climber and a small teeter tater in the outdoor play area. The outdoor play materials and equipment are not age or developmentally appropriate for the Group Child Care (School Age) group. Licensing provided education on the importance of having age and developmentally appropriate and sufficient play materials and equipment.
Corrective action. Please ensure that all indoor and outdoor play materials and equipment accessible to children are suitable for the age and development of the children.
11180 - CCLR s.16(4)(a) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following accessible to children in care; - In the Preschool program a corner ledge that has paint chipped exposing the concrete in some areas and white underlay in other areas. - In the Preschool program a wooden shelf that has paint chipped in many areas. - In the Preschool program a lounge chair with a cloth covering. The cloth covering has many brown and black stains on it. - The diaper change table has a rip in it exposing the foam underlay. Licensing provided education that it is hard to clean and sanitize when the foam is exposed. - 4 mats in the Multi-age program that are ripped in several areas. - A teeter tater in the outdoor play area that is not secured and one of the handles is rusted all over and has chipped paint.
Corrective action. Please ensure that the furniture, equipment and fixtures within the facility are clean and in good repair while children are in care.

A licensee must have all of the following: (b) an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
Observation. Licensing observed that all the food and supplies in the on wheels emergency bag has expired. Education provided on having a system in place to ensure that the emergency supplies are maintained.
Corrective action. Please ensure that an emergency plan is in place that sets out procedures to prepare for, mitigate, respond to and recover from any emergency.
13020 - CCLR s.22(1)(b) - PROGRAM
A licensee must provide to children a comprehensive and coordinated program of indoor and outdoor activities that (c) complies with the program standards set out in Schedule G.
Observation. Upon discussion with the Manager Licensing observed that the children had not gone outside in the designated play area (playground) for 4 months. The Manager informed Licensing that walks were taken occasionally for about 30 minutes. Licensing provided education on the importance of programming to ensure that children are going outside for; the required amount of time for each program and that encourages physical development.
Corrective action. Please ensure that children are provided with a comprehensive and coordinated program of indoor and outdoor activities that complies with the program standards set in Schedule G.

Observation. Upon a review of 3 staff records, Licensing observed that 1 staff's certificate has not been validated through the Early Childhood Education Registry. This is a repeat contravention.

Observation. Upon a review of five days of the attendance records for the month of January 2020 Licensing observed the following: - One day that all children were not signed out - One day that several children were not identified as absent. This is a repeat contravention.

Observation. Licensing observed that there is still one staff missing a criminal record check. The Manager informed Licensing that the form has been completed and submitted to the criminal record check department and is being processed.
Corrective action. Please ensure that all staff records are complete prior to the staff working at the care facility.

Observation. Licensing observed that there is still one staff who's criminal record check is addressed to another facility. The Manager informed Licensing that the form had to be resubmitted as it was not completed correctly. Licensing provided suggestions to ensure that all forms are completed correctly.
