A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. The following items were noted at the time of inspection: -Large play structure is in disrepair with many areas that require maintenance. There are multiple areas where the wood is rotting and the wood has split which can cause splinters, paint has started to peel in many sections. Some of the accessories of the climber are in disrepair. Licensee stated that they do not use the play structure at this moment due to the areas of disrepair. This is a repeat contravention. -Wheelbarrows is store on stop of various planting items within the outdoor play space, discussion had about making the wheelbarrow inaccessible. -Two areas within the back yard that is accessible (deck, side of home), there multiple items stored within this area that are accessible to children (old pieces of fencing, light bulbs, rotting wood) which can be hazardous to children. -Spray bottle which is used for sanitizer is not labelled.
Corrective action. Please provide a written response to indicate how you will address the contravention noted above on or before May 13, 2025

Observation. Emergency drill for 2024 had not been documented. Licensee stated that it was carried out and the records are available online, however, they did not know how to access this information.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. The following was noted at today's inspection: -Outdoor play structure has some areas where the wood has started to rot, rough exposed edges of the wood in some areas. -Fence at the back of the yard has started to cave into the property exposing a nail and creating a large gap which can act as an entrapment hazard. Various items from the neighboring property now accessible.
Corrective action. Please provide a written response to indicate how you will address the contravention noted above on or before April 30, 2024
11020 - CCLR s.13(2) - OR SUPERVISION
Without limiting subsection (1), a licensee must ensure that (a) children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E.
Observation. There have been multiple days noted on the attendance where there were 4-5 children under the age of 3 years of age in care at the same time.

14020 - CCLR s.34(2)(a) - POLICIES AND PROCEDURES
A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment]. Observation (CORRECTED DURING INSPECTION): Documentation to indicate that the emergency drill was conducted was not written down within the log.
Corrective action. Licensee wrote down this information at the time of inspection. Discussion had about ensuring yearly emergency drill is documented in order to confirm compliance.

A licensee must not employ a person in a community care facility as (a) an educator or an assistant unless the person holds a certificate issued under Division 2 [Employee Qualifications], which must be verified by the system maintained by the early childhood educator registry. Observation. One of the staff who is an ECEA does not have proof of verification of their Certificate within their file.
Corrective action. Licensee stated that they would verify this staff's Certificate.

Observation. Licensing observed the following; - Several sleeping mats are ripped and have tape on them. However, the tape is peeling off and dirty underneath. - One cushion chair, in one of the play rooms, is ripped and the foam underlay is exposed. - The outdoor equipment, climber, chairs, tables and swings have green and black grime and are dirty. - Nine deflated balls. - The wooden climbing and slide structure has the metal shade broken on one side and another wooden area broken.
Corrective action. Ensure that the community care facility and the furniture, equipment and fixtures in it are clean and in good repair.

Observation. Upon a random review of three children's records, Licensing observed that two children's records were missing their medical insurance practitioner and emergency contact information. Licensing had a discussion with the Manager about what would meet the intent of this regulation.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following; - an area in the outdoor grass area where the grass has stopped growing and there is a big hole. The Manager has put more dirt and grass seeds on it. - The cloth shade on the wooden climber is still broken. The Manager informed Licensing that the climber shakes and lifts up when children use the swings. The Manager wants to replace the entire wooden climbing and slide structure. Licensing re-examined the climber and noticed that it is not secure. There are also other parts of the wooden panels that are coming out of the screws. The Manager informed Licensing that the outdoor play area is not being used at the moment and an off-site playground is being used.
Corrective action. Ensure that community care facility and the furniture, equipment and fixtures within it are clean and in good repair.

A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. Licensing observed the following: - Two sleep mats that are ripped at the corners exposing the foam underlay. - Overgrown grass in the outdoor grass play area. - The cloth shade on the wooden climber and slide structure is broken. The Manager informed Licensing that the outdoor play area will not be used and are using the park across the street.
Corrective action. Please ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in care.

Licensing observed the following:- - A big hole in the one part of the outdoor grass area. - Two big cubby shelves in the covered outdoor cement area not secured. - Several branches scattered in the outdoor area. (REPEAT) - Four shelves in the indoor play areas not secured. (REPEAT) - a big wooden door resting against the wall in one play room. - A dirty mop in the tub that is accessible to children. (REPEAT)
Corrective action. Please ensure that a healthy and safe environment is provided at all times while children are in care.
11010 - CCLR s.13(1) - LICENSING
A licensee must display in a prominent place in the community care facility both the licence, including any terms or conditions of the licence, and the name of the manager. Observation (CORRECTED DURING INSPECTION): Licensing observed that the Manager's name was not posted anywhere in the facility. This is a repeat contravention.
Corrective action. Please ensure that both the Licence and the name of the manager is displayed in a prominent place within the community care facility.
10050 - CCLR s.11(1) - LICENSING
A licensee must display in a prominent place in the community care facility the certificate, or a copy of the certificate, of any educator or assistant employed to work in the community care facility.
Observation. Licensing observed a staff who works at the community care facility whose certificate is not posted.
Corrective action. Please ensure that the certificate or a copy of the certificate of all staff working is displayed in a prominent place within the community care facility.
10070 - CCLR s.11(4) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation. Upon a review of six days of the attendance records for the month of April, Licensing observed five days that children's departure dates were missing.
Corrective action. Please ensure that attendance records are accurate on a daily basis.

A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.
Observation. Upon a random review of three out of seven children's records, Licensing observed one child's record missing their immunization status.
Corrective action. Please ensure that children's records are complete prior to the child attending the community care facility.
19070 - CCLR s.57(2)(a) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (e) any illness, allergy or medical disability disclosed to the licensee by the child or his or her parent or medical practitioner.
Observation. Upon discussion with the Manager, Licensing observed a child in care who had a medical procedure. Upon a review of the child's record, Licensing observed that there is nothing in the child's record explaining their medical condition that has been disclosed by the parent to the Manager. Licensing provided education on the importance of keeping, for each child, a record of any illness, allergy or medical disability disclosed by the child's parent or medical practitioner.
Corrective action. Please ensure that children's records are complete prior to the child attending the community care facility.
19110 - CCLR s.57(2)(e) - STAFFING
A licensee must ensure that children have at all times immediate access to an employee who (a) holds a valid first aid and CPR certificate, provided on completion of a course that meets the requirements of Schedule C.
Observation. Upon a review of two staff records, Licensing observed one staff's record missing a valid first aid certificate.

Observation. Upon a review of two staff records, Licensing observed that one staff's criminal record check (CRC) is not site specific. The Manager informed Licensing that there is a cleared CRC addressed to the facility but was unable to provide it.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following:- -Plastic slide and climbing structure in the outdoor play area that is placed close to the cement walkway. Therefore, the fall zone around the climber/slide does not have sufficient resilient surfacing. Potential for a child to fall on the cement area and hurt themselves. - Two tall shelves in one play room that are not secured to the wall and wobbly upon touch. - A dirty mop in the bathtub that is accessible to children in care. -several shelves in the other play area not secured to the wall.
Corrective action. Ensure that a safe and healthy environment is provided at all times while children are in care.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must supply equipment, furniture and supplies that are (a) of sturdy and safe construction, easy to clean and free from hazards.
Observation. Licensing observed a couple of sleeping mats that were ripped exposing the foam underlay. Licensing provided education, to the Manager, that the mats cannot be effectively cleaned and sanitized with rips in them as it is a habitat for bacteria build-up.
Corrective action. Ensure that all equipment, furniture and supplies are of sturdy and safe construction, easy to clean and free from hazards.
11080 - CCLR s.15(1)(a) - HYGIENE AND COMMUNICABLE DISEASE
A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.
Observation. Licensing observed five children in care napping. The children's mats were right beside each other and no space in between them. Additionally, a couple of the children had there legs and blankets on the adjacent child's mat. Potential for cross contamination and spreading of germs, especially during a pandemic. Licensing suggested having space between each child's mat and placing the children head to toe.
Corrective action. Ensure that a program has been established to instruct children in and to practice the rules of health and hygiene.

Observation. Licensing observed following that is accessible to children in care; - A metal gate and wooden gate resting on one side of the shed. (This is a repeat contravention). -Many branches that have fallen and scattered all over the outdoor play area. - A large tree branch that has broken and fallen into the outdoor play area. - A rusted metal screw sticking out of one panel of the wooden fence at the back. - A hose sprawled on the cement area. - A diaper rash cream in an unlocked cupboard in one of the bathrooms. - Toilet bowl scrubber beside the toilet in both bathrooms.
Corrective action. Please ensure that a healthy and safe environment is provided at all times while children are in care.

Observation. Licensing observed the following: -There are several brown stains on the bathroom walls. -There are brown stains on one of the bathroom door. - There is a highchair that is ripped at the top. - The toilet seat in one of the bathrooms is ripped. On Licensing second inspection day, January 21, 2020, Licensing observed that the toilet seat has been replaced. - One medium sized shelf in the playroom, of the Multi-age program, that is wobbly and not secured. - A small piece of the vinyl flooring that is ripped and uplifted. Potential tripping hazard.
Corrective action. Please ensure that the furniture, equipment and fixtures within the care facility are clean and in good repair while children are in care.
