    Observation. Furniture, shelves, walls, floors all have visible dirt and debris present and are not clean. Gate broken. Cupboard accessible to children has a door broken.

In one classroom, this table is close to the classroom entrance door and partially in the play / activity area thus limiting the level of privacy that would be appropriate to support the privacy and dignity of children being changed.

    Observation. LO observed staff coming from behind and picking up a child, without warning , and taking the child for a diaper change. LO observed a staff telling children what not to do, rather than using positive guidance strategies, staff were using very loud voices when directing children. Staff telling the children, "no thank you" in response to undesired behaviours rather that offering age appropriate support / teaching / guidance.

    Observation. LO observed a staff using a very loud voice telling a child they had a stinky poo poo diaper, the staff said this several times very loudly. The physical environment continues to be unclean, have visible debris on multi surfaces which does not support the health, safety nor dignity of children in care. The schedule is for the whole group of children not based on individual needs.

    Observation. The classrooms continue to be in need of a thorough cleaning and regular daily cleaning. There was food and debris on the floor, the windows and mirrors had a considerable amount of grime on them, some of the small area rugs are visibly dirty and in need of replacement or a deep clean. The washroom had large amounts of toilet paper on the floor, there were toys on the floor as well as water on the floor creating a slipping hazard. The latch to one of the nap rooms has come off and a child kept running into the nap area, the sliding door was easily pushed open and shut - there is a risk to a child getting caught in the door or being isolated and left behind in this separate space.

    Observation. LO observed toddlers colouring, with very few wax crayon options on white paper, the outline of a mitten. The crayons were circular , 1" in diameter, one child put the whole thing in their mouth, this was not noticed by staff but pointed out to staff by LO. Some of the classrooms have limited materials / supplies and toys and age appropriate furniture.

    Observation. Some area rugs in classrooms have edges turned up creating a tripping hazard. By nature of this light weight style of carpet, the edges will continue to turn up easily. Double sided tape or furniture placement may help reduce this issue as this has been an ongoing concern. All classrooms continue to exhibit debris on walls, floors, shelves and counters.

    Observation. There were several carpets with the edges curling up. All the classrooms had debris, dirt build up on shelves, surfaces, children's seats and trays. Area foam mats were worn and torn, preventing them from being adequately cleaned and sanitized. Fridge Thermometers were above permissible temperatures. Diaper creams were being stored on low shelves accessible to children. Diaper change mat cleaning practice ineffective. Soothers stored in fridges all together in a tray. Potty seats on floor and in a pile in bathrooms, adequate sanitation not evident, multiple step stools in bathrooms that created tripping hazards. Child walking around with drinking cup / bottle with straw. Long strings on bags accessible to children presenting a choking hazard. Toys on shelves of diaper changing stations in classroom. Children's drinking cups on floor.

The quad strollers that are used to transport children to the roof top playground had food debris in them and in the bottom baskets.
