    Observation. The ride 'em toys are showing signs of wear and tear, missing pedals, dirty. Several toys / equipment are dirty , missing parts to support use. ( equipment disrepair previously mentioned)

    Observation. The entire outdoor play space continues to be an area of concern as previously noted on inspections conducted by other LO's. ( July 2020, April 2018) The wood is broken, rotting, uneven. The fence is loose in areas, panels are loose, paint is peeling. ( deadline to have entire playground painted was July 2021, given by CB in July 2020). The underside of the roof over the covered area is warped and showing signs of age, it would be appropriate to have this area evaluated for safety.

    Observation. Surfaces are not cleaned prior to sanitizing. Several windows open greater than 4". One window had blind cords hanging within children's reach. Back room had an electrical device plugged into an outlet with a cord and some plastic tape around it.

    Observation. Sleeping mats are showing signs of wear and have open and cracked edges - may limit the ability to adequately clean and sanitize.

    Observation. LO observed large bottles of soap in cupboard under the children's sink Children have access to the laundry room where laundry detergent and other hazardous substances could be available to children. Cupboard under the sink in children's bathroom was full of plastic bags.

    Observation. Many of the outdoor play materials , ride'em toys, trikes are broken and in a state of disrepair. On the playground was a cement cylinder with a metal pipe looking piece coming out of it that could prove hazardous should a child fall on it.

A deadline of July 2021 was given for completion. However, there are currently areas of the playground that are not safe for use, specifically the brown climber / slide.
