    Observation. Expired emergency medication on-site for one child. During the inspection, the manager contacted the parents to provide current medication.

    Corrective action. This medication will remain on-site until the current medication is obtained by the facility. Once the current medication is obtained, the manager will notify Licensing.

  • MEDICATION

    If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (a) administered to the child in the amount and at the times specified by the child's parent or in the child's record or care plan.

    Observation. One child’s registration file indicates instructions given by the parents to the facility to administer medication to the child. As per the Manager, the medication was never on-site and the facility did not administer the medication because the parents verbally told the Manager that the medication was not required.

    Corrective action. By the second day of the inspection, the Manager followed up with the parents regarding this. The Manager will obtain current documentation related to the adminstration of this medication.

    Without limiting subsection (1), a licensee must ensure that (a) children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E. Observation (CORRECTED DURING INSPECTION): Under 36 months program: a temporary retention was not applied for a child that turned 3 years old early in July. As per the Manager, the child will be moving into the next program in August and the facility is closed next week. Licensing and the Manager discussion how to apply for a temporary retention.

    Corrective action. The Manager will apply for a temporary retention for any child over 36 months to remain in the under 36 months program.

    A licensee must: (b) have an emergency plan that sets out procedures to prepare for, mitigate, respond to and recover from any emergency. Observation (CORRECTED DURING INSPECTION): Expired food items in the emergency kits. This is a repeat contravention. Upon notifying the Manager, all the expired items were removed. On the second day of the inspection, expred food items were replaced.

    Corrective action. The Manager will implement a check list system to track expirey dates for food supply.

    Observation. A care plan for a child with an allergy was not annually reviewed with the parents. As per the Manager, she verbally reviewed the care plan with the parents, but did not document the date of review. By the second day of the inspection, the Manager had followed up with the parents and awaits the current care plan.

    Corrective action. Please ensure a current care plan is obtained that documents the date of review along with the parent's signature.

Observation (CORRECTED DURING INSPECTION): Absences are not indicated on the attendance sheets. On the second day of the inspection, the Manager had filled out the attendance sheets documenting absences.

    Observation. Logbook is not being maintained by the programs as the last entry date was from 2023. As per the Manager, she communicates with the parents via texts.

    Corrective action. The program will regularly use the logbook as per legislation.

    Observation. 2 older children in highchairs during snack time. As per the Manager, the children run around while eating. In discussion with the Manager, the program is having challenges with additional behaviours as well. The Manager and Licensing discussed creating a care plan in collaboration with the parents that includes how to support the children and these behaviours while at the childcare facility.

    Corrective action. The Manager will develop a care plan in collaboration with the parents.

    A licensee must ensure that all indoor and outdoor play materials and equipment accessible to children are (b) safely constructed, free from hazards and in good repair.

    Observation. There are multiple gaps between the outdoor rubber tiles due to shifting. These gaps present a tripping hazard to children. As per the Manager, she has already connected with the Licensee contact regarding this.

    Corrective action. Please ensure all indoor and outdoor play materials and equipment are in good repair.

    A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

    Observation. Licensing observed rips on the diaper changing mat and multiple dents on the walls (in the under 36 months program) that have exposed the dry wall. As per the Manager, she has already spoken to the Licensee contact regarding these needed repairs.

    Corrective action. Please ensure the facility and its equipment are in clean and good repair.

    If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (a) administered to the child in the amount and at the times specified by the child's parent or in the child's record or care plan.

    Observation. 1) Licensing observed 1 child's file that indicated medication for allergies and emergency medication for a health condition, but no medication adminstration form available for either medications. A medication adminstration form must be completed by the parents for both medications. 2) In discussion with the Manager, sunscreen and diaper rash creams are applied to the children, but there is no written consent from parents permitting the application of the 2 products. Licensing and the Manager discussed having a form readily available to give to parents regarding the application of sunscreen and diaper rash creams specific to the child.

    Corrective action. 1) Please ensure medication adminstration form is obtained from the parents for both medications. 2) Please ensure written consent is obtained from parents regarding the application of sunscreen and diaper rash creams.

    Observation. Upon reviewing the emergency food supply, Licensing noted multiple food items that have expired and 2/3 cases of expired water bottles.

    Without limiting subsection (1), a licensee must ensure that (b) the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E.

    Observation. After reviewing staff qualifications and discussion with the Manager, Licensing notes the following: -in the GCC (30 mos to SA) program, the staff present with the 7 children has not yet obtained her ECE certificate from the ECE Registry. The Manager said that the staff has applied for her certification and is waiting for its arrival. Until it arrives, this staff is considered a Reponsible Adult. As per Schedule E, at minimum, an ECE must be present in order to fullfill the ratio of employees to children in this program. -in the GCC (under 36 months), 3 staff and 11 children were present with the following qualifications: 1 IT and 2 ECEAs. As per the Manager, 1 ECEA has applied for their ECE certificate and awaits its arrival. As per Schedule E, 1 I/T, 1 ECE and 1 ECEA must be present in order to fullfill the ratio of employees to children in this program. Licensing and the Manager discussed applying for exemptions for these 2 staff.

    Corrective action. Please ensure that the ratio of employees to children is being followed as per Schedule E.

    A licensee must ensure that children are adequately supervised by qualified employees in sufficient numbers to meet the needs of each child.

    Observation. During the routine inspection the following was observed: - upon arrival, the licensing officer observed 4 children in the hallway adjacent to the 3-5 classroom unsupervised. The remained unsupervised for approximately 3-5 minutes when the manager called a staff and the staff came and took the children outside. - the staff in the 3-5 classroom set up some books at the table and asked all children to look at books while she left the room to heat up lunches. The children were under the supervision of the student doing her observation, however she was not a staff member, therefore she was not part of the ratio. This is a repeat contravention from 2019 and 2020.

    Corrective action. Please ensure the children are supervised at all times and according to the legislation.

    A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

    Observation. During the inspection it was noted that the cots in the infant/toddler nap room were covered with sheets and blankets and were stacked one on top of each other. The sheets and the blankets were touching promoting cross contamination.

    Corrective action. Please ensure that the cots are stacked in a way that cross contamination is avoided. Please ensure a healthy and safe environment is provided at all times while children are in attendance.

    A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

    Observation. During the inspection #SMOY-C7DN7Y from September 29, 2021 it was noted that two areas with transition strips have become porous. During today's follow up inspection it was noted that one transition piece between the 3-5 classroom and the kitchen area was lifting and causing a tripping hazard.

    Corrective action. Please ensure that the transition piece is repaired or replaced as to not cause a tripping hazard to staff an/or children.

    Observation. During the inspection #SMOY-C7DN7Y from September 29, 2021, it was noted that some items were beyond their expiry date. During today's follow up inspection it was noted that the emergency supplies have been replaced, however, two items (vanilla cookies and baby food) presented an expiry date of January 31, 2022. This is a repeat contravention from 2021.

  • EQUIPMENT AND FURNISHINGS

    A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.

    Observation. The following was discussed with the Manager during the inspection: 1) rubber tiles located within the outdoor play area are separating and shifting. The tiles are posing a tripping hazard and the resiliency of the surface is compromised as there is no longer a continuous surface. 2) the facility has 1 sink located within the kitchen therefore dishes and cutlery are not able to be cleaned and sanitized. 3) various plastic bags were stored in unsecured drawers and cupboards within the kitchen 4) a Step 2 climber, slide combination is being used indoors without any fall protection as per manufacturer's recommendation.

    Corrective action. Ensure rubber tiles are installed in a secure manner in which the tiles do not separate. Due to the absence of a dishwasher or 3 compartment sink the facility may use an approve system to wash and sanitize dishes or alternately single use items must be used or families may provide these items for their child. Ensure all plastic bags are stored in a manner that are inaccessible to children. Remove the climber from use indoors until installed as per the manufacturer's recommendations.

    A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.

    Observation. Licensing observed the following in the group child care (under 36 months) program: 1) the flooring is damaged. 2) the wall where the high chairs are stored is chipped. 3) there is a gate between the activity room and the hallway leading the to washroom. Where the gate is secured to the wall the gyproc is damaged. 4) there are two areas with transition strips. All of these items are now porous and are no longer able to be cleaned and sanitized.

    Corrective action. These items must be repaired or replaced in a manner where the areas are sealed and able to be cleaned and sanitized.

    A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.

    Observation. The toilet paper holders located adjacent to the toilets have been removed. The Manager indicated that staff provide toilet paper to children as needed. Toilet paper was observed on a upper window ledge out of children's reach.

    Corrective action. Children must have access to toilet paper without having to ask a staff member for it.

    Observation. Of the four staff employed one staff did not have their early childhood educator certificate posted.

    Observation. Upon review of the facility's emergency supplies it was noted some items are beyond their expiry date. Additionally there were insufficient supplies for the licensed capacity and staff for a period of no less than 72 hours.

    Corrective action. Ensure all supplies (food, water,shelter, toileting, clothing, activities, medication etc) are in sufficient amounts for all children and staff. It is recommended the Manager implement a system to review the expiry dates on the food and beverage items so they may be used prior to their expiry date and replenished as needed.

    Observation. Upon review of 5 staff files 1 file did not contain evidence the Manager and/or Licensee verified the individuals early childhood educator certificate.

    Corrective action. The Manager and/or the Licensee may go on to the ECE Registry website and verify a certificate. The Manager has done this for other files and has placed a copy of the verification in the staff file. Ensure all current and future staff files have this documentation contained with their file.

    Observation. Licensing reviewed 5 staff files and noted that the references within the files were not all verified. The Manager informed licensing she has spoken with the references however she has not documented having done so. This contravention has been noted on the inspection report dated March 5, 2021 #SMOY-BYWMYP. In order to demonstrate that the references have been checked the Manager and/or Licensee must implement a system to indicate that they have done so.

    Corrective action. All current and future staff files must contain evidence that the Manager has contacted the staff's references.

    Observation. Licensing reviewed 5 staff files and 1 file contained a criminal record clearance letter addressed to another agency. This contravention has been noted on the inspection report dated March 5, 2021 #SMOY-BYWMYP and on October 16, 2020 inspection report #MPEE-BUFTK3.

    Corrective action. The Manager must ensure that all criminal record clearance letters are addressed to the Licensee prior to the individual providing care to children. The Manager will need to implement a system to prevent this from occurring with any future individuals.

    Observation. Licensing observed the following: - The carpet in the 3-5 room is slippery posing as a tripping hazard - Emergency medication was stored in an unsecured cupboard which was accesible to the children - A bolt lock has not been removed from the 3-5 room door despite being informed by the Fire Inspector last year. The supervisor was observed closing the 3-5 room door and using the bolt lock to secure the door. - Alcohol wipes with a Drug Identification Number (DIN) was stored in the first aid kit. Parent permission to administer this was not available.

    Corrective action. Please ensure a healthy and safe environment is provided at all times.

    A licensee must ensure that children are adequately supervised by qualified employees in sufficient numbers to meet the needs of each child.

    Observation. Licensing observed the following: - Upon arrival to the facility a staff member greeted licensing and called the supervisor to the main door. Licensing entered the facility and noticed no teacher in the 3-5 room. The supervisor requested a staff member from the infant/toddler room to go to the 3-5 room to supervise the children. - A child was sitting in the corridor between the infant/toddler room and 3-5 room for several minutes when she came to daycare. The infant/toddler staff asked the child to return back to the 3-5 room. The supervisor was unaware the child was sitting in the corridor until the infant/toddler staff mentioned it. After several minutes the child returned back to the 3-5 room. The child was able to leave the 3-5 room as the door to the bathroom/corridor area was left open. Licensing observed this door left open during the entire inspection and children would run back and forth often.

    Corrective action. This is noted to be a repeat non-compliance. Please ensure children are adequately supervised by qualified employees in sufficient numbers to meet the needs of each child.

    Observation. A review of the sign in/out sheet and staff schedule, licensing determined the following: - From 7:30 am to 8:30 am in the infant/toddler room there is an I/T staff with 3 children younger than 36 months of age. In the 3-5 room there is an ECEA with 6 children 36 months of age or older. - The staffing plan in the infant/toddler room is an 1 Infant/Toddler Educator, 1 ECEA and 1 Responsible Adult with 12 children

    Corrective action. This is noted to be a repeat non-compliance. Please ensure the ratio of employees to children is no less than that permitted in Schedule E

    Observation. Licensing observed a bolt lock placed at the top of the egress door. The door was locked and secured. This egress door is one of the emergency exits for the facility. The fire inspector informed the manager last year the lock cannot be used during operating hours.

    Corrective action. Please ensure all emergency exits and a fire drill system has been appoved by a local assistant within the Fire Services Act.

A certificate from the college was posted on the wall. The staff stated she has not applied for her ECE certificate but holds an ECEA certificate, however was unable to produce one.

    Corrective action. Please ensure a copy of an employee's certificate is on file.
