Observation. Unsecured tall shelving in one room of the upper level.
Corrective action. The manager said she will secure this shelving to the wall. In the meantime, children will not be playing in this room until the shelf is secured.

A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene. Observation (CORRECTED DURING INSPECTION): Children did not wash their hands after eating lunch.
Corrective action. The manager said moving forward children will immediately wash their hands after eating.

Observation. Two children without emergency contacts (1 child with the parent's contact information listed).
Corrective action. The manager said she will follow up with both families to obtain emergency contacts that is neither parent.

Observation. 1) Toilet bowl cleaner accessible under sink of children’s designated washroom. 2) In the upstairs’ washroom, there are cleaning supplies and personal products accessible to children.
Corrective action. Please ensure hazardous items are inaccessible to children.
11200 - CCLR s.17 - EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. 1) No safety gate on stairs that are accessible to children. This is a repeat contravention from Inspection Report # SWAR-CJNPMZ. Upon notifying the Manager, the safety gate was implemented. CORRECTED DURING INSPECTION. 2) Outdoor gate was not closed that leads onto the driveway and road. In addition, the lock is faulty and prevents the gate from securely closing. This is a repeat contravention from Inspection Report # SWAR-CJNPMZ. 3) Children’s bedding is touching. This is a repeat contravention from Inspection Report # SWAR-CJNPMZ. 4) Multiple toys are stored on top of two tall shelving units. These stored toys present a falling hazard. 5) In the tenant’s outdoor area, the following are accessible to children: small BBQ, dangling electrical cord, and a container of fertilizer.
Corrective action. 1) Please ensure there is a safety gate on stairs that are accessible to children. CORRECTED DURING INSPECTION. 2) Please ensure the lock is fixed to ensure that the gate can securely close. This gate must be closed when children are outside in order to prevent children running onto the road. 3) Please store children's bedding in a way that is not touching one another. 4) Please ensure these items in the tenant's outdoor area are not accessible to children.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation. On the outdoor rubber surfacing, there are multiple piles of dead leaves.
Corrective action. Please ensure the piles of dead leaves are removed.
11020 - CCLR s.13(2) - OR SUPERVISION
A licensee must ensure that children are supervised at all times by a person who is an educator, an assistant or a responsible adult.
Observation. Multiple observations where children were in the kitchen while the Manager was in either the main activity area or washroom conducting diaper changes resulting in insufficient supervision. Licensing and the Manager discussed active supervision practices and implementing a safety gate between the two areas.
Corrective action. Please ensure all children are superivised at all times.
14060 - CCLR s.39(1) - RECORDS AND REPORTING
A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation. 1 child present was not recorded on the attendance sheet. As per the Manager, the child does drop-in care when there is another child absent.
Corrective action. Please ensure all children that are present are accurately recorded on the attendance sheet.
19090 - CCLR s.57(2)(c) - STAFFING
A licensee must ensure that each employee (a) is trained in the implementation of the fire drill system and emergency plan described in subsection (1), including the use of any equipment noted in the fire drill system and emergency plan.
Observation. On the fire extinguisher, the next service due date indicates: September 12, 2024 making it over due for servicing.
Corrective action. Please ensure the fire extinguisher is serviced.

A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation. Upon reviewing 7 children's files, 4 are missing the name and telephone number of a medical practitioner and/or emergency contact. The Manager explained that these families are new to the country to which Licensing explained that walk-in clinic information satifies the medical pracitioner contact information. Licensing acknowledges that newly immigrated families may not have relatives or close friends to place as emergency contacts. Licensing explained that parent's supervisors/managers can satisfy this piece of information. Licensing also explained the importance of an emergency contact to be an individual other than the parents that can readily contact the parents when the Manager cannot.
Corrective action. Please ensure children's files are complete with all required information.
19100 - CCLR s.57(2)(d) - STAFFING
A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (a) a criminal record check for the person.
Observation. A valid criminal record check was not available for the Manager. As per the Manager, she has applied for it be renewed.
Corrective action. Please ensure a valid criminal record check is available for all facility staff.
12040 - CCLR s.19(1)(a) - STAFFING
A licensee must ensure that children have at all times immediate access to an employee who (a) holds a valid first aid and CPR certificate, provided on completion of a course that meets the requirements of Schedule C.
Observation. There was no evidence of a valid First Aid certificate for the Manager. As per the Manager, she has a current certificate, but oculd not present it during the inspection. The Manager showed Licensing a copy of the receipt for the First Aid class that was taken in 2022.
Corrective action. Please ensure a valid First Aid certificate is available on-site.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following: - Outdoor play space has lots of weeds and large holes in the ground which require filling regularly to avoid injury. There is a chair in disrepair and a hose pipe laying on the ground which is a tripping hazard. - The back gate has a faulty catch and is open, leading onto the street - Tenants outdoor area containing a BBQ is accessible to the children - Indoor drawer unit is not anchored and tips when the drawers are open. This is a safety concern - Nap bedding was touching others' bedding - Licensing acknowledges that each bed is assigned to individual children and mats are separated. - There was no safety gate on the stairs which are accessible to children. This was corrected during inspection.
Corrective action. Please ensure a healthy and safe environment is provided to children at all times. Please provide a compliance plan to Licensing by the date below
11010 - CCLR s.13(1) - HYGIENE AND COMMUNICABLE DISEASE

Observation. Licensing observed : - several occasions when children didn't wash hands after using the washroom. - Bleach and water solution is not changed on a regular basis. This reduces it's effacacy for appropriate sanitizing.
Corrective action. Please ensure appropriate hygiene practices are in use.

A licensee must display in a prominent place in the community care facility both the licence, including any terms or conditions of the licence, and the name of the manager.
Observation. Licensing observed that the Manager name was not displayed as per requirements.
Corrective action. Please ensure the public are aware of who the Manager is by posting the name along with the Licence.
10050 - CCLR s.11(1) - NUTRITION AND FOOD SERVICES
A licensee must (b) promote healthy eating and nutritional habits.
Observation. Licensing observed that some children had unhealthy snacks provided by parents. The Manager said that she provides what parents supply. Some children's food was stored in backpacks with dairy products. Manager said that the parents forgot to add an ice pack.
Corrective action. Please ensure all food provided to children is safe and healthy according to the Canada Food Guide.
17020 - CCLR s.48(1)(b) - POLICIES AND PROCEDURES
A licensee must keep current records of each of the following: (c) a record respecting compliance with section 22 (2) (b) and (c) [emergency training and equipment].
Observation. The Manager was unable to provide records to attest to the monthly fire drills practiced. The Manager said that she hasn't practiced an annual drill for emergency and can't recall what the secondary location is.
Corrective action. Please ensure that emergency drills are recorded when practiced and are available for review.

A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.
Observation. Licensing observed that out of 4 child records reviewed, 3 were missing photos/digital images. The Manager said that she has videos of the children on her phone which is password protected, so not accessible to others if required in an emergency.
Corrective action. Please ensure children's records are complete.
19160 - CCLR s.57(2)(i) - RECORDS AND REPORTING
A licensee must keep current records for each child showing (a) the information set out in subsection (2), (b) if applicable, the information and agreements set out in subsections (2.1) and (2.2), and (c) the consents referred to in subsection (3).
Observation. Licensing observed the following: A new child did not have a file on site. The Manager said that the parent forgot to bring the forms at drop off, but does have a contact number for the parent. Some other records were missing individual items and must be current and complete.
Corrective action. Please ensure an audit of children's files to ensure complete and current.
19060 - CCLR s.57(1)(a)(b)(c) - RECORDS AND REPORTING
A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.
Observation. Licensing observed that a log book is not maintained. The Manager said that she texts Parents to inform them of any incidents. Licensing recommends having a written log that details all incidents and unexpected events.
Corrective action. Please ensure a log book is maintained to appropriately record and relay information. A bound book is recommended to protect the integrity of information.

A licensee must keep current records of each of the following: (b) for each employee, the records required under section 19 (1) [character and skill requirements].
Observation. Licensing observed that the Manager did not have a complete file on site containing all records required under Section 19.(1). This was further discussed. A criminal record check is current at the FH office.
Corrective action. Please ensure all documents are kept as required.

Observation. The facility has two washrooms that are accessible to children. Licensing noted in both washrooms the cupboard located under the wash basin was unsecured and both contained cleaning supplies such as ammonia, disinfectant wipes, Fantastic and Lysol products.
Corrective action. The items noted above must be stored in a manner that children do not have access to prevent a potential injury.
11200 - CCLR s.17 - LICENSING
A licensee must display in a prominent place in the community care facility both the licence, including any terms or conditions of the licence, and the name of the manager.
Observation. The community care facility licence was not posted nor was the name of the manager.
Corrective action. Post both the facility licence and the name of the manager in a place where members of the general public can view the documents.
10050 - CCLR s.11(1) - LICENSING
A licensee must display in a prominent place in the community care facility the certificate, or a copy of the certificate, of any educator or assistant employed to work in the community care facility.
Observation. The Manager did not have their certificate posted.
Corrective action. Ensure all staff have their early childhood educator certificates posted in a manner that may be viewed by the general public.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following: - Bleach solution was stored underneath the bathroom sink in an unsecured cupboard (CORRECTED DURING INSPECTION) - The fridge temperature is at 6 degrees celcius - The children's personal bedding was stacked together and stored in the closet. Licensing discused the importance of having separate bins or hooks to store bedding to mitigate the spread of communicable diseases.
Corrective action. Please ensure a healthy and safe environment is provided at all times.

Observation. Upon discussion with the Manager it was noted that the emergency evacuation for 2019 was not practiced completely as the meeting place was a 30 minute walk. Licensing discussed the importance of a meeting place that was closer.
Corrective action. Please ensure an emergency plan is in place sets out procedures to prepare for, mitigate, respond to and recover from any emergency.

Observation. Licensing observed the following: - Licensing noted on various days in the month of March there was no indication whether a child was absent or present. - A school age child was picked up by an individual and taken to school. The time of departure was not indicated on the sign in/out sheet. - 4 children were signed in, 6 children were present.
Corrective action. Please ensure a record indicating for each day whether the child is absent or present and the time of arrival and departure.

Observation. Licensing observed a child enrolled in the facility that does not have any records. The Manager has the contact information of the parents on her cellphone.
Corrective action. Plese ensure current records for each child showing the information set out in subsection (2) is on file.

Observation. Licensing noted two individuals in the house who have not obtained criminal records check. These individuals are not present while the facility is open.
Corrective action. This is noted to be a repeated non compliance. Please ensure individuals ordinarily present on the premises of a community care facility have obtained a criminal record check.
