Observation. A section of the outdoor artificial grass presents a tripping hazard increasing the risk of potential injury to the children. Repair or replace the damaged section to eliminate the hazard. The refrigerator temperature measures above the acceptable range for the safe storage of food increasing the risk of bacterial growth and foodborne illnesses. Adjust or repair the refrigerator to maintain food at the required food safe temperatures and continue to monitor temperatures regularly. The change mats are not non absorbent and impermeable, as evidence of liquid passing through the surface observed. This increases the risk of contamination and the transmission of infectious agents. Replace the change mats with non-absorbent, impermeable mats that can be effectively cleaned and sanitized between uses. Submit to licensing on or before August 7, 2026 a detailed written response, including evidence on how each identified contravention has been corrected.
