Observation. Upon review of all employee records, it was noted that COVID-19 vaccination status is not present. In order to know which employees could or could not work during a potential COVID-19 outbreak, this information is required. Notify Licensing that COVID-19 statuses of immunization have been added to employee records on or by June 5, 2025. Upon review of records it was noted that the Emergency Preparedness Plan Practice record was not available for review. This was resolved during the inspection.
