    Observation. During a review of the facility’s records and documentation, the Licensing Officer found that a child's record was incomplete. Specifically, it lacked the start date of attendance, the child's height for identification purposes, information about individuals who may not have access to the child, and the medical plan number. Keeping comprehensive and accurate child records is crucial for safeguarding their health, safety, and overall well-being. An examination of the program's facility minor incident log revealed that some recent minor incidents were not promptly communicated to parents. Immediate notification is crucial to safeguard children's safety, fulfill legal obligations, and uphold trust with families. Licensing discussed the requirements with the Manager during the inspection and requests an email outlining how the facility has addressed these concerns no later than August 22, 2025.

    Observation. During a review of the outdoor play area, the Licensing Officer noted that a swing set was not properly anchored and may not have been installed in accordance with the manufacturer's instructions. The Licensing Officer was able to lightly push against the frame and have it move. Proper installation and secure anchoring of swing sets, following the manufacturer's guidelines, are crucial to prevent accidents and safeguard children from avoidable injuries. Additionally, the sandbox in the area was uncovered when children were not present. Covering the sandbox after each use helps keep the environment clean and safe by preventing contamination, lowering the risk of injuries, and reducing exposure to pests. The Licensing Officer notified the Manager of the concerns during the inspection and requested prompt action to address any potential risks. Submit to the Licensing Officer an email with confirmation and details of the corrective measures by the end of the operational day (August 20, 2025).

    Observation. The complaint information received alleges a rabbit present at the facility is not properly contained and frequently urinating and defecating in the classroom environment. Observations in a Group 30 Months to School Age program room included a rabbit in a cage, a litter bin, feces, and shavings on the floor accessible and adjacent to children eating. The Manager informed the Licensing Officers that the cage and surrounding area had recently undergone cleaning. Exposure of children to rabbits, including their feces and bedding, poses potential health risks from zoonotic diseases, allergens, and contamination. Submit a plan detailing how you have addressed the identified risks noted above by the end of the business day on May 6, 2025.

    Observation. The outdoor play area and equipment was inspected. The rail and a board for the wooden bridge present in the area requires repair and presents a tripping/falling hazard. A climbing structure made from pallets has broken edging creating a risk of injury. The Manager was notified of the hazards observed at the time of inspection. The proposed area for the swing set installation was reviewed and discussed with the Manager. The Director of Licensing Standard of Practice for Safe Play Space was reviewed during the inspection with the Manager and the website link was provided. Submit to Licensing by the follow up date requested, not only how the hazards were immediately addressed to ensure the health and safety of the children in care, but also how the system for inspecting the outdoor play space and equipment will be revised to ensure any future hazards or needed repairs are identified, immediately addressed, and recorded in the outdoor play space log.
