Observation. Upon review of a sampling of child records. it was noted that one required a care card number and one required Emergency Contact phone numbers in order to be complete. Care Card Numbers are required to be available to provide to emergency services in the event of a medical emergency. Emergency Contact phone numbers are required in the event the parent or guardian cannot pick up a child who needs to leave due to illness, injury or unexpected event. One care plan that has been effect for longer than 12 months does not have a review date noted on file. Care Plans are required to be reviewed and updated at least annually to ensure that care plans are accurate and up-to-date. The care plan also did not identify actions to take in the event of symptoms of a medical condition. In order to ensure all employees are informed of preferred response to symptoms, this must be documented in the care plan. Notify Licensing of addition of the required information and review of all child records and care plans to ensure completeness on or by October 30, 2025. The Minor Incident Log, Outdoor Inspection Log, Fire Drill practice log, Emergency Preparedness Plan practice log were reviewed and determined to be complete and up-to-date.

Observation. Upon review of a sampling of employee records, one was noted to require an up-to-date validation for current certificate of qualification and a statement as to the employees status of immunization regarding COVID-19. The validation of certificates is required to ensure that employees are adequately qualified for the position they hold. The COVID-19 immunization status statement is required in the event there is a COVID-19 outbreak in the facility, the Licensee will know who may or may not be able to work. Notify Licensing of inclusion of these items in the employee record, and review and completion of all employee records, on or by October 30, 2025.

Observation. During review of care plans on file, it was observed that there is no indication that these plans have been reviewed annually with parents to ensure they are correct and up to date. Observation. Upon review of records it was noted that the Emergency Preparedness Plan practice was not documented for 2022. Upon discussion with Licensee it was noted that the practice was held, but documentation is not available for review.

Observation. Allegations noted that the Group Child Care (30 Months to School Age) program and the Group Child Care (School Age) program were mixing in the morning, exceeding the Section 34 (4) requirements. During an inspection it was observed that the Group Child Care (30 Month to School Age) program was mixing with the Group Child Care (School Age) program within the first 30 minutes of operation in the morning, and that there were more than 8 children total present. This allegation was substantiated.

Observation. During a site inspection, and review of a sampling of child records, it was noted that one care plan is incomplete. It notes a diagnosed condition, but no outline of "expected/anticipated" symptoms/behaviours, or how to address them.

Observation. During a site inspection and review of employee records it was noted that one employee does not have a Criminal Record Clearance letter on file at this time, although the completed application form was noted to be on file.

Observation. Due to the allegations, facility policies and procedures were reviewed during an on-site inspection. Through review of facility policies and procedures, as well as interviews conducted, it was determined that the Behaviour Guidance policy and the Nutrition policy require revisions/updates in order to guide employees in matters regarding care and supervision of children.

Observation. Allegations noted that children were being separated from the group to sit in the hallway as a behavioural guidance technique. This allegation was substantiated. Through interviews it was determined that employees have, or have observed, use(d) of the hallway next to the Group Child Care (30 Months to School Age) room as an area for children to sit quietly when unable to maintain a quiet demeanor in the care program area. This hallway is not licensed space, and is not to be used for care. Also, it was alleged that children were being directed to sit at the table for "table top activities" and not allowed to leave for the rest of the day, and being refused use of a toilet. Based on interviews conducted this allegation was not substantiated.

Observation. The complaint alleged that employees were using cell phones and not adequately supervising children. Upon review of the complaint, Licensing reviewed photographic evidence of employees on cell phones while caring for children during a fieldtrip. Being on a cell phone during work hours goes against the facility policy, and reduces the employees ability to adequately supervise children.

Observation. Reviewed attendance records. fire drill records, emergency preparedness plan records, outdoor inspection record, Minor Incident Log book, all appeared complete. Upon review of a sampling of child records it was noted that three children with a diagnosed condition (physical, behavioural, developmental or emotional) required care plans to be developed with consultation of parents and any persons the parents feel appropriate.
