Observation. The Licensee has not submitted required documentation to update the Manager information for this site. The name of the new Manager is posted in the the facility in a prominent location. The current Manager information must be kept up-to-date in Licensing records in order for the Licensing Officer to have correct contact information in the event an inspection must be scheduled, or there is a complaint or incident report to be followed-up. Ensure that the new Manager information is submitted to Licensing on or by April 24, 2026.

Observation. Upon review of a sampling of child records it is observed that some require Medical Practitioners' names and numbers. Medical Practitioners' names and numbers are required so that the facility can provide this to emergency services if necessary. If a family does not have a family doctor, then they can list the local Emergency room, Primary Care/Urgent Care, local Clinic or other medical services as appropriate. Ensure child records indicate the service parents would access if their child was ill or injured and required medical attention. Notify Licensing of review of records and updates to records as necessary by April 24, 2026.

Observation. Upon review of a sampling of child records it was noted that one did not have a phone number for the listed Medical Practitioner. This is required in case the medical practitioner needs to be contacted in an emergency situation. This was resolved during the inspection. Upon review of the Outdoor Inspection Log, it was noted that documentation of the play area inspections is inconsistent. This record is required in order to demonstrate that the Licensee is ensuring that the outdoor play area is safe for use, and that any repairs or equipment replacements/removals are conducted in a timely manner. Ensure corrective measures are submitted on or by April 10, 2025 as per the Director of Licensing Standard of Practice-Safe Play Spaces. The Minor Incident Log, Fire Drill Practice Log and Emergency Preparedness Practice Log were all determined to be complete, up-to-date and used consistently during inspection.

Observation. Upon inspection of the physical site it was noted that some posts in the play area have base boards with missing pieces which could cause an injury if a child fell on the edge that extends beyond the post. A corrective action plan outlining how this will be resolved and a timeline for implementation is required on or by April 10, 2025 as per Section 13 (2) of CCLR.

Observation. During inspection it was observed that the fridge temperature was measured at 6 degrees Celsius, above the maximum 4 degrees Celsius allowable. Observed that dish washing does not appear to meet the recommended process to ensure adequate cleaning and sanitization. Two posts' baseboards in the play area require replacement or repair as sections are missing. The protective covering on the tables used for eating and art are worn and the wood underneath is exposed. The wood appears to have indentations in it that may be difficult to clean and sanitize. Chairs are also not age appropriate size. One toilet is currently not available for regular use, there is a sign on the stall door stating that the toilet is out of order. The lock on the stall door is broken which does not support the dignity of the children in care, allowing them to have privacy while using the toilet.

Observation. Upon review of a sampling of employee records it was noted that immunization status statements were not present on file, including current status of COVID-19 vaccination level.
