A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. While on site, the following was observed: Inside: -Sanitizer and clarifying face cream accessible to the children in care (CORRECTED) -The side of the diaper changing mat that was being used was not made of a non porous material, the other side was made of a non porous material but had some rips. Outside: -Pruning sheers accessible to the children in care as stored on top of a fence. -Sharp piece of wood is sticking out from the fence post. -Patio glass top table stored on its side within outdoor play space, can act as a topple hazard.
Corrective action. Please provide a written response to indicate how you will address the contravention noted above on or before January 21, 2026
11010 - CCLR s.13(1) - RECORDS AND REPORTING
The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.
Observation. Care plan has not been dated and Licensee was unsure as to when it was made. Licensee noted that the parent had noted that the care plan was possibly no longer needed but no formal discussion/action was had about making amendments to the care plan. Additionally, the course of action was not clear as there is no documentation on what to do once medication was administered.
Corrective action. Please provide a written response to indicate how you will address the contravention noted above on or before January 21, 2026.

- EQUIPMENT AND FURNISHINGS
A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation. The following was observed: -Cleaners, medical ointment, hairspray, mouthwash and electrical toothbrush plugged into outlet were found at child level in the washroom area. -Vitamins in the cubby area at child level -Lawnmower, sledge hammer, jug of motor oil, oil pan, extension cord, and other various tools in the backyard were accessible to children.
Corrective action. - Items need to be removed and made inaccessible to children in care - A system in place to check for hazards both inside and outside the facility
11200 - CCLR s.17 - OR SUPERVISION
A licensee must do all of the following: (b) operate the community care facility in a manner that will promote (i) the health, safety and dignity of persons in care,
Observation. There were more than 4 children younger than 48 months old and more than 2 children younger than 12 months old from September until December 2024. From December 2024 until present there have been 5 children younger than 49 months.
Corrective action. Ensure facility has no more than 4 children younger than 48 months and of those 4 no more than 2 younger than 2 years old.

The licensee must not permit a person over the age of 12 to be ordinarily present on the premises of a community care facility while children are present, unless the person is of good character and the licensee has obtained a criminal record check for that person.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. LO observed and discussed the following with the Licensee. All below items were accessible to the children: - spray paint bottle (rust paint) was stored with other art supplies. - air freshener and various perfume spray bottles stored over the toilet shelf. - 3ft high wooden storage unit was placed on top of a table (2ft high); this storage unit was located right against a playpen used for napping -gardening hose and seasonal furniture were within the outdoor play area; posing tripping and fall hazard
Corrective action. Ensure that a healthy and safe environment is provided at all times while children are in care.
11010 - CCLR s.13(1) - EQUIPMENT AND FURNISHINGS
A licensee must supply equipment, furniture and supplies that are (a) of sturdy and safe construction, easy to clean and free from hazards.
Observation. LO observed and discussed the following with the Licensee: - Sleeping equipment,i.e. folded mats are used and stored within a ottoman with their beddings on them. All mats and beddings were touching each other. - Diaper changing mat: single fabric cover is used, not allowing to be sanitized/disinfected after each diaper change.
Corrective action. Ensure appropriate supplies that are easy to clean are provided to the children.

A licensee, other than a licensee who provides a care program described as either Preschool (30 Months to School Age) or Group Child Care (School Age), must have in a community care facility a sturdy surface for changing diapers, located outside the food preparation area, next to (a) a covered container for soiled clothing.
Observation. LO observed that diapering area had no garbage can with a cover.
Corrective action. Ensure that a covered container is available next to diapering area.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following in the inside play area: - The Licensee is using a cloth change pad cover. The licensee cannot disinfect the surface between each child's diaper change. In discussion with the licensee it was suggested to have 1 cloth change pad cover for each child or just use the plastic change pad surface. Licensing observed the following in the outdoor play area: - 2 carpets rolled up and propped up against a tree - 5' shelf unsecured. There are several light objects on it however the shelf is not sturdy and could fall down easily with light wind - Large dresser mirror with rust propped against the shed - Large clear garbage bag left on children's table - Rusted metal shovel propped up against a tree - 2x4 wooden plank propped up against the fence behind a tree
Corrective action. Ensure a healthy and safe environment.

Observation. Licensing observed the following in the indoor play area: - Paint chipped on the wall in the main play area - Paint chipped on the wall in the hallway - A clear plastic bag used to insulate the bathroom window Licensing observed the following in the outdoor play area: - The chain links for 2 of the swings are rusted - There is a rusted tricyle - Toy baby bath stand stored under the stairs is showing signs of wear and tear - 3 adult chairs with paint chipped and several areas of rust
Corrective action. Ensure all equipment, furniture and supplies are clean and in good repair.

Observation. A random selection of children's files were reviewed. Out of the 5 reviewed: - 4 files did not contain the sex of the child. - 1 file did not contain the child's immunization status

Observation. A random selection of chidren's files were reviewed. Out of the 5 reviewed, 2 did not contain information for a medical practioner.
Corrective action. Ensure a record showing the name and telephone number of a medical practioner is on file.

A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation. Licensing observed the following: -holes in the change mat exposing foam making it unsanitizable. -a single reusable hand towel next to the children sink. -tape around the fireplace is puckering and the glue is collecting dirt. -large play kitchens placed on a low fire place mantle posing a tipping hazard as children sleep next to the mantle. -2 adult chairs and a table in the backyard that have chipping paint and some rust. -A broken rubber maid container in the outdoor play space.
Corrective action. Ensure that a healthy and safe environment is provided at all times while the children are under the supervision of employees.
