Observation. Evidence that care plans were developed in consultation with the parent is missing from three children's plans. This is a reoccurring contravention originally found out of compliance on June 18, 2024. Care plans are to be developed in consultation with the parent and any other person as requested by the parent to ensure the health and safety of the child. Submit care plans for review by July 18, 2025. Two care plans on file have not been reviewed in the past year as required. Care plans are to be reviewed with the parent at minimum once each year to ensure the information is accurate and up to date for the health and safety of the child. Provide evidence by July 18, 2025 to confirm the care plans have been reviewed.

Observation. A climbing dome in the outdoor play area does not have a six foot fall clearance around it. A six foot clearance is considered safest to reduce the risk of injury in children. A response is required by July 21, 2025 that outlines how the risk has been mitigated.

Observation. Two employee files did not have evidence documenting employee ECE certification has been verified through the ECE Registry. A person employed as an educator or assistant who holds a certificate must be verified by the Licensee using the system established by the ECE Registry. Outline the system developed to ensure certificate validations are completed and documented as required.

Care plans are to be developed in consultation with the parent and any third party as requested by the parent to ensure transparency of the child's care. Observation. Some children's files were found to be missing required information including sex, immunization status, and a photo or digital image of the child. Registration forms include an opportunity for parents to add information about custody, but a record of persons not permitted access to a child encompasses a larger range of issues. A medication consent form was observed to have a medication name 'antibiotic.' The actual name of the medication is necessary on all medication consent forms.

Observation. The Manager said the facility had an Active Play and Screen Time policy; however it had not been included in the Parent Handbook Licensing inspected.

Observation. A random selection of employee files were checked, several employees did not hold a criminal record check completed by the Criminal Record Review Program.

Observation. Some of the wooden equipment in the outdoor play space is damaged and splintering. The diaper mat is split, exposing the foam interior, and making it difficult to adequately clean. Some movable equipment was placed within the "fall zone" of climbing equipment.
